Summary:
Determines and verifies insurance coverage and coordination of insurance benefits from all sources. Requests and/or verifies authorizations for ordered services. Schedule and/or reschedule physician orders for outpatient treatments and/or diagnostic services. Discuss financial responsibilities for upcoming appointments with patient and/or guarantor. Collect upfront payments and/or review for financial assistance. Handles inbound and/or outbound calls related to the above functions.
Responsibilities:
Inputs and/or reviews insurance benefits for the patient account. Re-runs estimates when needed to correct expected patient out of pocket liability for that service.Updates patient insurance information into the patient record as needed to ensure correct billing and net revenue estimation occurs.Confirms insurance coverage. Determines necessity for pre-authorization and obtains authorization for that service(s). Documents authorization information within the patient account.Reviews order for appropriate authorizing signature and accuracy of scheduled service(s).Communicates with patients and practitioners regarding order, insurance and/or authorization issues.Notifies patient of their financial responsibility for services to be provided. Educates guarantor about their out of pocket responsibility for the service and collects pre-service payments in accordance with available financial options. Ensures compliance with departmental procedures for payment. Document details of the call and notate any collections that need to be performed at time of registration.Handles inbound and/or outbound calls to perform all pre-service functions included but not limited to scheduling, authorizations, benefits, and patient responsibilities. Documents appropriate record with the details of the call and the call disposition.

Key Responsibilities

- Develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence and of mental health and/or substance abuse problems in patients and family members.
- Completes a full assessment based on the social work assessment, identifying social determinants of health. Completes screening by patient/family interview, review of the medical record including previous episodes of care, H&P, lab and other test results/findings, plan of care, physician orders, nursing and progress notes. Uses clinical knowledge and screening tools to identify need for case management and/or social work intervention.
- Establishes mutual educational goals with patient and family, provides appropriate resources, incorporating planning for care after discharge.
- Uses knowledge of levels of care, working with patient and family, to ensure discharge disposition is to the appropriate level and facilitates transfers. Provides brief, goal-directed counseling services to assist patients/families to cope more effectively with the transition.

- Modifies care based on continuous evaluation of the patient’s condition, using problem-solving and critical thinking, and makes decisions using evidence-based analytical approach. Documentation reflects completed patient screening/assessment and reassessment upon admission and concurrently, as needed. - Continuously reviews the patient for opportunities for care facilitation and needs for discharge planning. - Collaborates with staff from the interprofessional health care team concerning safety data to improve outcomes and the safe transition of care through effective patient handoffs

- Completes timely and thorough assessment on all unfunded patients to identify community resources required for effective transition and able to utilize alternative resources to fill gaps in established community resources.

- Provides timely, efficient pertinent patient information to healthcare team when coordination with significant or intensive resources is necessary for effective discharge planning outcomes and decreased length of stay.

Aurora - Burlington Medical Center located in Burlington WI is looking to fill a class with students that would transition into employment as a CNA at our Burlington Medical Center.
Training is held onsite at Aurora-Summit in Oconomowoc (36500 Aurora Drive, Summit, WI 53066) with the exception of a practice day and testing date that is held near our St. Luke's campus in Milwaukee.
Aurora Health’s CNA training program is a Wisconsin approved program for education and training to be prepared for Wisconsin Certified Nursing Assistant Certification Exam. In order to meet the requirements for the state certification exam all 88 hours must be completed. PERFECT ATTENDANCE IS REQUIRED (including being on time for every class.)Â
Compensation: You will be paid $19.80/hr during class and clinicals. Once you pass your CNA exam and transition into the CNA role, you will receive an increase in pay to $20.80/hr.
Benefits: You will become benefit eligible once you are placed into a CNA position. Â
Once the CNA training class is completed, you will be assigned position as a CNA at Aurora-Burlington. You will want to ensure that the medical center in Burlington is a long term fit as we will not make exceptions to our transfer guidelines to other sites.
Major Responsibilities
Adheres to syllabus rules, attends all 88 hours of training, completes all classroom assignments and passes final exam with at least a 75%.

Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
Measures and documents TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR) with instructor support.
Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately with instructors support.
Collects urine, stool, and sputum specimens as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection and assistance in eating.
Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to observe data reflective of the patient's status and report the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

The Behavioral Health Pavilion is also the largest inpatient psychiatric service in Southern Arizona. Our team provides patients a safe, supportive environment that fosters healing and facilitates a return to the highest possible level of independence. Other services include; crisis stabilization, psychiatric assessments, 24-hours psychiatric & nursing care, expressive arts therapy, group therapy, family support therapy, medication management, and a separate behavioral health unit within the Emergency Department. You can become an integral member of our patient's team, along with psychiatric doctors, NPs, pharmacists, Registered Nurses and security officers.
As a Behavioral Health Specialist on the Geriatric Psych Unit, you will assist with our geriatric patient population who often have high acuity medical needs on top of their behavioral health needs. Many of these patients require total hands on patient care and are often unable to communicate their needs on their own. On this team, you will spend your days assisting with basic daily living activities, making BHS assignments for the floor and DPO's, supporting your fellow BHSs and RNs on the unit, communicating with Charge RN, hand hygiene audits, delegate patient admission and/or ensure discharge process is completed (belongings, admissions folder, etc), and that Direct Observer/Q15 documents are completed throughout each shift. Candidates with a total patient care experience along with a passion for the elderly population will be preferred on this unit.
Floating to the other 2 Behavioral Health units on this campus may be required (determined daily by census numbers).
This position will be for night shift. The hours will be from 7PM-7:30AM which will include a set rotation for weekends and holidays. You will have the opportunity to utilize our self-scheduling tool as well as enjoy an additional $1/hour Weekend Shift differential, when applicable.

We are an Arizona Department of Health Services-accredited Cardiac Receiving Center and a Nurses Improving Care for Health system Elders-designated senior-friendly hospital. The hospital is staffed by physicians who are full-time faculty of the University of Arizona College of Medicine - Tucson and is managed by Banner Health under an operating agreement with Pima County. Our specialty services include inpatient and outpatient behavioral health, treatment and education for diabetes, innovative geriatrics care and comprehensive orthopedics.POSITION SUMMARY
This position provides direct care to psychiatric patients working under the direction of a registered nurse.
CORE FUNCTIONS
1. Provides basic nursing care to patients including technical monitoring, taking vital signs and physical care. Orients patients to treatment setting and assists with activities of daily living as needed. Reviews and participates in implementation of the patient treatment plan. Makes rounds and monitors patients activities, physical and mental status and reports to nursing staff. Checks patient belongings for contraband. Conducts a variety of patient safety monitoring activities such as observational checks. Documents observations, interventions, and group activities in the patient medical record while maintaining confidentiality of all health information.
2. Assists in maintaining a safe, orderly & therapeutic environment. Maintains patient safety and protects patient rights through observation and reporting patient behaviors, communications, interactions and physical status. Recognizes patients who are behaving aggressively, acting as a danger to themselves or others, or are in a crisis or medical emergency situation. Provides intervention including, but not limited to, redirecting of patient and/or participating in restraint or seclusion as needed.
3. Assesses and interacts therapeutically with patients and families. Provides treatment that promotes patient dignity, independence, individuality, strengths, privacy, and choice. Demonstrates respect for cultural diversity and patient boundaries. Plans and facilitates social and didactic group activities. May participate in other rehabilitation and treatment programs.
4. Provides quality patient care by maintaining knowledge and skill level related to the symptoms and treatment of behavioral disorders relevant to position responsibilities and in accordance with state agency regulations. Maintains knowledge to identify types of medications commonly prescribed for patient group and the common side effects and adverse reactions of the medications.
5. Assists in the admission, discharge and transfer process of patients and completes required documentation in the patient medical record.
6. Position completes work under moderate supervision. Position interacts with all levels of clinical staff on assigned unit/program and members of the multidisciplinary teams, as well as patients, families, and medical and psychiatric physicians.
MINIMUM QUALIFICATIONS
Requires a Bachelor’s Degree in a behavioral health related field OR High School diploma or equivalent with 1 year of direct patient care in an inpatient behavioral health setting.
This should include but is not limited to experience in an inpatient psychiatric facility; assisting with personal grooming and other activities of daily living; conducting educational, recreational and skills groups appropriate to current patient population; creating, maintaining, and encouraging an inpatient therapeutic milieu; maintaining observation precautions and applying non-violent de-escalation interventions in crisis situations; recognizing the common symptoms of mental illness such as depression, psychosis, mood dysregulation, substance abuse, maladaptive behavior, and anxiety; understanding how symptoms are related the patient’s mental illness diagnosis and providing ordered treatment interventions appropriate to the diagnosis. Such experience is commonly achieved in care of patients in psychiatric acute care hospitals, inpatient psychiatric facilities and State hospital programs. Some outpatient programs, therapeutic group home and residential programs may meet the experience criteria. Concentration on skills and experiences demonstrating competence in building therapeutic rapport, developing collaborative relationships with the multidisciplinary team, de-escalation techniques and limit setting.

