Join Wake Forest Emergency Providers as an Advanced Practice Practitioner (APP) in Monroe, North Carolina—part of Advocate Health, one of the largest nonprofit integrated health systems in the country.
Wake Forest Emergency Providers, affiliated with Atrium Health Wake Forest Baptist and Wake Forest School of Medicine, delivers high-quality emergency care across Hospital Emergency Departments and Free-Standing Emergency Departments throughout the Greater Charlotte Market and Central and Northwest North Carolina. Our APPs practice in settings that offer a strong sense of community and teamwork, while benefiting from the clinical resources, academic collaboration, and innovation of a leading nonprofit health system.
Location: Â Wilkes Medical Center - 1370 W D St, North Wilkesboro, NC
100 bed community hospital. 30,000-33,000 annual patient visits in the ED. Proximity to Winston and Boone .

Staff: 10 physicians and 3 APPs
Role: “True Emergency Medicine” – Classic and cutting edge EM work with APPs fully integrated in the care teams.
Training: Depends on individual experience level
Licensure: Must have NC PA/NP license (NPs must be ENP boarded or eligible), BLS, ACLS before starting. Must obtain ATLS after starting eventually.
Schedule: Variable

Registry/Per Diem positions do not have guaranteed hours and no medical benefits package is offered. Registry/Per Diem positions require a minimum of 2 shifts a month commitment. POSITION SUMMARY
This position performs all cleaning of patient rooms and/or assigned areas. Works as a member of the environmental service team to ensure the safety, health, and high morale of patients, visitors and staff by helping to provide a clean, safe, and aesthetically pleasing environment with minimal interference to others.
COREÂ FUNCTIONS
1. Performs a variety of assigned cleaning tasks, including dusting, mopping, scrubbing, disinfecting, vacuuming and polishing according to departmental policy and procedures. Must plan the completion of these tasks daily.
2. Empties hampers and transports of soiled linen. Empties trash containers and removes all trash. Empties all medical waste containers, including sharps.
3. Inventories, restocks and replenish supplies such as toilet tissue, paper towels, hand soap, etcetera, located in all assigned areas of the facility.
4. Maintains cleanliness of departmental equipment and notifies appropriate personnel as repairs become necessary. Protects equipment from damage by proper use; properly logs out and in equipment that has been used.
5. May perform special projects such as: windows washing, wall washing, removing heavily soiled items or furnishings.
6. Responds to questions and requests from patients, staff and visitors. Meets or exceeds goals for customer service satisfaction.
7. Under regular supervision, this position has responsibility at an assigned facility/area only. Internal customers include facility staff, physicians, nurses, patients, visitors, supervisors, managers, and executives. External customers include visitors, patients, and employees from other company and non-company facilities.
MINIMUM QUALIFICATIONS
Requires the ability to work independently and as part of a team without continuous supervision. Must be able to interpret and respond to verbal and written instructions. Must be able to communicate effectively with others by phone and verbally. Must be able to read and understand material safety data sheets and instructions for mixing and use of cleaning chemicals, and able to fill out chemical identification stickers.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, and BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. An Arizona Criminal History Affidavit must be signed upon hire. Employees working at Olive Branch Senior Center must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Previous related experience and familiarity with environmental services equipment and cleaning techniques is preferred.
Additional related education and/or experience preferred.
Estimated Pay Range:
$16.00 - $24.00 / hour

Summary:
Performs an integral role with the care team for coordination of complex chemotherapy and cancer treatment scheduling. Manages all aspects of scheduling of multi-day and multi-week treatment cycle schedules for high acuity patients. Responsible for scheduling treatment including infusion treatments, transfusion, labs, surgery, and radiation treatment for oncology patients. Demonstrates and conveys a cheerful and favorable disposition during stressful patient care delivery for a positive patient experience.
Responsibilities:
Schedules and coordinates complex chemotherapy and radiation therapy treatment plans and cycles including intervening labs, imaging, and MD visits as per specific treatment schedule. Ensures scheduling is accurate and timely and without delay in care. Facilitates frequent changes for all associated appointments due to patient acuity. Facilitates inpatient transitions and discharges for chemo infusions and radiation in coordination with the treatment plan schedule. Utilizes complex algorithms, scheduling guidelines and medication lists for chemotherapy, non-chemotherapy medications and/or transfusions.Performs other duties as assigned and as required.Coordinates care with physician, advanced practitioners (APPs), nurses and fellows to confirm and review treatment regimens and overall plan of care. Coordinates with Pharmacy on drug availability.Reviews in-basket message pools and collaborates with providers and clinical team (infusion nursing and radiation therapists) to ensure timely scheduling upon office visit and/or hospital discharge. Communicates with treatment unit leadership to maximize clinic flow.Identifies symptoms and refers patients to appropriate APP for symptom management, when needed.Coordinates with Infusion Clinical Nurse Manager or Radiation Oncology care team for same day add-ons for sick or urgent patients and for transitions from inpatient or transplant. Alerts the authorization team.Contacts patient to coordinate appointment scheduling. Communicates with Preservice/Authorization department to ensure prior authorization of treatment is obtained. Handles pre-registration prior to date of visit to ensure prior authorization for treatment.Coordinates the scheduling of surgeries and related procedures. Schedules appointment times using computerized scheduling programs. Communicates with physicians and other healthcare team members to ensure availability of facilities and staff.

