This is an overnight position (3rd shift)
7 on 7 off, 10-hour shifts.
Pay Range:
$21.85 - $32.80Major Responsibilities:
- Develops competence in dispensing and delivery of medications. This includes developing competence in the department’s use of information systems, technology, and automation used for dispensing and storage of medications.
- Develops competence in preparing non-sterile products following USP, state, and federal recommendations, as required.
- Develops competence in entering medication orders in the computer system and/or processing medication requests via the computer system.
- Develops competence in managing phone calls from customers and team members such as nurses, physicians or patients. This includes accurately understanding and interpreting the issue, timely resolution, or timely hand-off to another team member as appropriate.
- Develops competence in pharmacy procedures and documentation as required, satisfying legal, regulatory, and department requirements of pharmacy practice.
- Complies with legal, regulatory, accreditation standards. This includes (but is not limited to) controlled substance procedures, USP procedures and DNV requirements.
Licensure, Registration, and/or Certification Required:
Licensure (IL only): State of Wisconsin (registration):
Pharmacy Technician or Pharmacy Technician Student; Certification (CPhT) issued by PTCB or ExCPT within two years of hire, or sooner if required by law. Must sit for the certification exam at least one time within the first year of employment.
Patient facing teammates may be required to have two certifications: Immunization and non-vaccine injectable to be granted from the state of WI or if from out of state, the program is approved by the accreditation council for pharmacy education (ACPE) or by the WI pharmacy examining board.

Physical Requirements and Working Conditions:
- Operates all equipment necessary to perform the job
- Frequent fine motor motion of hands and wrists to operate keyboard and manipulate syringes
- Lifts, carries, and/or pushes/pulls various items (bags, boxes, carts, etc.) while utilizing proper technique
- Will be frequently required to lift to 35 lbs., lift and carry up to 35 lbs. at waist height a reasonable distance, push/pull with 30 lbs. of force
- Must be able to sit, stand, walk, lift, bend, kneel, climb, twist, crawl and reach above shoulders
- Must have functional speech, vision, touch, and hearing
- May be exposed to chemical hazards
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.

The Maintenance Tech Evaluates a variety of functions in the maintenance, repair and upkeep of the physical plant, grounds, and equipment. Supports, facilitates, and participates in the elder-focused social model of care.

General Requirements:
- Must have valid Driver's License.
- Must be able to maintain, repair and replace existing utility systems and write up material specifications for projects.
- Mechanical aptitude and proven ability to use hand and power tools, to work on ladders at various heights, and demonstrate a safety-conscious attitude.
- Ability to operate large vehicles, tractors, plows, and various lawn equipment.
- Must be able to work with limited supervision to complete tasks assigned in a timely manner and have the ability to handle multiple demands and competing priorities.
- Ability and willing to work in a variety of environments, including office settings, classrooms, laboratories, mechanical rooms, service tunnels, and outside areas.

Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. May conduct hospice initial and comprehensive assessments and establish and update the plan of care. Ensures continued skilled nursing/home bound criteria are met. Initiates appropriate preventive and rehabilitative nursing procedures.
Utilizes proper body mechanics, assistive devices and safety techniques when performing various patient positioning/repositioning and transporting duties, which require lifting, and pushing/pulling. Orients and mentors new staff as requested.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration and/or Certification Required:
Registered Nurse license issued by the state in which the team member practices, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained upon hire unless department leader has determined it is not required, and
A valid driver’s license issued by the Division of Motor Vehicles.

Job Title: Patient Care Technician II
Job Profile Summary:
Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team. In order to become a PCT II a caregiver must meet the full competencies outlined by the Nursing Professional Practice.
As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.
I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.
7. Takes and records vital signs (temperature, pulse, respirations, blood pressure and pulse oxymetry). Measures and records patients’ height and weight.
8. Performs EKG’s, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels and prepares specimens for transport to lab.
9. Phlebotomy in certain locations.
10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.
17. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
18. Has the ability to precept new Patient Care Technicians.

Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.
Assists the physician/provider with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.

