DO NOT POST AS NEW REQUnder the direction of the appropriate licensed clinician, the Patient Care Technician I performs a variety of patient care activities. Receives workplace education and training to assist members of the healthcare team in the care of patients.
I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning, and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.
7. Takes and records vital signs (temperature, pulse, respiration, blood pressure and pulse oxymetry). Measures and records patients’ weights and heights.
8. Performs EKG’s, Point of Care (POC) tests and bladder scanning. Collects, labels and prepares specimens for transport to lab.
9. Performs other selected unit specific procedures, such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
10. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient, obtaining equipment and supplies, helping to position patient, providing the patient with emotional support, and cleaning up equipment and supplies following the treatment and/or procedure.
11. Observes, measures, records oral food and fluid intake. Refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
12. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
13. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
14. Replenishes patient and unit supplies and equipment as needed.
15. Requests assistance from identified resources when more direction is needed.
16. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures, and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

1. Minimum of a high school education or GED.
2. Basic Life Support certification within 30-days as a condition of employment.
Experience/Skills:

1. Ability to read, write and communicate in English in a clear and concise manner.
2. Basic computer skills
3. Basic Medical Terminology knowledge.
4. Must be able to remain calm when under stress or in emergency situations
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Position requires work indoors in a patient care environment.
Hand dexterity, hearing and vision ability to take direction from a patient or provider, adjust equipment, observe and describe patient observations. Must be able to lift, up to 50 pounds.
Work is of heavy demand, standing and walking most of the time with the possibility of pushing heavy equipment and bending frequently.
FOR TA PURPOSES:
Job Title: Patient Care Technician II
Job Profile Summary:
Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team. In order to become a PCT II a caregiver must meet the full competencies outlined by the Nursing Professional Practice.
As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.

Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.

2. Prior inpatient/outpatient care experience or clinical care training.

5. Computer skills.
6. Strong interpersonal skills to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients.
7. Must have proven problem-solving abilities for routine situations.
8. Must be able to remain calm when under stress or in emergency situations.

Under general supervision, serves as expert resource for performing and teaching in CV/Cath/EP labs. Utilizes professional cardiovascular theory, technology and processes to implement direct patient care on adult patients that require diagnostic or therapeutic procedures within the cardiovascular service. Essential Functions
- Plans, provides, and documents care of cardiovascular invasive labs patients in accordance with physician orders and established policies and procedures; uses professional clinical judgment in assisting the physician to plan the care for the patient based on the assessment of the patient's diagnosis, needs and response to care.
- Operate, monitors, records, evaluates, and maintains patient information.
- Meets challenges of case volumes and patient care issues to ensure optimal care and outcomes; works in an organized and timely fashion by prioritizing tasks to ensure efficient and efficacious patient care.
- Ensures quality and accuracy of reported laboratory data; reports variances to physician and ensures that variances have been addressed; ensures all recorded data is accurate and makes corrections as needed.
- Operates, inspects, troubleshoots and performs routine maintenance for variety of invasive lab equipment; effectively operates all devices utilized in procedures; initiates maintenance or service request for equipment that is damaged or malfunctioning.
- Performs daily room QCs and notifies appropriate people of variances; prepares procedure room and equipment for daily use and each procedure; ensures that stock on hand is sufficient to accommodate routine daily case volumes; conducts in-room inventory and supply management; is aware of and stores all patient care products according to established regulations and standards.

Education, Training, and Experience
- Associate or Bachelor’s Degree in a health science program (including, but not limited to, cardiovascular technology, ultrasound, radiologic technology, respiratory therapy, or nursing). Must have documented competency in interpretation of EKG and hemodynamics. Cardiac Catheterization or Electrophysiology Lab experience preferred. Allied health professional experience in cardiology preferred
- RCIS, RCES, or IBHRE certification within 24 months of hire and remains in active status Basic Life Support Certification required. Advanced Cardiac Life Support Certification required within 6 months of employment

