The Carolinas Hospitalist Group is looking for an APP to join our Virtual Hospital Medicine Swing team supporting hospitals across the Greater Charlotte Region.
Position Details
Virtual Hospital Medicine APP position
Swing work schedule: 7 on / 7 off
Rotating weekends and holidays
Perform virtual admissions, inpatient rounding, and cross-coverage responsibilities
100% inpatient adult medicine
Utilizes Advocate Health’s virtual care platform to provide remote clinical support across multiple Greater Charlotte Region hospitals
Collaborative practice with Hospital Medicine physicians, bedside APPs, nursing, and interdisciplinary care teams
Epic electronic medical record
Required to live in one of the following states: North Carolina, South Carolina, Georgia or Alabama
Experience Required
Graduation from an accredited Nurse Practitioner or Physician Assistant program
Nurse Practitioners must hold a Master’s Degree or Doctorate in Nursing Practice with Acute Care Nurse Practitioner (ACNP or AGACNP) board certification
Physician Assistants must hold certification from the National Commission on Certification of Physician Assistants (NCCPA)
Current license to practice as a Nurse Practitioner or Physician Assistant in North Carolina
Current prescriptive privileges and DEA license
BLS for Healthcare Provider (AHA) certification must be valid at least 90 days beyond start date
Hospital Medicine, Internal Medicine, Critical Care, Emergency Medicine, or other acute inpatient experience preferred
Demonstrated ability to work in a fast-paced, high-acuity environment with strong critical thinking and interpersonal skills
Ability to perform thorough history and physical assessments
Ability to function effectively within a multidisciplinary team
Strong verbal and written communication skills
Experience utilizing Epic electronic medical record preferred
Job Summary
Specialty Care APPs provide concentrated services on a single system, caring for both acute and chronically ill patients. This Virtual Hospital Medicine position provides comprehensive inpatient care through virtual admissions, inpatient rounding, cross-coverage, and ongoing clinical management utilizing Advocate Health’s virtual care platform. The APP collaborates closely with Hospital Medicine physicians, bedside providers, nursing, and interdisciplinary teams to deliver safe, efficient, high-quality, patient-centered care across multiple Advocate Health hospitals.
Essential Functions
Perform comprehensive admission history and physical examinations for adult hospitalized patients.
Perform virtual admissions and formulate appropriate treatment plans.
Conduct virtual inpatient rounding and follow-up assessments.
Record findings of examinations, assessments, and plans of care.
Develop, implement, and modify treatment plans in collaboration with supervising physicians.
Order, interpret, and evaluate laboratory, radiology, and other diagnostic studies.
Write prescriptions and administer medications within the scope of practice.
Monitor patient progress and respond appropriately to changes in clinical condition.
Coordinate patient care with physicians, bedside APPs, nursing staff, consultants, pharmacy, rehabilitation services, case management, and ancillary departments.
Document all patient encounters, assessments, interventions, and clinical decisions accurately within the electronic medical record.
Act as a patient advocate for the population served.
Attend, participate in, and contribute to department/practice meetings, quality improvement initiatives, and policy development as requested.
Maintain compliance with Advocate Health policies, clinical practice guidelines, and regulatory requirements.
Perform other duties as assigned within the scope of licensure and credentialing.
Physical Requirements
Frequent sitting, computer use, speaking, hearing, reaching, and repetitive hand movements while utilizing virtual care technology.
Ability to work for extended periods using multiple computer monitors and virtual communication platforms.
Must possess intact senses of sight, hearing, smell, touch, and finger dexterity.
Must speak English fluently and in understandable terms.
Must be able to react and perform effectively in stressful clinical situations.
Schedule may include swing shifts, weekends, holidays, and additional coverage as operationally needed.

Minimum Job Requirements
Education
Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing services

Experience

- Coordinates development and revision of policies and protocols as related to ostomy and wound care management in collaboration with nursing and other disciplines and medical staff based on evidence-based practices.
- Serves as a consultant providing expert knowledge related to ostomy and wound care management support to physician, staff, patients, families, and instructors and students of nursing and allied health programs.
- Tracks and supports outcomes to improve the quality of care along the continuum.
- Evaluates and appropriately incorporates evidence-based findings in wound, ostomy, and continence care into clinical practice.
- Plans, implements, evaluates and provides direct patient care plans based on the individual patient and family needs, situational and procedural complexities.

Major Responsibilities:
- Uses the nursing process to assess the needs of the patient, plan and implement individualized interventions and evaluate the effectiveness of the plan of care. Includes the patient and family in planning and implementing care and develops and maintains a therapeutic relationship throughout the healthcare continuum.
- Anticipates and recognizes significant changes in the patient’s condition, status, and need for care. Provides ongoing assessment of the patient and takes appropriate actions when changes occur. Anticipates and alters the plan of care as needed, mobilizes appropriate resources, and collaborates with other health care team members to coordinate patient centered care.
- Implements strategies to reduce patient risk and increase patient safety. Assesses patient and family readiness to learn and individualizes the approach as necessary. Works collaboratively to develop strategies to meet the learning needs of the patient and family.
- Actively coordinates the discharge plan utilizing an interdisciplinary approach. Applies evidence-based practice to deliver patient care. Consistently incorporates nursing and regulatory standards of care into practice.
- Provides ongoing evaluation of patient, team, and unit outcomes and is empowered to take appropriate action when changes are needed. Supports shared governance activities and initiatives to improve processes and patient outcomes.
- Demonstrates effective communication, feedback, and conflict resolution skills. Promotes collaboration among healthcare team members. Assumes a leadership role in the care team and delegates appropriately to unlicensed personnel.
- Promotes a culture of safety through identifying threats to patient safety and intervening to prevent patient harm. Reports patient safety events and near misses in a timely manner. Seeks to identify potential safety issues and assists in the implementation of corrective actions.
- Applies ethical decision making, demonstrates respect and understanding for peers, and other clinical disciplines. Participates as an effective member of the patient care team to formulate an integrated, unbiased, individualized approach to care.

- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices, and
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
Education Required:
- Completion of an accredited or approved program in Nursing.
Experience Required:
- Typically requires 1 year of experience in clinical nursing.

- Demonstrates progression to provide holistic patient care and the ability to view clinical situations from a patient and family focus.

Physical Requirements and Working Conditions:
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to:
- lift up to 50 lbs. from floor to waist.
- lift up to 20 lbs. over the head.
- carry up to 40 lbs. a reasonable distance.
- Must be able to:
- push/pull with 30 lbs. of force.
- perform a sliding transfer of 150 lbs. with a second person present.
- Position requires use of foot pedals on carts or machines.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Operates all equipment necessary to perform the job.
