BBC Cardiac Sonography Extern- PRNUnder direct and indirect supervision, the Bronson Cardiovascular Services (CVS) Sonography Extern works with a Registered Sonographer to conduct peripheral vascular and/or cardiac ultrasounds producing two dimensional ultrasonic recordings, color flow, and doppler flow analyses of peripheral arteries, veins, heart, and related structures according to established organization policies, procedures, and guidelines. The CVS Sonography Extern is responsible for communicating learning needs to the Registered Sonographer. The CVS Sonography Extern is accountable, under the direction of a Registered Sonographer, for setting priorities, meeting deadlines, and providing selected aspects of patient care within the CVS Sonography Extern Scope of Practice.
CVS Sonography Externs must be enrolled and maintaining satisfactory progress the final year of an accredited college program for Cardiac and/or Vascular sonography. Successful completion of CVS Department requirements for orientation includes Hospital, Core, and Department specific orientation.
BLS required upon hire or within 6 months.
• Must have basic PC skills to operate the database and office programs used in the department
• Must be able to work and communicate with all managers, employees, physicians, and customers with which Cardiovascular Services Department does business
• Must be able to work well within a small work group which includes: communication effectively with team members, assertively handling conflict, diplomatically resolving issues, participating in problem-solving
• Must treat everyone in a professional manner
Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time.
Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects
- Regularly communicates with the Registered Sonographer partner.
- Assumes responsibility for professional development.
- Under supervision of a Registered Sonographer, the Extern will:
- Familiarizes patient with test to be performed and prepares for the test; obtains any relevant information from the patient and available clinical records.
- Performs all tests according to established laboratory protocols and guidelines, documenting any exceptions to the protocols.
- Adjust equipment controls in accordance to diagnostic information requested and moves transducer (by hand) over patient's peripheral arteries or veins, observes ultrasound display screen, and listens to Doppler signals to acquire data for measurement of blood flow velocities.
- Differentiates artifacts from anatomic/pathologic structures and adjusts equipment accordingly; recognizes the significance of all structures that are visualized. Measures the appropriate structures as indicated.
- Performs measurements and calculations in accordance to vascular laboratory protocol for evaluation by reading physician. Measures blood flow velocities, vessel compressibility, and calculates data for evaluation of irregularities by physician. Communicates appropriately and effectively with other members of the healthcare team regarding all aspects of patient care

At Houston Methodist, the Social Worker II (SW II) position comprehensively provides compassionate, clinical social work, psychosocial assessments, diagnosis and treatments, and complex discharge planning to patients and their families of a targeted patient population on a designated unit(s). In collaboration with physicians and the interprofessional health care team, this position sensitizes other health care providers to the social and emotional aspects of a patient's illness to collaboratively facilitate efficient quality care and achievement of desired treatment outcomes and affect positive patient and family outcomes. The SW II position holds joint accountability with case manager, assuring that psychosocial and continuing care issues are addressed and treated as needed across the continuum of care and has responsibility for unit or departmental social work program development. This position serves as a hospital-wide, service-line leader for psychosocial related issues, complex discharge planning activities, and population disease management.

SERVICE ESSENTIAL FUNCTIONS
- Serves as a hospital/post acute-based leader for comprehensive case management activities including assessing high-risk patients and leading team to identify at-risk patients, participating in daily Care Coordination rounds, and identifying and leading resolution to barriers of efficient patient throughput. Completes a full assessment based on the social work assessment, leading and addressing solutions of social determinants which is accomplished by patient/family interview, review of the medical record including previous episodes of care, H&P, lab and other test results/findings, plan of care, physician orders, nursing and progress notes. Uses advanced knowledge and clinical expertise and screening tools to identify need for case management and/or social work intervention.
- Addresses and manages conflict associated with a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups. Maintains ownership of the psychosocial component, assessments, diagnosis and treatment, of the discharge planning process on assigned units. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence and of mental health and/or substance abuse problems in patients and family members.
- Establishes mutual educational goals with patient and family, providing appropriate resources, incorporating planning for care after discharge. Provides education to physicians and other interprofessional health care team members on mutually identified goals of care and uses knowledge of levels of care, working with patient and family, to ensure discharge disposition is the appropriate level and facilitates transfers.

- Completes timely and thorough assessment on all unfunded patients to identify community resources required for effective transition by demonstrating an effective community resource knowledge base and judgment/ability to effectively select and coordinate available resources, including referrals to regulatory agencies, i.e. CPS/APS.
- Identifies, obtains and utilizes alternative resources to fill gaps in established community resources.
- Guides discharge planning activities for assigned patients and collaborates with the case managers and other members of the interprofessional health care team, as well as patient and family by intervening and coordinating cost-effective, complex discharge planning outcomes and decreased length of stay.

Minimum Job Requirements Education:
- Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License:
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing services

Position Purpose:
According to established policies and procedures performs a variety of duties related to receiving, sorting and replacing par levels of linen on carts. Delivers carts containing linens to various Medical Center areas. Picks up soiled linen to be sent out for reprocessing.
What you will do:
- Sorts all incoming clean linen and sets up par stocking on all carts for all.
- Delivers hand pushed carts containing clean linen to various Medical Center areas according to established delivery schedules or upon request.
- Retrieves previously delivered carts and returns to Linen Room with unused linens or garments.
- Collects soiled linen bags from designated areas and delivers to the Soiled Linen Room or puts bags down the linen chute, using hand pushed carts and weighs carts and records weights before sending soiled linen out for reprocessing.
- Additional duties as assigned.
Working Conditions:
- Requires working with equipment or performing procedures where carelessness would probably result in minor cuts, bruises or muscle pulls.
- Requires handling of hot liquids, toxic or flammable chemicals, or contaminated materials, which requires caution and use of established safety guidelines and equipment.
- May be required to wear Personal Protective Equipment (PPE) at times such as masks, eye protection, ear protection, gloves and gowns.

The APP will provide comprehensive care to orthopedic trauma and foot/ankle patients in both the outpatient clinic and surgical settings. Responsibilities include evaluating and managing patients independently and collaboratively with physicians, assisting in the operating room, and coordinating treatment plans across the continuum of care.

Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team. In order to become a PCT II a caregiver must meet the full competencies outlined by the Nursing Professional Practice.As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.
I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.
7. Takes and records vital signs (temperature, pulse, respirations, blood pressure and pulse oxymetry). Measures and records patients’ height and weight.
8. Performs EKG’s, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels and prepares specimens for transport to lab.
9. Phlebotomy in certain locations.
10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.
17. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
18. Has the ability to precept new Patient Care Technicians.

2. Current Basic Life Support.
Experience/Skills:

1. Demonstrated competency attainment in the Patient Care Technician I level, work a minimum of 800 hours and at least 6 months as a PCT I.
or 6 months of prior inpatient/outpatient direct patient care experience.
2. Ability to read, write and communicate in English in a clear and concise manner.
3. Medical Terminology.
4. Computer skills.
5. Strong interpersonal skills to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients.
6. Must have proven problem-solving abilities for routine situations.
7. Must be able to remain calm when under stress or in emergency situations.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

III. Physical Demands and Environmental Conditions:
Position requires work indoors in a patient care environment.
Hand dexterity, hearing and vision ability to take direction from a patient or provider, adjust equipment, observe and describe patient observations. Must be able to lift, up to 50 pounds. Work is of heavy demand, standing and walking most of the time with the possibility of pushing heavy equipment and bending frequently.
