Summary:
The Clinical Exercise Physiologist provides exercise and fitness screening, testing, and counseling and develops and implements exercise prescriptions for patients with chronic diseases. This role demonstrates skill in assessing patient response to exercise, modifies exercise plans to meet individual needs, and generates clinical documentation that meets all regulatory requirements.
Responsibilities:
Evaluates patient progress in Cardiac and Pulmonary Rehabilitation and Medical Fitness programs. Recommends appropriate modifications to exercise prescriptions. Reassesses and documents 30-day patient progress and medical fitness reports in compliance with Centers for Medicare and Medicaid Services (CMS) time requirements.Conducts and evaluates clinical stress tests, performing continuous patient assessment through electrocardiogram monitoring and observation of physiological responses, including symptoms, appearance, heart rate, and blood pressure.Develops and documents discharge treatment plan with patient (and his/her family), physician and rehab staff.Maintains professional and technical knowledge by attending educational workshops and reviewing professional publications.Delivers high-quality patient care by assessing goals and needs through intake evaluations and test result interpretation, developing safe and effective exercise prescriptions, and creating and documenting individualized treatment plans in accordance with physician orders, medical history, patient limitations, and goals.Delivers inpatient cardiac and pulmonary rehabilitation services by assessing patient eligibility, providing education, and developing and implementing individualized exercise prescriptions as appropriate. Facilitates referrals for enrollment in outpatient cardiac or pulmonary rehabilitation and medical fitness programs.Supports patients in executing individualized treatment plans and exercise prescriptions by instructing, motivating, and assisting them with prescribed physical activities.Provides inpatient exercise physiology services in collaboration with physicians and clinical staff.

Description:
POSITION PURPOSE
Provides safe, secure, and timely delivery and pickup of medications, supplies, and related pharmacy items for home infusion, enteral nutrition, long term care, and specialty pharmacy patients. Maintains accurate delivery documentation and custody controls. Protects patient privacy and product integrity during transport and delivery. Serves as a primary in person point of contact for patients and caregivers and is the face of pharmacy operations during deliveries, demonstrating professionalism, respect, and a high level of customer service at every interaction.
ESSENTIAL FUNCTIONS
1. Knows, understands, incorporates, and demonstrates the Trinity Health and MercyOne Mission, Vision, and Values in behaviors, practices, and decisions. Assists the pharmacist in outpatient prescription services, including retail, specialty pharmacy, home health care, and nursing home services, as assigned by location and role.
2. Completes scheduled delivery routes to patient homes, facilities, clinics, and other authorized locations using safe driving practices and required security procedures.
3. Maintains chain of custody for all items from pickup to delivery, including required signatures, photo documentation if used by the department, and complete delivery records.
4. Verifies delivery destination and recipient using department procedures. Obtains signature proof of delivery when required. Resolves “no answer,” refused delivery, address issues, and other exceptions per procedure and returns items to the pharmacy promptly.
5. Transports medications and supplies in a manner that protects product integrity. Uses required temperature control processes for refrigerated or temperature sensitive medications and documents required checks per procedure. Temperature controlled transport expectations are a common home infusion requirement.
6. Handles items containing patient information in a HIPAA compliant manner. Limits visible PHI on labels and paperwork to what is required. Protects items from loss, theft, or unauthorized access. HIPAA controls are a standard expectation for medical couriers.
7. Transports controlled substances and other high risk medications using required security controls, including secure storage in the vehicle, minimizing stops, and immediate reporting of loss, theft, or suspected tampering per policy.
8. Delivers home infusion medications and supplies and may support basic delivery related paperwork that must be signed at the home or facility when assigned. National pharmacy guidance recognizes drivers may support delivery and initial paperwork collection in home infusion settings.
9. Performs pickups as assigned, which may include returns, recall returns, paperwork, pumps and equipment, and supply pickups from approved sites. Ensures returned items are separated and labeled per procedure and delivered to the correct receiving point in the pharmacy.
10. Immediately reports delivery incidents and safety events, including vehicle accidents, temperature excursions, missing packages, controlled substance discrepancies, and privacy incidents. Completes required incident documentation.
11. Performs daily vehicle safety checks and maintains required vehicle logs, mileage, fuel, maintenance, and cleaning documentation. Keeps the delivery vehicle clean and secured.
12. Uses route optimization tools and follows dispatch instructions to meet delivery windows while maintaining safety and compliance.
13. Provides professional customer service during deliveries. Communicates expected arrival times when instructed. Does not provide clinical counseling. Routes clinical questions to a pharmacist, nurse, or the appropriate team.
14. Supports basic non clinical pharmacy logistical tasks as assigned when not driving, such as staging deliveries, restocking delivery supplies, printing delivery manifests, and organizing totes.
15. Follows applicable requirements for transport of hazardous drugs if the driver is ever assigned to handle or transport hazardous medications. Hazardous drug transport expectations are addressed in USP <800> and related guidance, and should be reflected in your SOPs when applicable.
16. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other Trinity Health and MercyOne policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
MINIMUM QUALIFICATIONS
- Must possess a comprehensive knowledge of safe driving practices, delivery logistics, chain of custody documentation, proof of delivery processes, and customer service in a healthcare environment, as normally obtained through a high school diploma or equivalent, and 1 to 3 years of delivery, courier, logistics, or customer facing service experience.
- Must possess a valid driver’s license and be able to meet organizational standards for driving eligibility, including required motor vehicle record review and insurance requirements.
- Must obtain and maintain the Iowa Board of Pharmacy Pharmacy Support Person designation and any other state registration required for this position based on assigned duties and service area, within established onboarding timelines, and maintain them throughout employment.

