SERVICE ESSENTIAL FUNCTIONS
- Follows the patient and family-centered care standards and provides direct patient care under the direction of a registered nurse. Documents, where applicable, care administered, diagnostic measurements, treatments and procedures in accordance with established policies and procedures. Responds to the call light system, contacting nursing personnel as appropriate, and follows through with meeting patient needs.
- Assists with maintaining, cleaning, and stocking of equipment and supplies as established on assigned unit. May serve as an in-house courier which may include retrieving blood, hand delivering labs, tele boxes, etc. Organizes the workflow, problem-solves basic and routine matters, and manages multiple ongoing priorities, seeking guidance and assistance from preceptor, licensed nurse, or management.
- Contributes towards improving department scores for patient satisfaction through peer-to-peer accountability, i.e., bedside shift handoff/report.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Reports observations, conditions and problems of patients to licensed personnel to achieve desired patient outcomes. Provides for the privacy of patients and families, keeping the safety of the patient in mind.
- Provides appropriate hand-off at change of shift at patient bedside to communicate and collaborate, promoting patient-centered care.
- Conducts hourly rounding to ensure patient needs are met (four P’s). Reports near misses and collaborates with the interprofessional health care team to improve patient safety.
- Contributes towards improving quality and safety scores, through peer-to-peer accountability. Supports initiatives to prevent conditions such as pressure injuries, patient falls, and hospital-acquired infections.

- Assists with patient and staffing needs (floats) across the service line or hospital as competencies allow. Utilizes time between heavy workloads efficiently.
- Utilizes resources with cost effectiveness and value creation in mind. Self-motivated to independently manage time effectively and prioritize daily tasks, minimizing incidental overtime, assisting coworkers as needed.
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
- Offers innovative solutions in performance improvement projects and shared governance activities.
- Seeks opportunities to identify self-development needs on knowledge base, skill level and decision-making, as necessary, especially in areas of question, from preceptor and assigned licensed personnel and takes appropriate action. Job Summary

Minimum Qualifications
Education: Bachelors or graduate degree in Physical Therapy from an accredited university-based Physical Therapy program
Licenses/Certifications:
Current license to practice Physical Therapy by Texas Board of Physical Therapy Examiners or by the Physical Therapy Compact Commission.
Current Certification in Basic Life Support.
A physical therapist practicing direct access:
Education: Doctoral degree along with a residency or fellowship program, OR
Licensed therapist with 30 hours of continuing competence activities in differential diagnosis
Licenses/Certifications:
Current license to practice Physical Therapy by Texas Board of Physical Therapy Examiners or by the Physical Therapy Compact Commission.
Experience / Knowledge / Skills:
At least two (2) years clinical experience required or completion of an accredited residency program
The position requires occasional/frequent lifting of up to 75/100 pounds and carrying of up to 30 pounds; prolonged standing; frequent walking, stooping and squatting
Fine hand dexterity, eye-hand coordination, the ability to receive oral and written communication, and to communicate verbally and in writing are also necessary
The visual ability to monitor instruments, computers and equipment, recognize alphanumeric characters, distinguish colors and observe physical characteristics are essential to this position
Principal Accountabilities
Assumes responsibility in the care of the patient. Performs complete, comprehensive evaluation of patients. Accurately interprets evaluation findings. Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
Implements a physical therapy treatment program and provides advanced physical therapy treatments. Demonstrates competency in performing advanced physical therapy skills. Responsible for patient care performed by assistants and rehabilitation technicians. Assesses patients regularly to monitor progress towards treatment goals, and alters goals, plan or program as indicated. Motivates and instructs patient/caregiver using appropriate methods.
Communicates effectively with patients/caregivers, professional colleagues to include physicians and insurance companies.
Manages time effectively.
Documents appropriately in the medical record and completes other documentation duties accurately and efficiently.
Serve as a clinical resource, mentor, and clinical instructor to staff, students, hospital, schools and/or the community in general.
Actively pursues clinical expertise in specific treatment techniques or management of a particular patient population.
Actively participates in organized system/facility/departmental committees and taskforce.
Presents relevant special topics in professional literature at clinic meetings, grand rounds, conferences, university settings and meetings as delegated or requested by the faculty.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

