Summary:
Acts as a resource to all levels of staff and management in evaluating patient care and nursing practice standards. Provides clinical and administrative supervision to the case managers assigned on unit including ongoing performance evaluations of staff. Is involved in program planning, feedback on both policymaking, and decision-making for the unit and/or department. Communicates information to unit staff as well as to the manager.
Responsibilities:
Assists with supporting the department manager and director through the evaluation of department operations, including information technologies, service level determination, and complaint management, to achieve performance and quality control objectives. Analyzes and submits statistical reports monthly and as requested.Collaborates with interdisciplinary staff to design and implement appropriate treatment plans, including discharge planning, to ensure quality outcomes. Develops, leads or co-leads groups as needed. Supports and maintains existing standards of the department and of the profession.Participates in the orientation, training, evaluation, clinical performance improvement, disciplinary actions, and continuing education initiatives of the case management staff. Manages and constantly evaluates staffing levels, assignments, skills, learning needs, and deployment of staff. Performs problem resolution activities to maintain quality patient care.Identifies practice and system issues, and trends utilizing medical criteria, protocols, and clinical data systems utilized by the organization. Assists with the development and implementation of Case Management goals, plans, and standards consistent with the clinical, administrative, regulatory, legal, and ethical requirements.

Complexity of Work: Plays a pivotal role in overseeing the daily operations of the case management team ensuring that all activities align with organizational objectives and meet regulatory standards. Requires critical thinking skills, effective communication (verbal and written) skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Have knowledge of State and Federal regulations governing discharge planning conditions of participation, hospital information systems (e.g., logician and IDX), staff development techniques, and performance improvements processes. Requires a comprehensive understanding of medical necessity criteria, payer guidelines, and regulatory compliance standards. This role involves high level analytical and decision making skills to evaluate clinical data against medical necessity criteria while balancing financial and operational objectives. Responsibilities include real time problem solving and prioritizing urgent cases, coordinating with physician advisors on challenging cases, or escalating to leadership as needed. Required Work Experience: Three (3) years of experience in an acute care setting as an RN in case management. Leadership experience preferred. The Registered Nurse provides compassionate, evidence-based nursing care through assessment, treatment, care coordination, patient and family education, and individualized care planning while supporting admissions, transfers, and discharges.
Job Description:
The Registered Nurse (RN) implements professional nursing practice centered on the patient as a whole person, and the creation of a caring environment focused on healing and wellness.
The Registered Nurse (RN) is an exemplar for compassion, acceptance, respect, and empathy honoring nursing as a distinct discipline and supporting an environment focused on healing and wellness.
As a clinical nurse, this individual spends majority of his/her time (> 50%) coordinating, ensuring and providing direct patient care activities on the unit through: provision of nursing rounds, assessments and treatments; facilitating admission, transfer and discharge activities; provision of patient and family-centered education; care coordination and individualized treatment planning as applicable, etc.
Minimum Education, Experience, Training and Licensures Required:
Current RN licensure or work permit in Kentucky.
Bachelor's or an associate degree in nursing from an accredited college/university.
Basic life support- Applicants will have 30 days from their hired date to provide completion of this certification if they don’t have it already.
ACLS/PALS/NRP and successful completion of dysrhythmia class as required by department of Assignment.

Primary Duties and Responsibilities:
• Ensures proper patient identification and maintains patient safety, privacy, confidentiality and follows guidelines under HIPAA.
• Obtains and records vital signs, pain screens, weights and heights accurately into Electronic Medical Records (EMR).Reports changes and/or abnormalities in vital signs; findings that are outside of defined parameters; and changes in patient’s conditions to the nursing/medical staff in a timely manner.
• Obtains specimens (urine, stool, sputum, etc.) and ensures accurate labeling in presence of patient.Instructs patients/family members in collecting specimens.
• Performs phlebotomy and EKG as applicable and is competent in the two patient identifier practice.

• Assists with disease center specific procedural needs as applicable.
• Transports patients, medical records, specimens, blood components, equipment and supplies in accordance with Institute policy.
• Ensures that all patients on schedule are accounted for and communicate with providers/patients and colleagues as to approximate wait times.

