Job Description:
- Provides enhanced patient care under the direction of the licensed nurse.
- Documents in the appropriate sections to create a complete and accurate medical record.
- Identifies, observes, and documents the physical needs of the patient according to established standards. Reports any changes in patient condition to licensed personnel in a timely manner.
- Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
- Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
- Provides one to one patient safety watch as assigned following established policies and procedures.
- Accurately processes specimens and provides timely transport of the specimen to the laboratory.
- Performs point of care patient testing and point of care instrument maintenance and quality control.
- Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
- Serves as a preceptor and mentor for new staff and students in a positive manner.
- Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard.
- Releases and confirms ordered tests to appropriate departments.
- Coordinate's transportation of patients to multiple tests.
- Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consult, etc.
- Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies.
- Communicates with food services to ensure appropriate patient diets.
- Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc.
- Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit.
- Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed.
- Assists with transporting blood products to and from Blood Bank as needed.
- Discharge follow up phone calls / coordinating virtual discharge appointment team.
- Support / Assist staff with 5 min Drills and/or Quality Rounds - participate in regulatory readiness as needed.

Position Details:
NICU Neonatal Intensive Care
Schedule: Three 12-hour shifts weekly including Day/Night hours (7:00am - 7:30pm/7:00pm-7:30am) and every other weekend and holiday requirements.
Required Orientation: At the start of employment, a two-week classroom orientation is required with a schedule of Monday - Friday, 7:00am - 3:30pm.
No experience needed. Comprehensive on the job training provided.

Where You Will Work:
The Medical/Surgical Unit is a 36-bed, patient-centered inpatient unit within a small community hospital, caring for adult patients with a wide range of acute and chronic medical conditions. Nurses on this unit provide comprehensive care for patients recovering from illness, surgery, infection, exacerbation of chronic disease, and other general medical needs. The unit also offers Virtual Critical Care support for IMC-level patients, helping nurses deliver safe, well-supported care for patients with higher acuity needs. With dedicated charge nurses, a visible, available, and approachable management team, and a strong team-oriented culture, this role offers the opportunity to work in a supportive environment where collaboration, mutual respect, and shared commitment to excellent patient care are part of everyday practice. Expectations We’re Looking For:
- Completion of an accredited or approved program in nursing

Where You Will Work:
6CD is a dynamic Medical-Surgical Telemetry unit designed to support patient flow and throughput within the hospital. As part of a Level 1 Trauma Center, the unit plays a critical role in decompressing the Emergency Department by facilitating timely admissions and discharges.
The team on 6CD provides comprehensive care to patients with a wide range of clinical diagnoses, including acute medical conditions, post-operative recovery, and telemetry monitoring needs. Staffed by experienced nurses and clinical professionals, the unit emphasizes comprehensive assessment, stabilization, and transition planning to ensure safe and efficient patient movement across the continuum of care.

Monroe Carell Jr. Children’s Hospital at Vanderbilt offers nationally recognized, multidisciplinary pediatric specialty care through one of the Southeast’s largest Pediatric Gastroenterology, Hepatology and Nutrition programs, comprehensive Pediatric Pulmonary Medicine services, Pediatric Nephrology and Hypertension, and a dedicated Complex Care Program. Our specialists provide advanced evaluation, treatment, and long-term management for children with digestive, liver, pancreas, kidney, urinary tract, and respiratory conditions, including complex disorders such as inflammatory bowel disease (IBD), celiac disease, cystic fibrosis, asthma, bronchopulmonary dysplasia, interstitial lung disease, and chronic kidney disease. With specialty clinics located across Middle Tennessee, families have access to expert, compassionate care close to home. 4 Tower is a Hematology Oncology and Oncology ICU at Carolinas Medical Center. We have 30 beds, 8 of which are ICU capable. We care for patients with Leukemia, Lymphoma, and Multiple Myeloma, during initial diagnosis, chemotherapy administration, neutropenic fevers, and complications that result in hospital admission. In our ICU beds, we care for any oncology patient that needs a higher level of care, hematology, solid tumor, and post stem cell transplants. Our patients tend to have multiple admissions for chemotherapy or stay inpatient for an extended period, as a result, the patients and their families build relationships and show appreciation for the care the nurses provide during this incredibly difficult time in their lives. 4 Tower is known for incredible teamwork, empathetic nursing staff, and opportunities to grow and learn from experienced nurses and providers (APPs and Physicians).

SUMMARY
The Physical Therapist reports directly to the Clinical Manager and/or Assistant Clinical Manager (ACM) of their respective department and may receive clinical guidance from senior staff and/or clinical specialists.
The Physical Therapist completes comprehensive examinations of body structure to determine functional impairments, activity limitations, and participation restrictions. The Physical Therapist establishes diagnosis and prognosis; develops an individualized plan of care; implements treatment interventions; and performs on-going evaluations throughout episode of care.

RESPONSIBILITIES

Principal Duties
Examines and evaluates patients
Establishes diagnosis, prognosis, and individualized plans of care
Implements treatment intervention based on plan of care, considering precautions, indications, and contradictions
Directs and coordinates patients’ rehabilitation plan of care
Evaluates patients’ progress and revises goals and plan of care accordingly
Educates/Trains patients/caregivers
Anticipates and communicates discharge/home needs and resources
Completes documentation and charges accurately and timely, in compliance with department policy
Completes clinical documentation reflecting eligibility/medical necessity of services in compliance with professional and regulatory standards
Makes recommendations and referrals for additional diagnostic testing and/or services
Consistently applies evidence-based practice to all aspects of clinical care
Seeks additional guidance when necessary to assure safe and effective patient care
Additional Duties
Complies with hospital and departmental policies and guidelines
Participates in strategic departmental quality improvement initiatives
Provides education to other hospital departments/personnel, students, and outside agencies/schools

MINIMUM QUALIFICATIONS
BASIC KNOWLEDGE
Bachelor’s degree in Physical Therapy from an accredited institution is required. Master or Doctorate level degree in Physical Therapy is preferred.
State license issued by the Rhode Island Department of Health is required.
EXPERIENCE
New graduates will be considered. Clinical experience in program specific settings is preferred.

SUPERVISORY RESPONSIBILITY
No direct supervision. However, pursuant to the state of Rhode Island Rules and Regulations for Licensing Physical Therapists, a licensed physical therapist shall at all times be responsible for the supervision of physical therapist assistants, supportive personnel, and students to ensure the safety and welfare of patients.
Provides consistent and thorough supervision of assistants, aides, therapists, and volunteers including cosigning notes, reviewing patient treatment plans and goals.