POSITION PURPOSE
The Patient Support Technician functions as a collaborative member of the health care team in partnership with and under supervision of the Registered Nurse utilizing principles of Relationship-Based Care, performs direct patient care to a specific group of patients and coordinates activities which relate to the flow of unit processes. Actively supports the activities of the unit based shared governance council. Promotes organizational commitment and excellence in patient care experience.
ESSENTIAL FUNCTIONS
Demonstrates principles of Relationship-based Care to cultivate and maintain therapeutic relationships as a means to optimal healing and collegial functioning as an interdisciplinary care team member.
Provides patient care that reflects respect for patients' rights, dignity, values, culture preferences, and expressed needs and attention to providing comfort, relieving pain, and alleviating anxiety.
Provides basic bedside care and selected procedures under the direction and supervision of the Registered Nurse. Including but not limited to: venipuncture, 12 lead EKG, basic blood glucose collection and monitoring, respiratory care (i.e. Cough and deep breathing, incentive spirometry, pulse oximetry, oxygen setup, hand held nebulizer), dressing changes, collection of vital signs, recognition of life-threatening conditions with initial responses and reporting, emptying and measuring drainage equipment.
Performs activities of daily living (ADL) appropriate to the patient's condition under the direction of and supervision of the Registered Nurse.
Initiates and maintains patient medical records.
Manages communication for the unit including placing and receiving calls from physicians, other departments, and visitors.
Maintains an accurate patient roster/census and available ready beds.
Assists with the provision of patient education under the direction of the Registered Nurse.
Integrates principles of the Personal.Connected.Journey and demonstrates Trinity Health Guiding Behaviors to provide excellent care of self, patients, and care team members.
Performs basic nutritional support to patients such as bed set up, assistance with feeding, and collection and documentation of intake and calorie count as appropriate.
Takes personal accountability to seek out experiences in order to maintain skills and competence.
Seeks and incorporates feedback concerning improvement in one's own practice, including participation in self-appraisal.
Assists with the admission process by collecting selected assessment data, applying necessary equipment, labeling patient's clothes and belongings, informing patient/family of unit routines, etc.
Provides individualized care consistent with the plan of care under the direction and supervision of the Registered Nurse.
Actively engages and participates in unit-based and other committees and indicatives that promote patient and workplace safety, optimal patient outcomes, and satisfaction.
Reports changes in patient condition to the Registered Nurse in a timely manner.
Demonstrates proper body mechanics in performing role.
Performs other various duties as assigned and needed. These duties may vary depending upon the unit, patient type and need, number and complexity of treatment provided. Additional duties may also include nonclinical duties consistent with meeting the needs of the patient and care team.

In accordance with unit or departmental practice, assists with tracking and monitoring of equipment assigned to the unit. Requests services for maintaining equipment as needed.
In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment, and care management, performing these activities concurrently with provision of care throughout the shift.
MINIMUM QUALIFICATIONS
Experience: Preferably at least one of the following:
6 months direct patient care experience within past 5 years.
Completion of one Medical/Surgical clinical rotation in a nursing educational program.
Completion of a "Patient Care Technician" program that requires successful completion of an externship in an acute care facility.
Previous Health Unit Coordinator/Unit Secretary working experience.
Education: Graduation from an accredited high school or GED program or equivalent.
Completion of one of the following preferred:
1. Certification program which requires a Medical/Surgical rotation.
2. Emergency Medical Technician (EMT).
3. Equivalent work experience as defined in Section D.
Licensure/Certification:
If EMT - current licensure in State of Michigan.
Current Basic Life Support (BLS) certification through American Heart Association.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
Maintains appropriate skill level through completion of required hospital and unit based education offerings.
In a typical day, the colleague may sit for 25%, stand for 25%, and walk for 50% of the day.
Working in the patient care environment may result in exposure to communicable diseases and hazardous substances.
Working weekends and holidays is necessary to meet staffing needs. Work in other units and shifts may be assigned as necessary to also meet staffing requirements.
Associate's position requires occasional (5-10% of work time) squatting, crawling, crouching, kneeling, and balancing, frequent bending/stooping, and reaching above shoulder level, pushing/pulling.
Position requires the use of hands for simple and firm grasping and fine manipulating.
Position may require frequent lifting and carrying of patients/equipment other not to exceed 51 pounds. Associates will not lift items/patients exceeding 35 pounds from the floor.