- Performs respiratory care to include but not limited to: arterial blood gas and analysis, pulse oximetry, spirometry and flow volume loops, lung volume determination, airway resistance measurement, breath diffusion capacity, bronchoprovocation testing, maximal inspiratory and expiratory pressures, bronchodilator administration, diagnostic radiology procedures. Prepares, administers and monitors treatment modalities and medication delivery. Implements, monitors and evaluates ventilator support.
- Performs other procedures and tests as needed and if assigned, such as Electrocardiograms, Holter’s, and Arterial Blood Gas.
- Provides care for the physical needs of patients by executing various respiratory care concepts to maximize performance, which includes positioning, physiology, procedure preparation, and technical factors. Modifies technical factors to achieve desired results and recognizes complications / limitations associated with certain procedures. Proactively identifies how to address these for optimum outcomes.

- Troubleshoots, calibrates and performs preventive maintenance and quality control on all equipment and analyzers per department guidelines, documenting activities. Summary:
Coordinates and provides clinical pharmacy services to patients and works collaboratively with the interdisciplinary team. The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
Verifies insurance benefits and obtains pre-certification/authorization as necessary.Communicates with physician's and their staff, nursing unit staff, and/or other appropriate personnel to exchange necessary information.Determines and accepts required payments, including but not limited to co-pays and deductibles.Ensures that all necessary demographic, billing, and clinical information is obtained and entered with timeliness and accuracy.

Other Information: In Memorial Physician Group (specialty practices), additional responsibilities include: (1) obtain specialty authorizations (2) authorization denial and peer to peer process (3) patient care navigation ex: surgical and procedural coordination and scheduling for patient specific populations (4) handle all incoming calls and physician and hospital back line (5) obtain and confirm referrals. In the Hospital, additional responsibilities include: (1) Upon validation of patient identity, place identification band on patient (2) obtain signatures for hospital specific regulatory forms not required in an ambulatory or office setting (3) obtain authorizations for walk-in appointments (4) determine when financial assistance is needed. In Memorial Primary Care, additional responsibilities include: (1) MIH-MPC program patient referral, payment collection and eligibility scheduling (2) process referral work-ques and same day access requests (3) work with Patient Access Center on real time patient requests (4) address prescription refill requests, patient advice requests through MyChart, and provider scheduling template.
For the Physician Practices (MPG, MPC and UCC): Must be willing to travel to other locations based on the needs of the business. Mileage reimbursement is provided.