$38.20 - $57.30Major Responsibilities:
Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
 Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative  .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. May conduct hospice initial and comprehensive assessments and establish and update the plan of care. Ensures continued skilled nursing/home bound criteria are met. Initiates appropriate preventive and rehabilitative nursing procedures.
Utilizes proper body mechanics, assistive devices and safety techniques when performing various patient positioning/repositioning and transporting duties, which require lifting, and pushing/pulling. Orients and mentors new staff as requested.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration and/or Certification Required:
Registered Nurse license issued by the state in which the team member practices, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained upon hire unless department leader has determined it is not required, and
Driver license issued by the state of Wisconsin, or
Driver license issued by the state of Illinois.
Education Required:
Completion of an accredited or approved program in Nursing.
Experience Required:
Typically requires 1-2 years of experience in nursing that includes experiences in home health, hospice, medical/surgical, long term care, emergency care/ICU or oncology or similar related experience. Â
Knowledge, Skills & Abilities Required:
Competent in the principles of deformity prevention, normal range of motion, body mechanics and body alignment; and fundamental principles of human behavior and an understanding of the effects of stress upon individuals and groups.

Job Duties
- Performs general inspections, troubleshooting and repairs of minor electrical, mechanical, and plumbing items within the hospital or clinic setting. Examples of works may include but not limited to: unclogging plumbing lines, replacing light bulbs, replacing ceiling tile, repair leaking faucets, and painting touch-up.
- Follows up on completed assignments ensuring equipment is working appropriately.
- Responds to and repairs specialty systems, such as, and not limited to, the nurse call system, fire alarm system, television system, medical gas system, and the ceiling system.
- Completes preventive maintenance inspections in accordance to scheduled checkpoints and maintains documentation appropriately.

Our medical center is an 885-bed tertiary care institution affiliated with the Wake Forest University School of Medicine. Large, Academic EM program with 15 EM Residents per year.
Our emergency department is the premier level 1Trauma Center emergency department in the region and a national leader in clinical care, education, and research. Our department, featuring a new state-of-the-art facility, serves as a high-volume, high-acuity level I trauma center, burn center, stroke center, and STEMI center. With an annual volume exceeding 110,000 patient visits (combination of adult and pediatric), we address the full spectrum of human illness and injury.

Training: New graduates are welcome and supported with a robust onboarding program. Paired 1x1 with an experienced APP for the first 3 months.
Licensure: Must have NC PA/NP license (NPs must be ENP boarded or eligible), BLS, ACLS before starting. Must obtain ATLS after starting eventually.

As a Radiation Therapist, you’ll be part of a high-performing, collaborative team dedicated to delivering exceptional patient care in a technologically advanced and supportive environment. This role offers the opportunity to work in a dynamic department with five treatment vaults, treating an average of 20–30 patients per vault each day.
Department Operations & Workflow
- Therapists play a key role in the patient journey, starting from simulation through treatment delivery.
- Patients are typically scheduled 3–10 days post-simulation, with clear communication and tentative schedules provided upfront.
- Therapists coordinate closely with Physics and Dosimetry to ensure timely QA and chart checks, maintaining a smooth and efficient workflow.

- The department utilizes Varian TrueBeam, Versa Elekta, and Flexitron HDR machines.

POSITION SUMMARY
This position performs prescribed planned course of radiation therapy as directed by following department/facility policies, procedures, guidelines and protocols and in accordance with standards of professional radiation therapy practice. Performs needed assessment, care and treatment planning for patient population.
CORE FUNCTIONS
1. Performs radiation therapy treatments by following and verifying referring providers’ orders. Uses independent judgment and applies learned methodologies according to established policy and procedures.
2. Educates patients/families regarding procedure and/or treatment to be performed. Demonstrates professional behavior/conduct in all interactions; fosters teamwork, efficient use of resources and quality patient outcomes.
3. Operates treatment and treatment planning equipment to include simulation in accordance with departmental policy. Prepares treatments and administers prescribed doses in accordance with established policies. Prepares and maintains accurate documentation.

8. Independently performs prescribed radiation therapy procedures in accordance with department/facility policies, procedures and protocols to patients. Under the general direction of the Chief Therapist, the Radiation Therapist I contributes to the organizational plan of the Radiation Oncology Unit, to the quality of patient care and supports the institutional philosophy. The Radiation Therapist I is technically competent at the entry level and interacts with other areas of the department. It is expected that the Radiation Therapist I will meet all initial performance competency standards for this position within one year. The Radiation Therapist I applies ionizing radiation to the patient, as instructed and prescribed by the physician according to departmental standards, state guidelines, and Memorial Healthcare System’s policy and procedures.
Responsibilities:
Prepares room and equipment properly for each treatment. Performs imaging (CBCT, KV, MV, Active Motion Management) as ordered, to position patient. Analyzes results, identifies issues and takes appropriate action including repositioning patient and repeating procedure to deliver therapy according to treatment plan. Assists special procedure treatments (e.g., HDR, TBI) and completes treatment documentation.Participates in scheduled rotations in all department equipment, serves as on call therapist, and as needed, rotates between both Radiation Oncology departments.Performs appropriate quality control warm-up procedures on equipment. Executes simulations in accordance with department policies and procedures.