Major Responsibilities:
- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team.As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.
The Patient Care Tech III (PCT III) brings an additional skill set to the table. PCTs that are performing unit secretary duties, telemetry monitoring, remote video monitoring, or are working in the Step-Down Unit, ED, Dialysis, Hospital at Home, or ICU are eligible for this level.

I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning, and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.
7. Takes and records vital signs (temperature, pulse, respirations, blood pressure and pulse oxymetry). Measures and records patients’ height and weight.
8. Performs EKG’s, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels, and prepares specimens for transport to lab.
9. Phlebotomy in certain locations.
10. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
11. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
12. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
13. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
14. Answers telephone using appropriate telephone etiquette. Takes and relays messages. Utilizes intra-department communication systems, such as pagers and intercom.
15. Replenishes patient and unit supplies and equipment.
16. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
The PCT III must fit into one of these specialties:
Unit Secretary
Functions as unit secretary assisting with patient admissions, transfers, and discharge procedures at least 5 shifts (40 hours) every 6 months. Has administrative responsibilities including, but not limited to, maintaining logs, organizing patient care charts for admission/discharge/transfer and maintenance of medical records. Enters data or retrieves medical information from the electronic medical record systems.
Remote Video Monitoring
Continuously observes multiple patients utilizing the remote video monitoring system.
Emergency Department, Step Down Unit, ICU, Dialysis, and Hospital at Home.
Telemetry Certified & Step-Down Unit - Harrington
PCT IIIs are cross trained in phlebotomy and telemetry competencies, as well as the unit clerk position. PCT IIIs will also be assigned to areas of higher acuity, such as the Step-Down Unit.

Summary:
Assists licensed anesthesia provider with duties related to the care and monitoring of equipment.
Responsibilities:
Assists anesthesia provider in the preparation of supplies and maintenance of equipment and anesthesia delivery systems before/during/after anesthesia.Assists Anesthesiologist with the placement of the A-line / Central-line and maintaining sterility of the room.Cleans and sets up all types of anesthesia equipment.Floats to other departments as required, e.g., L&D, Cath Lab, MRI, etc., in performing the duties described herein.Maintains a sufficient inventory of all equipment and supplies, ensuring stock is rotated to prevent expired supplies and to insure all equipment is functional.Performs other related duties as may be assigned.Processes laryngoscopes, bronchoscopes, and glide scopes in accordance with manufacturers' recommendations.Provides daily maintenance of all equipment and troubleshoots all equipment malfunctions including, but not limited to: monitors, anesthesia machines, ultrasound equipment. - Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Summary:
The RN Case Manager coordinates the care and service of patient populations from admission through discharge. Position Purpose
Functions as a member of the health care team under the direction of the RN. Utilizing a Patient Focused Care approach, provides sitting services to a specific group of patients within the St. Joseph Mercy Oakland System. - In accordance with unit or departmental practice, determines that appropriate charges have been entered for the correct patient, encounter, date/time of service, with any required modifiers.
- Make corrections as needed per charge capture policy/practice.
- In accordance with unit or departmental practice, actively participates in on-going education and communication regarding revenue management.
- In accordance with unit or departmental practice, assists with tracking and monitoring of equipment assigned to the unit. Requests services for maintaining equipment as needed.
- In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment, and care management, performing these activities concurrently with provision of care throughout the shift
Minimum Qualifications
- . Education • High school diploma, GED or equivalent.
Working Conditions
- Working in the patient care environment may result in exposure to communicable diseases and hazardous substances.
- Position may require frequent lifting and carrying of patients/equipment/other not to exceed 51 pounds. Associates will not lift items/patients exceeding 35 pounds from the floor.
- Associate’s position requires occasional (5-10% of work time) squatting, crawling, crouching, kneeling, and balancing, frequent bending/stooping, and reaching above shoulder level, pushing/pulling.
- May be working in an environment dimly lit, sitting for periods up to 100% of shift.
- Position requires the use of hands for simple and firm grasping and fine manipulating
