At Houston Methodist, the Staff Pharmacist position is responsible for providing day-to-day comprehensive, patient-oriented, pharmacy services in any or all areas of the department which includes processing orders, therapy review, and education to patients and other members of the direct patient care team. This position collaborates with other health care professionals to manage patients' drug therapies for effectiveness and overall safety. The Staff Pharmacist position contributes to the fulfillment of the department's mission by collaborating to provide innovative, personalized, cost-effective pharmaceutical care in a culture dedicated to quality and safety. - Guides and mentors pharmacy technicians and students to help build confidence in skills, knowledge and abilities. In the absence of Senior Staff Pharmacists or other higher level, serves as a coordinator for the shift overseeing and leading activities of technicians and other support staff.
SERVICE ESSENTIAL FUNCTIONS
- Processes all orders/requests accurately and efficiently, verifying appropriateness of all drugs in reference to the patient’s diagnosis, allergies, height and weight.
- Monitors workload fluctuations and work output of pharmacy technician(s), adjusting assignments as appropriate to ensure accurate and timely delivery of medications to patient care areas.
- Conducts proactive drug therapy reviews and monitoring, providing drug information, patient counseling, and management of pharmacist-based dosing protocols, collaborating with other health care staff for problem resolution for the achievement of patient-specific goals.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Collaborates with other health care professionals to manage patient’s drug therapy for effectiveness, side effects, adverse drug reactions, toxicities, drug interactions, and incompatibilities following guidelines for formulary management and clinical intervention tools in the achievement of patient specific goals. Documents patient care activities and patient outcomes as impacted by pharmaceutical care and clinical interventions.
- Participates in department quality management activities, i.e. medication use evaluations, adverse drug reactions and medication error reporting, clinical interventions, medication center reviews, and inventory management documentation and reporting.

- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Vanderbilt Clarksville Intensive Care Unit (ICU), is a 21 bed mixed medical/surgical adult ICU specialized in providing close monitoring and treatment for patients with a wide-range of diagnosis. We care for patients being treated for Cardiovascular conditions, Acute Respiratory Failure, Acute Respiratory Distress Syndrome (ARDS), Multisystem Organ System Failure, Sepsis, Drug Overdoses, Gastrointestinal Bleeds, Diabetic Ketoacidosis, Hypertensive Crisis, Acute/Chronic Renal Failure, STEMI, cardiogenic shock and post heart catheterization.
Our staff is committed to provide exceptional care to our patients. Our environment is challenging, team oriented, patient and family centered. We work collaboratively to provide opportunities for nurses to grow in critical thinking, professionalism, and participate in ongoing education.
Our team consists of Intensivists, advanced practice nurses, critical care nurses, and respiratory therapists. Daily multidisciplinary rounds include the intensivist, advanced practice nurse, ICU nurse, case manager, respiratory therapy, pharmacy, infectious disease, and a nurse leader. SWAT team consists of intensivist, advance practice nurse and charge nurse available as bedside consult. ICU charge nurse responds to codes, and RRT’s. 3K Medical Observation is a 22-bed observation unit. Our patients are here for a variety of behavioral health reasons requiring hospitalization. On 3K we do cohort most of the children at Levine Children's Hospital with a mental/behavioral health diagnosis. Majority of these diagnosis include IV hydration, rule out sepsis, ortho observation, and surgical observation.

- Selects and applies special monitoring electrodes, bands, thermistors, and other monitoring probes to appropriate areas of the patient's body. Prepares positive airway pressure (PAP) mask fittings.
- Follows sleep procedures to establish and document baseline values such as body position, movements, oxyhemoglobin saturation, respiratory and heart rates.
- Assists with scoring sleep/wake stages and with the generation of accurate reports by tabulating sleep/wake and clinical event data such as respiratory events, cardiac events, limb movements, arousals, etc.
- Implements appropriate interventions including actions necessary for patient safety and therapeutic intervention such as continuous (CPAP) and bi-level (BIPAP) positive airway pressure and oxygen administration.
- Assist with procedures and therapeutic interventions such as auto servo ventilation (SV) titrations, average volume assured pressure support (AVAPS), multiple sleep latency testing (MSLT), maintenance of wakefulness testing (MWT).
- Performs polysomnographic data acquisition while monitoring study-tracing quality to recognize and eliminate all artifacts observed during the test.
- Follows wake procedures to verify integrity of collected data and complete the data collection process e.g. repeat the physiological and instrument calibrations and instructs the patient on completing questionnaires.
- Sets up video and audio monitoring apparatus. Keeps the equipment in clean operating condition, detecting instrument malfunctions, making minor adjustments, and reporting the need for major repairs. Monitors equipment supply levels and maintains appropriate equipment at hand.
- Schedules and discusses the test process with patients, ensures their knowledge of the process, and answers their questions. Educates physicians and patients on sleep medicines.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 30 days, and

POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. 6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. 8. In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

Position Summary
We’re seeking an RN to join our incredible MultiCare Behavioral Health team. The RN Outreach position works program that provides community-based, comprehensive, and individualized services. Requirements:
- Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)

Within this unit, highly skilled clinical professionals from various disciplines provide continuous, comprehensive care to patients following intricate neurosurgical procedures or facing life-threatening neurological disorders. We employ a patient-centered approach, offering extensive neurological services that support seamless continuity of care.
Patient-centered care in the Neuro ICU is achieved through effective teamwork, compassion, innovation, education, and research. Our nursing staff are proficient in operating diverse ICU equipment, such as tubes, lines, drains, invasive hemodynamic monitoring devices, and continuous renal replacement therapy (CRRT). $19.80 - $29.70Major Responsibilities:- Cleans furniture and carpets in rooms, hallways, stairs, and offices using power equipment. Cleans and maintains floor surfaces with dry and wet mops. Refinishes floors, including machine scrubbing and/or stripping floors and applies floor finish.
- Operates various types of cleaning equipment, both mechanical and electrical. Notifies appropriate person of safety hazards and of equipment needing repair. Cleans equipment, cart, and service closet daily. Assists in cleaning emergency spills as requested.
- Uses chemicals, cleaning products and equipment for their intended purpose to maintain a safe, cost-effective environment. Ensures all cleaning supplies are labeled and references Material Safety Data Sheets (MSDS) as necessary.
- Sets up rooms for meetings and delivers audio visual equipment to the rooms.
- Cleans patient care areas, public/private restrooms, conference rooms, lobbies, offices, elevators, and stairwells. Performs scheduled terminal and project cleaning in surgical areas.
- Cleans all environmental surfaces including ceilings and vents, walls, doors, furniture, beds, bathroom fixtures, and windows. Dusts and mops floors in patient and exam rooms, corridors, stairwells, restrooms, and other areas of the facility.
- Gathers and disposes recyclables, trash, and medical waste. Collects hazardous trash following established infection control guidelines.
- Sorts, stocks and delivers linens. Gathers and prepares soiled linen for transportation. Inventories linens, cleaning supplies and paper supplies and orders additional in a timely and cost effective manner. Fills and cleans supply dispensers. Replaces cubicle drapes and window coverings.
- Cleans and prepares dismissal and transfer units and certain equipment used in the care of patients and distributes non-medical supplies to specific areas.

- Must have ability to wear protective clothing as will have exposure to strong germicidal solutions, cleaning chemicals, temperature changes, soiled linen, infectious waste (blood and body fluids). Requires that incumbent be familiar with policies on handling infectious and hazardous waste.
- Must have ability to safely operate power cleaning equipment such as machine scrubbers or carpet cleaners as well as non-power cleaning equipment and ladders telescoping up to 20 feet.

- Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
- Checks in and registers patients; obtains and verifies complete demographic, guarantor, and insurance information; discusses and collects co-pays and other out-of-pocket patient responsibilities. Obtaining accurate information at the point of registration helps ensure timely payment to the organization and prevents billing issues and patient complaints. Maintains complete confidentiality regarding patient personal/financial information and medical records in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
- Knows insurance basics and recognizes commercial and government plans. Understands which plans Advocate Health contracts with and when a statement of financial responsibility is needed. Understands and discusses financial information and obligations with patients. Knows how and when to refer patients to Financial Advocates.
- Has knowledge of which rules, forms and questions must be enforced to make sure Advocate Health remains compliant with government agencies and regulations.