Position Purpose:
According to established proceÂdures, performs repetitive cleaning tasks in order to maintain assigned Hospital area in an orderly and sanitary condiÂtion. In doing so, moves furniture, equipment and supplies in and around Hospital departments, and performs a variety of heavy and light housekeeping duties. Utilizes Seven-Step cleaning, dry and wet mops floors, strips and buffs floors, shampoos carpets and removes stains from floors, walls, furniÂture, and the like.Â
What you will do:
Receives cleaning assignments, requests cleaning materiÂals and supplies such as mops, buckets, electric buffer, vacuum cleaner, rags, disinfecÂtants, and the like from designated areas. Mixes cleaning soluÂtions, loads service cart and transports same to work areas
Cleans assigned areas by washing furnishings, tile, fixtures and equipment with germicidal cleaning solutions. Scours and polishes sinks, tubs, toilets, mirrors, and the like.
Upon dismissÂal of patients, strips, cleans and makes beds with fresh linens, cleans other furniÂture and disposes of refuse by emptying wastebaskets. Restocks supplies of soap, tissues, towels, and other disposable items.
Sweeps and mops limited floor areas such as patient rooms and Hospital offices. Vacuums carpets using light vacuum, spot cleans carpets using appropriate solutions and arranges furniture and equipment in an orderly manner after cleaning assigned areas. Dry and wet mops large areas of floors, and operates buffing equipment.
Cleans and performs routine maintenance on sanitation equipment and notifies supervisor of equipment needing further repair.
Visually inspects area upon completion of cleaning to ensure appropriate levels of cleanliness and neatness have been achieved.
Returns unused supplies to utility room, cleans carts and equipment and ensures items are stored in an orderly manner.
Performs heavy‑duty cleaning of floors and carpets. In doing so, removes soil and finish of floors by using mechanical stripping equipment and shampoos carpeted floors by operating mechanical shampooing equipment.
Washes, scrubs and disinfects walls, windows and doors and, when necessary, determines method of removing stains using a variety of cleaning equipment and supplies.
Gathers refuse from various Hospital locations and transÂports same in hand pushed cart to compactor or holding area. Following established sanitation procedures segreÂgates bagged refuse and loads into compactor or holding area.
Moves furniture and equipment (such as mattresses, beds, and cribs) by use of handcarts or dollies.
Working Conditions:
Requires working with equipment or performing procedures where carelessness would probably result in minor cuts, bruises or muscle pulls.

Minimum Qualifications:

Patient Care Assistant, PRN (Variable Hours), Pediatrics, Bronson Methodist HospitalUnder the supervision of a Registered Nurse, and in accordance with defined policies and procedures for the unit, the Patient Care Assistant (PCA) provides technical patient care to a defined patient population as assigned by the Registered Nurse. Duties may include the performance of basic technical skills not requiring licensure, such as, but not limited to: gathering patient data, assisting patients with activities of daily living (ADLs), transporting patients, supporting department maintenance activities (stocking patient rooms, moving equipment, ordering and procuring supplies, etc.), providing basic customer services (directions, visitor control, and other functions as needed). Employees providing direct patient care must demonstrate competencies specific to the population served. .
Orientation Requirement
Full-Time, Part-Time, and PRN team members must be able to commit to full-time orientation hours during the first 6–8 weeks of employment.
Orientation consists of three 12-hour day shifts (7:00 a.m. – 7:30 p.m.) each week.

The PCA is responsible for completing the Word day CBL –“Swing beds”. This is an annual CBL that describes the difference in a swing bed patient versus an in-patient or observation patient. The PCA will understand the documentation differences and the specific care required for each individual swing bed patient.

Education
Graduate of a Board of Nursing approved nursing education program.Â
Certification / Registration / LicenseÂ
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.Â

Summary:
