CPMC-California Pacific Med Center DaviesPosition Overview:
Integrates advanced knowledge, theory and practice to serve as a subject matter expert in assigned areas of the laboratory. With advanced training and education, maintains competence in all practices to perform pre-analytic, analytic, and post-analytic procedures on biological specimens to aid health care providers in the diagnosis, treatment, monitoring and prevention of disease. Participates in, and coordiantes all aspects of, the daily operation of the assigned laboratory areas. Monitors, assesses, organizes and coordinates the daily activities of co-workers to ensure a smooth and continuous workflow overall, or within an assigned area. Duties routinely include staff scheduling, arranging coverage for staff absences, assigning work, prioritizing work, directing problem-solving and trouble-shooting, ensuring shift-to-shift continuity, and managing customer complaints or inquiries. Leads the development and implementation of new policies, procedures and protocols to streamline workflow, improve efficiency, and enhance the safety and reliability of testing; trains and assesses the competency of co-workers; provides guidance and direction to staff, enabling the accurate, timely and efficient completion of instrument maintenance, quality control, non-conforming even event management and records management; provides constructive feedback and recognition to peers, reinforcing Sutter's values and focusing the team's efforts on providing safe, reliable care; and serves as the liaison between laboratory leadership and staff, physicians, patients, family and other hospital staff, leading communication huddles and other structured team meetings (e.g. Daily Engagement).Job Description:
EDUCATION:
Bachelor's: Biological sciences, allied health science or related field from a regionally accredited college/university
Other: Successful completion of a NAACLS accredited Pathologists’ Assistant program
CERTIFICATION & LICENSURE:
ASCP-American Society for Clinical Pathologist
TYPICAL EXPERIENCE:
8 years of recent relevant experience.
SKILLS AND KNOWLEDGE:
Knowledge/Skills: Demonstrated subject matter expertise and superior technical competence in gross and post-mortem examinations, laboratory operations, and quality assurance in surgical pathology.
Quantity of Work: Ability to organize and prioritize job duties and assigned tasks for self and team to insure completion within expected time-frames.
Quality of Work: Ability to lead team to accurately, precisely, and reliably perform job duties and assigned tasks. Demonstrates sound judgment and problem solving relevant to own and team's assigned duties.
Culturally Competent Communication: Ability to compassionately and effectively interact with individuals and groups of diverse ages, backgrounds, values, beliefs and behaviors.
Workflow Management: Ability to coordinate team's efforts to appropriately respond to fluctuations in volume, unexpected situations or problems such as equipment or information system failures, shift-to-shift hand-offs, priority test orders, and specimen integrity issues.
Adherence to Practices & Procedures: Ability to direct team to follow standard practices, processes, and procedures in sequence. Knowledge of applicable local/state/federal regulations, codes, policies, and procedures aimed at ensuring the privacy and safety of patients.

Summary:
Ensures documentation, data capture and opportunities for utilization are compliant, accurate and consistent.
Responsibilities:
Works in collaboration with the Medical Director of Centralized UR/HIM, utilization committees and medical staff to maintain and improve utilization and documentation efforts.Makes decisions on referred individual patient cases regarding: Pre-admission, authorization, Medical necessity and services/setting, transfers, appropriateness of admission, and continued stay. Conducts peer to peer discussions with health plans when required.Serves as a resource to the medical staff for Utilization Review activities. Educates and communicates to the Medical staff the benefits/need for utilization review and documentation.Performs regular, follow-up and focused chart audits/reviews (concurrent and retrospective) for the purpose of assessing the accuracy of clinical documentation and utilization. Ability to work weekends and holidays as needed.

Under the direction of the appropriate licensed clinician, the Patient Care Technician I performs a variety of patient care activities. Receives workplace education and training to assist members of the healthcare team in the care of patients.I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning, and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.

7. Assures the accurate recording of the electrical impulses of the neurological system by:

Interacts on a daily basis with the Lead E.S. Worker, other cleaning service personnel when applicable, maintenance and repair personnel, as well as persons served, guests, and staff. ESSENTIAL FUNCTIONS:
Follows established work schedule in cleaning and maintaining assigned internal areas of responsibility (public areas and hallways, program areas, residential areas, etc.). For example, cleans public restrooms daily, cleans dental clinic two times weekly, etc.
Follows established work schedule in cleaning and maintaining assigned external premises/grounds (front entrance area, back entrance area by dining room, parking lot areas, fence lines, etc.) of the facility.
When and where necessary/appropriate, cleans and maintains carpets and flooring, including tasks such as stripping/waxing floors, vacuuming/wet-vacuuming carpets, and spot cleaning soiled areas.
Maintains a constant awareness of the aesthetic values of the facility and contributes to the enhancement and improvement of these values by insuring clean and aseptic conditions throughout House of Mercy by scouring, dust mopping, high dusting, wet mopping, damp dusting, removing trash, washing walls and ceilings.
Regularly checks supply inventory and generates purchase orders when/where needed.
Picks up and returns keys, pager (when applicable) at the beginning and end of each work shift.
Promptly reports existing furniture needs, as well as equipment, maintenance and building issues to the E.S. Lead Worker and/or Director, completing work orders when/where appropriate.
Performs general cleaning services throughout the House of Mercy when/where necessary without having to be prompted or asked.
In absence of Lead E.S. worker and/or any contracted cleaning service staff, takes initiative in identifying soiled areas and promptly following through with necessary cleaning.
Assists with resident admissions/discharges, insofar as cleaning, shampooing carpets, painting walls, making other minor repairs as needed, and ensuring adequate bedding is available in rooms.
Assists with painting and maintenance of building’s interior walls.
Demonstrates initiative in addressing changing/unscheduled environmental service issues where and when they arise.
Maintains both high standards of courtesy and cooperation in dealing with co-workers, persons served and guests.
Assists in completing safety checklists and conducting monthly safety drills (i.e. fire and tornado).
Participates in House of Mercy’s Performance Quality Improvement Process as appropriate.
Assists Lead E.S. Worker in maintaining up-to-date Safety Material Data Sheets (MSDS) of chemicals used in the building in the MSDS book kept in the front office.
Promptly reports/seeks assistance from clinical staff when encountering inappropriate behavior from persons served, guests, visitors, etc. to ensure a safe environment.

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated. Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. - Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately. Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices.
- Performs tasks related to nutritional needs of the patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection. Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. - Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required. Under direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care and perform bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system.

- Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. This role requires travel between multiple sites within a single shift (up to 2 sites per shift) to support staffing needs across the region. Please note that mileage reimbursement is not provided; however, a $6/hour float differential is offered to help offset travel-related expenses.
Pay Range:
$20.80 - $31.20Major Responsibilities:
- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and
- Nurse Aide in good standing as indicated with the State Registry in which the team member practices.
Education Required:
- Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED), or
- High School Graduate.

- Basic computer skills.

Physical Requirements and Working Conditions:
- Must be able to sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift.
- Must be able to:
- lift up to 50 lbs. from floor to waist.
- lift up to 20 lbs. over the head.
- carry up to 40 lbs. a reasonable distance
- Must be able to:
- push/pull with 30 lbs. of force.
- perform a sliding transfer of 150 lbs. with a second person present.
- Will be required to use foot pedals on carts or machines.
- Must have functional vision and hearing.
- Will be exposed to mechanical, electrical, chemical, and radiation hazards as well as blood and body fluids on a continuous basis; therefore, protective clothing must be worn as necessary.

The Phlebotomist receives orders for laboratory testing, collects blood from patients of any age or status and issues appropriate paperwork to the Floyd lab or outside for processing. He/she maintains the laboratory in an efficient working state and performs lab procedures while ensuring quality control.
Major Responsibilities:
Consistently obtains high quality specimens by venipuncture.
Performs special procedures as needed.
Consistently obtains high quality specimens by capillary procedure.
Collects blood cultures with contamination rate below established goal.
Collects timed specimens appropriately.
Collects outpatients in a timely manner with waiting kept to a minimum.
Establishes positive patient identification.
Follows all Continuous Quality Improvement standards.
Ensures timely collection of all specimen by placing advances orders in proper slots on collection board and by collecting stats within
established turnaround times.
Correctly labels all specimens and ensures that collections are not missed due to errors and echos from the computer system.
Keeps the phlebotomy area clean and stocked with supplies.
Follows state, OSHA and other agency regulations, performs special procedures and assists the lab aide when necessary.
Processes all specimens quickly.
Handles cancellation of orders properly.
Accurately logs collection and received times into lab computer system.
Maintains a good working relationship with technologists and supervisors.
Displays professional attitude while in phlebotomy area.
Modifies work schedule as needed.
Follows hospital policy for attendance, punctuality, lunches and breaks.
Attends all meetings.
Assists new employees.
Effectively utilizes allotted time and resources.
Licensure, Registration, and/or Certification Required:
- PBT (ASCP) or equivalent certification
Education Required:
- High School Diploma or equivalent
- Completion of a formal training program in Phlebotomy

- Three years of hospital experience required.
Physical Requirements and Working Conditions:
Full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Standing and walking for
extensive periods of time. Occasionally lifts and carries weight up to 50 pounds. Ability to distinguish letters, numbers and symbols with vision
and hearing within normal range to respond to Code alerts and equipment alarms. Work-related stress due to volume and time constraints with
irregular working hours.
