Unit Clerk - GMU PT DUnder the supervision of the Unit Leadership, the Unit Clerk/Receptionist performs clerical and receptionist functions for the unit. Duties include preparing and compiling records on the nursing unit, coordinating unit communication systems (e.g. phones, nurse call system, hospital information systems); relaying information; maintaining optimal public relations; assisting in maintaining supplies, equipment and overall unit environment and other duties as assigned by the Unit Leadership. Employees providing direct patient care must demonstrate competencies specific to the population served.
High school diploma or general education degree (GED) required for those hired on or after 6/1/2017.
1-3 months related experience and/or training preferred.
Previous healthcare experience preferred.
Ability to successfully complete the organizations' competency requirements for English, writing, spelling and reading comprehension. Ability to successfully complete the hospital’s orientation, core orientation and unit specific orientation.
• Working knowledge of medical terminology.
• Ability to input and retrieve data from the organization's computerized systems.
• The organizational skills to maintain records and orderly filing systems and to manage multiple processes simultaneously.
• Must be able to communicate effectively with patients, visitors, and all members of the care delivery team.
Work which produces very high levels of mental/visual fatigue, e.g. CRT work between 70 and 90 percent of the time, and work involving extremely close tolerances and considerable hand/eye coordination for sustained periods of time.
Involves considerable standing or walking, regular lifting of light-weight objects (i.e., 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients. Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.
• Coordinates the flow of information throughout the unit/department for all staff.
• Answers telephone and nurse call system and provides information or relays messages to patients, visitors and medical staff.
• Directs visitors to patients’ rooms or visitor waiting areas.
• Distributes mail, newspapers, and flowers to patients.
• Compiles census of patients, maintains records and distributes information in accordance with unit/department procedures.
• Coordinates with the charge nurse to insure smooth, efficient and timely patient flow into, through and out of the department.
• Responsible for performing clerical duties and processing equipment and staff requests relative to department needs.
• Records name of patient, address, and name of attending Physician to prepare medical records on new patients.
• Keeps file of medical records on patients in unit.
• Monitors and requisitions supplies designated by nursing staff, organizes and stores such in accordance with unit/department procedures.
• Gathers/inputs/retrieves information into various computerized systems.
• Transports patients and guests to locations within facility, under direction or as needed.
• Stocking to facilitate patient care in unit assigned to.
• Take admission paperwork to room, facilitate and complete Medicare/important message, and other assigned paperwork as directed by RN.
• Manage incoming Tubes from tube station and deliver meds or other items to the RN.
• Work with transport to monitor elevator bays for beds/carts.
• Assist in monitoring Environment of Care.
• Other duties as assigned (passing water, getting towel) attending to patient and family needs.
• Maintains unsupervised access to the unit medication room(s) for stocking and/or retrieving supplies.
• Hold and cuddle stable newborn infants under the direction and supervision of the RN. (OB Mother Baby/Antepartum only)
• Manage incoming tubes form tube station and deliver meds or other items to the RN/medication cart.

Function as a Patient Safety Assistant for suicide precautions only, which includes,
- Safe patient handling required.
- Responsibilities include observation and reporting patient activities and behaviors and
appropriate response intervention to assure patient safety. The Patient Safety Assistant will demonstrate appropriate communication skills with patient/patient family and other healthcare providers, and practice confidentiality and safety procedures as defined by agency. The Patient Safety Assistant provides services in accordance with policy and procedure manuals. Employees providing direct patient care must demonstrate competencies specific to the population served.
- Maintain appropriate knowledge level to assist in patient turns and assist in patient transfer to commode/bathroom.
- Uses correct body positioning, proper body mechanics to avoid injury, utilizes safe work practices (e.g. pushing vs. pulling, patient transfer/lifting devices, etc.) and uses personal protective equipment when necessary.
• 80% Observation and prevention of unsafe patient behavior/activity
• 10% Communicating with the responsible R.N. about patient status and pertinent input to facilitate care planning.
• 5% Accompanies patients for diagnostic tests occasionally.
• 5% Maintains a clean and orderly environment.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:
Valid State of New York Armed Security Guard License (includes firearms training and a valid NYS pistol permit) OR active police officer status (New York State) at time of hiring is required.
Active police officers must obtain armed security license and a NYS pistol permit within 6 months upon being hired.
Successful completion of the DCJS (division of criminal justice) Basic Course for Police Officers preferred.
High School Diploma/GED Certificate required. Associate degree preferred.
A valid NYS Drivers License is required.
IAHSS Basic Security Certification must be completed within one year of hire.
FEMA Emergency Management Institute courses must be completed within 90 days of hire - IS - 100c.

Security Services
Performs all security/safety rounds/fixed post assignments utilizing departmental prescribed procedures; as well as utilizing training and experience as an Armed Security Guard/Law Enforcement Personnel to de-escalate workplace violence, mitigate threats and properly handle emotionally disturbed persons (EDP).
When not on a fixed post assignment, respond to calls for service timely and in an appropriate manner.

Pay Range: $25.00 - $35.70
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Maintains the departments equipment in a safe manner.