PRIMARY DUTIES AND RESPONSIBILITIES:
Clinical Responsibilities:
- Performs radiotherapy on site at assigned department.
- Responsible for daily patient care and education. Appropriately involves the patient and their family in care and care decisions. Maintains and demonstrates respect for patient confidentiality and modesty. Explains all treatment and imaging procedures. Arranges daily radiation treatment schedule. Attentive to patient needs and concerns by continuous monitoring of the patient’s physical and physiological response and reactions to treatment and refer patient for appropriate management as needed.
- Administers radiation therapy treatment accurately and safely, following departmental policy and procedure.
- Positions and immobilizes patients according to the treatment plan and effectively evaluates images according to standard guidelines. Mobilizes patients using appropriate mechanical assistive devices, such as ceiling lifts and lateral transfer devices, during the provision of patient care.
- Assesses patient relative data as deemed pertinent to age-specific needs and provides care as described in the department’s policy and procedure manual.
- Participates in the clinical instruction and evaluation of the Radiation Therapy students. Accurately perform and teach all policy, procedure, and QA measures including New Start and Cone Down chart checks, weekly chart checks, scheduling, treatment procedures, image review guidelines, and billing procedures.
Simulation Procedures:
- Explains simulation procedure, including the administration of permanent tattoos for set up purposes.
- Operates equipment and software, including but not limited to CT scanner and software, Treatment planning software (Eclipse), LAP Lasers, IV contrast Power injectors.
- Create immobilization devices, bolus, and accurately documents set up instructions.
- Assists and interacts with all departmental Radiation Therapists, demonstrating the ability to answer all questions and details pertaining to simulation to ensure quality treatment.
- Participates in the IV contrast program, including Patient Screening, and Peripheral IV placement.
- Administers Oral Contrast.
- Assists in scheduling/rescheduling appointments, as needed.
Administrative Responsibilities:
- Assists the Lead Radiation Therapist in the efficient operation of the assigned clinical area. Performs daily QA measures to ensure proficient operation of all imaging and treatment equipment. Performs daily shutdown procedures. Assists in organizing team during equipment failures and notifies patients as well as all functional areas of the department. Assists in resolving clinical problems and difficult set-ups.
- Assumes Lead responsibilities in the absence of the Lead Radiation Therapist, when requested. Assists the Chief and Lead Radiation Therapist to ensure coverage for meeting attendance and administrative duties.
- Strives for continuous improvements and technique standardization.
- Assists with the triage of emergent cases and demonstrates the ability to negotiate the scheduling of emergent cases with other care team members.
- Accurately performs all new start QA measures, weekly chart checks, patient scheduling, treatment delivery, image review, and billing procedures involving the electronic medical record (EMR) system.
- Utilizes the Quality Assurance (QA) program to encompass all aspects of treatment, treatment delivery and quality patient care.
- Accurately documents treatment variances and inconsistencies using the appropriate tools.
- Practices the QA program of each treatment unit and assists with the instruction of new staff members and students as part of orientation.
- Responsible for the safe and accurate utilization of all aspects of the electronic medical record (EMR) and Record and Verify system. Accurately documents all treatment delivery information in the EMR.
- Participates in and appropriately documents the Time Out procedure for every treatment.
- Assists with orientation, teaching, and instructing new staff, students, visitors, and other staff members who are unfamiliar with the department or specific procedures.

OVERVIEW
Supplemental Staffing: Per-diem employees are used to cover staffing gaps due to absences, vacancies, or increased census, but are not guaranteed hours.
Weekend Hours: The Weekend Differential Zone starts at 11:00 PM on Friday and ends at 11:00 PM on Sunday. To qualify for the weekend premium there must be at least 2 hours in the zone.
GUIDELINES
Scheduling Process:
- After the 4-week schedule (graph) is completed, per diem shift needs are posted in Kronos.
- Per-diem staff are expected to self-schedule via Kronos Advanced Scheduling or contact their manager in areas using Basic Scheduling.
Monthly Commitment:
- Minimum of 24 worked hours per 4-week schedule, working 12 hour shifts.
- At least 12 of these hours must be night shifts (eligible for differential).
- At least 12 of these hours must be weekend hours, if available.
- If they work a 12 hour shift on the weekend at night it will meet the commitment requirement.
Holiday Commitment:
- One winter holiday: Thanksgiving, Christmas, or New Year's
- One other holiday: MLK Day, Memorial Day, Independence Day, or Labor Day
- Shifts canceled by the hospital will still count toward holiday commitments.
Cross Department Requirements:
- Per diem scheduling obligations must be met on the employees’ home unit before working in other units.
Failure to Meet Requirements:
- May result in corrective action or status review.
Canceled Shifts:
- Shifts canceled by management will count toward scheduling minimums.
- Working a shift to cover another employee does not count toward required hours.
STATUS CHANGE TO PER DIEM
- Must be approved by the current director/manager.
- Manager of record remains the same unless transferred to a new department.

11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
- Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.
- Assists the physician/provider with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.
- Administers medications, under physician/provider orders, including: immunizations, antibiotics, pain medications, vitamins, topical agents, and insulin. With limitations, initiates, maintains and discontinues IV therapy; administering selected fluids with pre-mixed medications.
- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. About Us – Operating Room
Freeman Health System’s main operating room has nine surgical suites and offers a variety of specialties including gynecology, ear, nose and throat, orthopedics, cosmetic and neurosurgery
In this fast-paced procedural area, the staff uses their experience, knowledge, and skills to work as a team to take patient care to the next level of patient safety and satisfaction
We offer an in-depth individualized orientation process to prepare employees for success in the perioperative setting
What You’ll Do
Under the direction of the nurse, performs clerical and receptionist duties associated with unit operations.
Preferred Requirements
Graduate of medical terminology class preferred.