The UCLA Center for Prehospital Care (CPC) is the leading emergency and prehospital academic center on the West Coast. The Center is dedicated to advancing the systems and methods of EMS. We teach, test, and certify the people who provide care to the ill or injured prior to hospital admission. Clinical Instructors provide skills presentations and skills instruction in the UCLA Paramedic Education Program and must work a minimum of two shifts per month. Navigates the Electronic Health Record (EHR) to effectively answer questions and provide documentation as needed for prior authorizations and/or financial assistance application. Refers patients to medication manufacturer assistance programs and/or aids with application to said programs when applicable. Secures medication refills and prior authorization renewals proactively. Assists pharmacists in identifying ancillary supplies necessary for select medications such sharps containers, needles/syringes, alcohol swabs and bandages that may not have been prescribed for the patient. Assumes responsibility for assigned areas of the pharmacy including, but not limited to, Specialty Pharmacy call center operations, non-sterile compounding, medication purchasing, and prescription dispensing operations, including delivery support. Ensures medication supply areas are neat and orderly and medications are stored under proper conditions for light, temperature and sanitation. Supports accreditation maintenance and payor and medication access through the documentation of required data points related to patient care and operations. Participates in quality assessment and improvement activities. Assists in identifying improvement opportunities and in the development of new processes. Maintains current unrestricted Rhode Island Pharmacy Technician licensure. Holds and maintains current National Pharmacy Technician Certification (CPhT) or has a minimum of one-year licensed pharmacy technician work experience, sits for National Pharmacy Technician Certification (CPhT) exam within first 6 months of hire, and achieves and maintains National Pharmacy Technician Certification (CPhT) within 12 months of hire. RESPONSIBILITIES:
A. Participates in maintaining a safe and therapeutic environment. Actively engages with assigned children throughout shift. Participates in the development and operationalization of the milieu care plan for assigned patients according to established department and unit-based procedure/policy. Maintains and documents level of patient observation as ordered by the physician/charge RN each shift. Provides support to visiting parents as well as information according to treatment team request. Supervises assigned children in daily routines and actively assists with activities of daily living and basic hygiene needs. Identifies, reports, and resolves potentialctual problems on the unit. Demonstrates appropriate critical incident follow-up skills by timely verbal reporting to charge RN and documentation in medical record. Completes other documentation (i.e., incident reports, intakes, etc. as applicable). Participates in restraint debriefings. Carries out daily assignment as delegated by Charge RN and reports back prior to change of shift. Intervenes in disruptive milieu or destructive to self/other patient situations providing timely therapeutic intervention(s) to children in distress. Maintains appropriate boundaries.
B. Adheres to Hospital, Departmental, and Program policies and procedures. Attends peer review and staff meetings as assigned. Maintains current certification in crisis management. Maintains current CPR certification. Maintains current OSHA, ERS, Safety Certifications Attends unit/hospital staff development as assigned.
C. Actively participates in the patient care planning process under the direction of the primary/charge RN. Works to support initial therapeutic alliance with patient and family on admission.Assists with the separation process and performs other duties necessary to provide a smooth transition to the hospital environment. Within the 8-hour shift, documents objective observations within a shift note and shares observations with assigned RN. Reviews milieu care plan and problem list on a daily basis and suggests revisions based on observations. Communicates individual patient milieu care plans in an ongoing manner to other unit staff including liaison with other interdisciplinary treatment providers. Actively participates in treatment team, rounds, patient-specific discussions, and hand-off shift report, as assigned. Accurately documents significant patient/family observations/milieu care on the Kardex and medical records in a timely fashion. Offers information and knowledge acquired through experience to other staff via formal/ informal staff development forums. Is able to adapt care “style” to meet needs of various children and adolescents.
D. Supports and contributes to the development of care plans/behavioral management. Assists and teaches patients in activities of daily living and personal hygiene based upon patient’s abilities. Co-leads community meeting/milieu meeting/social skill group. Co-leads other program specific groups such as activity or recreation. Demonstrates ability to set consistent and supportive therapeutic limits within guidelines of hospital, unit, and department standards. Follows behavioral programs specific to the needs of the patient in collaboration with the assigned RN and treatment team. Organizes unit activities. Coordinates, supervises and actively participates in children’s activities such as physical &social development activities. Arranges for transportation of materials if required. Participates in teaching visits/family and parent training as assigned. Consistently models to patient/families appropriate and therapeutic interpersonal staff/patient interactions. Co-leads therapy groups with a clinician. Supports and actively participates in recreation therapy programs. Handles crisis situations in a safe, effective manner, utilizing the least restrictive, safe patient care alternative. Takes the initiative to seek formal supervision, incorporating suggestions for improvement/changes in job performance. Performs other related duties as required, assigned, or directed by Charge RN. While an individual is hired to work on a particular program, they may at times be asked to perform comparable duties in other clinical settings throughout the hospital system as program needs arise.
BASIC KNOWLEDGE:
Baccalaureate Degree in a Human Service field preferred. Other education and experience considered and may be substituted for educational requirements at the discretion of the Hiring Manager.
Must demonstrate aptitude and willingness to learn role related skills.
Demonstrated knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Safety Care Certification: Employees in this position work in a psychiatric setting under stressful conditions associated with care of psychiatrically impaired, aggressive, and sometimes, self-injurious individuals. Employees are expected to utilize de-escalation techniques taught by Bradley Hospital to minimize need for physical intervention. They must have the ability to remain calm during a crisis situation as evidenced by the ability to make quick rational decisions and prioritize immediate patient care needs to maintain a therapeutic environment and ensure safety. Employees in this position must also be able to participate in the physical management of patients which can include, but is not limited to, awareness and demonstration of safe body positioning at all times, awareness and coordination to block and deflect possible aggression, and the ability to physically hold struggling patients, using approved methods only, to maintain safety for extended periods of time. Specifically, such employees must be able to: lift up to fifty pounds; reach with their hands and arms; grasp and hold with their hands; bend/squat/kneel without limitation, and physically escort patients who may become combative for short distances. Position requires occasional running for short distances, and frequent standing, walking, and sitting.

- Real-Time Documentation: Accompany clinicians during exams to transcribe visits into the EHR using precise medical terminology.
- Order Entry: Pend labs, imaging, medications, and referrals with appropriate diagnoses for clinician approval.
- Chart Completion: Update patient histories, enter follow-up instructions, and submit final records for physician sign-off.
- Clinical Assistance: Measure vital signs, room patients, and assist with non-sterile procedures as needed.