Security Services
Performs all security/safety rounds/fixed post assignments utilizing departmental prescribed procedures; as well as utilizing training and experience as an Armed Security Guard/Law Enforcement Personnel to de-escalate workplace violence, mitigate threats and properly handle emotionally disturbed persons (EDP).
When not on a fixed post assignment, respond to calls for service timely and in an appropriate manner.

Fully investigates incidents occurring in the SJH facilities with an imminent threat to the life and safety of colleagues, patients, visitors, or others.
Document incidents and events through use of the Video Surveillance System, access control system and still photographs using the departmental cameras.
Maintain confidentiality of all investigations, situations, conditions, or other matters of concern and not disseminate outside of the Safety and Security Department without approval of the Security Manager or Sr. Director.
Pay Range: $25.00 - $35.70
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

$20.80 - $31.20What is a Sterile Processing Tech (SPD Tech)?
SPD Techs have a critical role in preventing infection with tasks such as sterilizing, cleaning, processing, assembling, storing, and distributing medical supplies. This role will also handle soiled and contaminated items, so you need to be comfortable with handling. This is a very tactical position that requires strong attention to detail and will work behind the scenes. This position does not interact with patients. Previous SPD or healthcare experience is preferred. SPD Certification for this position is not required.
Major Responsibilities:
- Receives soiled and contaminated items in decontamination area. Sorts equipment, rinses as needed, places equipment through sonic tanks and washers, reassembles trays, performs safety checks, hangs and sorts instruments, wraps sets according to set policy, and wraps towels and basins appropriately.
- Sterilizes instruments and equipment using the aerator, gas sterilizer or steam sterilizer and operates glass dryer. Packages items in steri/peel as required. Performs quality assurance testing for all methods of sterilization to ensure sterility of supplies. Hand cleans miscellaneous instruments (Baxter pump, hypothermo units, etc.) and operates the cart cleaner.
- Ensures proper care and handling of high cost instrumentation and equipment to include items requiring special handling, laser instrumentation, orthopedic implants, cardiac assist devices, endoscopic and microscopic instrumentation.
- May build case carts according to procedure and physician preference as specified. File case cart sheets and requisitions, responds to Code 4s with defibrillator and back up supplies, maintains CPR annies, tests autoclaves with the tests according to procedure, and changes graphs as necessary.
- Checks incoming trays and follows up with departments as to missing instruments or parts.
- Takes immediate and appropriate action if a question of sterilizer failure occurs.
- Receives and records orders for supplies as requested by departments. Records accurate product number and assures all forms are correctly completed for proper billing. Marks, dates and stocks equipment, which may include returning to appropriate department on a timely basis.
- Opens and unpacks supply boxes, checking for damaged goods and number of items received. Reports any discrepancies to the supervisor. Performs credits for unused products returned, checks stock levels, copies supply sheets, checks carts for outdated supplies, and performs general cleaning of shelves.
- May performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

Essential Functions
- Prioritize and deliver basic patient care, including toileting, bathing, linen and clothing changes, fresh water and ice, ambulating, comfort measures, and other patient needs, while providing excellent customer service when responding to patient and family requests.

- Follows all Infection Control policies. Completes the Environmental Safety Checklist each shift and during hand-off, ensuring ligature risks are mitigated in coordination with the primary nurse.

Ensure the patient is in a gown with no strings, and remove all patient clothing, shoes, jewelry, and belongings from the room, preferably sending them home with family. Secure any valuables not taken by the family. Do not allow visitors to give any items to the patient or bring large bags into the room; consult nursing if visitors refuse.

Essential Functions
- Performs inventory control on stock and non-stock items (e.g., sets par levels, identifies and adjusts to trends) for assigned units. Keeps supplies binned and organized. Acts as liaison between Supply Chain and assigned department, facility, or clinical program.
- Orders and delivers supplies for assigned department, facility, or clinical program using appropriate technology, systems, and suppliers.
- Communicate information concerning supply levels, new products, standardization, and implementation relevant to the department staff. Troubleshoots issues for problem resolution.
- Responsible for Par Location Management, PIRS Replenishment, Location Maintenance, Delivery, Label maintenance and Kanban visuals.
- Monitors quantity on hand issues reconciling and solves any discrepancies.
- Utilizes the SCIS system ensuring distribution, inventory, receiving, departmental files, and reports are managed in accordance with Auditing, Accounting, and other regulatory agencies.

This position performs all cleaning of patient rooms and/or assigned areas. Summary:

Responsibilities:

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
- Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.

- Administers medications, under physician/provider orders, including: immunizations, antibiotics, pain medications, vitamins, topical agents, and insulin. With limitations, initiates, maintains and discontinues IV therapy; administering selected fluids with pre-mixed medications.
- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.

This is a PM shift position from the hours of 3 PM to 11:30 PM 3 evenings per week. Every other weekend and holiday rotation required.

Pay Range:
$20.80 - $31.20Major Responsibilities:
- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and
- Nurse Aide in good standing as indicated with the State Registry in which the team member practices.
Education Required:
- Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED), or
- High School Graduate.
Experience Required:
- None.
Knowledge, Skills & Abilities Required:
- Basic computer skills.