$19.80 - $29.70Major Responsibilities: 2-Bin Inventory Management 9)Be responsible to maintain all assigned 2-Bin inventory locations, ensuring that the inventory is placed in the appropriate bin, the room is organized and orderly, the 2-Bin standard has been adhered to and not disrupted in any manner, stock is not placed in bins incorrectly, bins have not been removed from the room and that debris is not on the floor or under the inventory racks. 1)Be responsible to pick and pull all regularly scheduled inventory to replenish all assigned inventory locations at 100% accuracy. 2)Deliver all 2-Bin replenishment orders to the appropriate 2-bin inventory location. 3)Restock all 2-Bins with items picked and restock in appropriate bin and perform an inventory cycle count in bin to insure inventory accuracy. 4)Check for expired product and replace with appropriate product and adjust the inventory count by performing an inventory cycle count. 5)Perform a supplement restock when requested or when requested. 6)Pick up all empty bins and any product returns from the assigned inventory locations. Receiving 1)Be responsible to perform receiving functions for all incoming orders, whether bulk or logical unit of measure related. 2)Review all incoming orders for accuracy and correlation to appropriate PO and ship-to location. 3)Identify incoming quality issues and errors. Interface with all applicable customer service representatives associated with the third-part shipping entities. 4)Perform all receiving functions via the scanning technology and work standard provided. 5)Maintain a clean work environment on a daily basis, performing all housekeeping functions as needed or assigned, including the use of red tag and visual management tools to ensure that all tools, equipment and work space items are located in specified location at all times. 6)Deliver all inbound product received, including the analysis, problem solving and trouble-shooting for all incoming product that is not properly identified with the correct ship-to or requisitioning location. 7)Cleans and disinfect patient care equipment according to Infection Control policies. 8)Perform all other duties related. Distribution 1)Be responsible for inventory management and an accurate pick process for all requisitioned inventory items. Maintain, on average, no more than five (5) days on hand for any inventory SKU, unless otherwise approved or directed by the Supply Chain Leader. 2)Pick, as required, and deliver all requisitioned orders or other product to the requester. Utilize bar code scanning technology, as provided, to capture electronic signatures and order delivery / receipt confirmation record. 3)Coordinate reviewing and obtaining all non-compliant product placed into any green bin within any assigned inventory location at a frequency assigned, or no less that once a week of a frequency is not assigned, transporting all such inventory back to General Stores. Enter all such product obtained from the green bins into the standardized on-line database in a timely manner. 4)Be responsible for returning all General Stores equipment used for Distribution to General Stores when such use is complete. 5)Coordinate product returns from assigned requisitioning locations back to the General Stores. Picking 1)Be responsible for picking a minimum of 100 lines per hour, achieving this benchmark within a period of time determined to be acceptable by the Supply Chain Leader, but at no time picking less than 60 lines per hour. NOTE: The lines per hour pick time is intended to be fully inclusive of all the complete replenishment to the respective point-of-use location, thereby taking into account pick time, travel time to the department, and replenishment time. Pick all orders requested by unit department by time basis unless directed by lead in shift. 2)Be responsible at all times for the accuracy of your respective inventory picks. Be responsible for the cycle count error generated within your inventory area of responsibility within the General Stores or, if different, the inventory locations you are assigned to pick product. Develop a corrective action plan to mitigate the cycle count errors generated from your respective area of inventory responsibility. Chart a trend chart of the cycle count error rate, by quarter, for your respective inventory location. Maintain the trend chart on the General Stores Quality Board, identifying your trend chart with your name and inventory location. Be responsible for continuous improvement over time. 3)Be responsible for the inventory pick associated with all 2-Bin inventory locations assigned, complete with returning the empty bins to the General Stores so as to follow the defined pick / replenishment process. 4)Be responsible to use the designated picking automation and associated process to perform all inventory picking / replenishment process. Tag all product with the appropriate and designated stickers. 5)Be responsible to release all picked orders using Lawson WH32.2 and sign paper work. 6)Be responsible to utilize and follow the process and procedures with the assigned velocity picking format to ensure proper utilization of inventory product to facilitate Items requiring re-order. Non-Conforming Policy and Procedure Management 1)Be responsible for analyzing the non-conforming product incident rate by SKU for any assigned inventory location. Be responsible for making process improvement recommendations to the Supply Chain Director or their designee in regard to actions to mitigate the recurrence of non-conforming supplies. 2)Be responsible to maintain the non-conforming green bins, placards and laminated job aids in all inventory locations, as assigned. 3)Be responsible for rotating all stock located in the respective inventory location bins or kardex, ensuring a first expired, first out (FEFO) approach. Remove any non-conforming product and transport to General Stores as specified under the Non-Conforming Policy. 4)Be responsible pick-up all non-conforming product from the green bins on a frequency no less than weekly. Transport all non-conforming product to General Stores and enter all items into the appropriate cells in the non-conforming product log located at: http://demandtrack.advocatehealth.com/supply_incident_report/ 5)Be responsible for communicating non-conforming clinical supply trend and analytical information to the appropriate clinical leadership responsible for the areas for which your assigned inventory location(s) reside. Job Summary
The Supplemental Physical Therapist assesses patients, develops and initiates treatment plan based on the results of the evaluation. The evaluation may include measurements of neuromuscular and skeletal functions. The incumbent documents findings, progress and provides instruction to patients and caregivers. ESSENTIAL FUNCTIONS: 1. Manages flow of materials between the Operating Room and Sterile Processing Department; organizes, directs, and implements daily activities regarding surgical instrumentation ordering, presentation, maintenance, and preparation. 2. Reviews the surgery schedule and ensures, together with the Production Supervisor, that all surgical procedures have an accurate case cart. Works closely with OR coordinators to ensure that procedure cards remain accurate. 3. Assesses case flow during the day, monitors instrument needs, and notifies Sterile Processing Department personnel in advance of any possible additional/emergent instrumentation needs. 4. Plans and coordinates all instrument turnarounds. Assists in developing, communicating, implementing, and trouble-shooting instrument issues that may affect or delay surgery start time. 5. Acts as a liaison between the OR and the Sterile Processing Department; interfaces with the OR Coordinators, OR staff and management, and SPD staff and management. 6. Coordinates the development and implementation of standards of performance with team input regarding surgical instrumentation. 7. Collaborates with OR Coordinators, OR managers, staff educators, Sterile Processing Department and Sterile Processing Department managers to develop a training and development plan for surgical instrumentation. 8. Demonstrates knowledge and understanding of the application/use of instrumentation. 9. Performs duties in accordance with hospital, JCAHO, AORN, and other appropriate policies and procedures. 10. Verifies that required instrumentation has been sterilized and is available for next-day, call or weekend cases. 11. Performs, without supervision, all the functions of the Instrument Technician, and serves as a resource on any of these functions when needed. 12. Performs other duties as assigned by the Sterile Processing Department manager.

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and
- Nurse Aide in good standing as indicated with the State Registry in which the team member practices.
Education Required:
