ESSENTIAL FUNCTIONS
PEOPLE ESSENTIAL FUNCTIONS
- Communicates in an active, positive and effective manner to all health care team members and reports pertinent patient care and family data in a comprehensive and unbiased manner; listens and responds to the ideas of others. Uses therapeutic communication to establish a relationship with patients and families and communicates the discharge plan, facilitating transitions and hand-offs. Supports patients and families in clinical or ethical issues.
- Uses patient and family-centered approach in collaboration with interprofessional health care team. Facilitates discharge planning activities for assigned patients and collaborates with the case manager and other members of the interprofessional health care team, as well as patient and family. Maintains ownership of the psychosocial component, assessments, diagnosis and treatment, of the discharge planning process on assigned units.
SERVICE ESSENTIAL FUNCTIONS
- Serves as a unit-based participant for comprehensive case management activities including assessing patients and collaborating with team to identify at-risk patients, participating in daily Care Coordination Rounds, and identifying any barrier(s) of efficient patient throughput.
- Develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence and of mental health and/or substance abuse problems in patients and family members.
- Completes a full assessment based on the social work assessment, identifying social determinants of health. Completes screening by patient/family interview, review of the medical record including previous episodes of care, H&P, lab and other test results/findings, plan of care, physician orders, nursing and progress notes. Uses clinical knowledge and screening tools to identify need for case management and/or social work intervention.
- Establishes mutual educational goals with patient and family, provides appropriate resources, incorporating planning for care after discharge.
- Uses knowledge of levels of care, working with patient and family, to ensure discharge disposition is to the appropriate level and facilitates transfers. Provides brief, goal-directed counseling services to assist patients/families to cope more effectively with the transition.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Modifies care based on continuous evaluation of the patient’s condition, using problem-solving and critical thinking, and makes decisions using evidence-based analytical approach. Documentation reflects completed patient screening/assessment and reassessment upon admission and concurrently, as needed. Considers variables that impact treatment plans including diagnosis of emotional, social, and environmental strengths and problems related to their illness, treatment and/or life situation.
- Continuously reviews the patient for opportunities for care facilitation and needs for discharge planning. Works with case manager for routine discharge and anticipates/prevents and manages/elevates emergent situations with specific focus given to discharge plan and elimination of psychosocial barriers.
- Collaborates with staff from the interprofessional health care team concerning safety data to improve outcomes and the safe transition of care through effective patient handoffs
FINANCE ESSENTIAL FUNCTIONS
- Completes timely and thorough assessment on all unfunded patients to identify community resources required for effective transition and able to utilize alternative resources to fill gaps in established community resources.
- Establishes an effective community resource knowledge base and the judgment/ability to effectively select and coordinate available resources, including referrals to regulatory agencies, i.e. CPS/APS.
- Provides timely, efficient pertinent patient information to healthcare team when coordination with significant or intensive resources is necessary for effective discharge planning outcomes and decreased length of stay.
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
- Provides education to unit-based physicians, nurses, and other healthcare providers on community resources and psychosocial impact on care needs.
- Identifies areas for improvement based on understanding of evidence-based practice literature. Assists in evidence-based practice/performance improvement projects based on observations by offering solutions and participating in unit projects and activities.

Phlebotomists are responsible for collection and procurement of blood and human specimens in the clinical setting. Position Details:
As a phlebotomist, you'll support Geisinger’s mission of providing exceptional patient care through innovative, efficient, and high-quality laboratory services. At Geisinger, our phlebotomists are the friendly faces that collect samples to be analyzed by our state-of-the-art labs. Supporting a varied patient population, you’ll be responsible for venipunctures/skin punctures on patients in our inpatient setting.
There are many other tasks our phlebotomists complete, such as troubleshooting physician orders, processing specimens for testing, as well as aiding our patients.
We have many different skill levels in our labs, from new to experienced phlebotomists, and all skill levels are well utilized. Geisinger employs phlebotomists in roles I, II, and III. We also have numerous staff members who pursue their ASCP certification and become trainers for our department!
Phlebotomist I, II and III qualifications will be considered at appropriate compensation. Salary commensurate with experience.
- Experience as a nursing assistant (NA), certified nursing assistant (CNA), medical assistant (MA), certified medical assistant (CMA) or similar will be considered!
- Military experience will be considered. US Army 68W Combat Medic, 18D, US Navy Hospital Corpsman, and related medical specialties.

There are many other tasks our phlebotomists complete, such as troubleshooting physician orders, processing specimens for testing, as well as aiding our patients.
We have many different skill levels in our labs, from new to experienced phlebotomists, and all skill levels are well utilized. Geisinger employs phlebotomists in roles I, II, and III. We also have numerous staff members who pursue their ASCP certification and become trainers for our department!

Position Summary
The Pediatric Cardiac Services PA provides direct patient care and assumes primary responsibility for clinical management across ambulatory, cardiac catheterization lab, and inpatient settings. - Human Resources: Onboarding - conduct financial analysis to determine availability of funding, assist in the development of job descriptions, post positions, approve salary offers, process visa applications and payments as appropriate. Ongoing - process merit, equity and promotions requests. Manage salary allocations in adherence to funding guidelines and availability. Offboarding - manage termination process checklist, process termination paperwork.
- Compliance: Partners with faculty and researchers to ensure adherence to internal and external policies. This includes consulting with faculty/OGC/Innovations on MTA, DUA and consulting agreement processes. Also includes advising faculty and researchers and/or referring them to appropriate resources for other compliance issues such as financial conflict of interest and foreign influence. Educate new researchers and manage the federal time and effort process for applicable researchers. Continuous learning and educating of researchers on NIH Grants Policy Statement and DFCI Policies. Review Time and Effort prior to routing to researchers for certification.
- Cores, Centers and Chargebacks: Assist in pricing development and annual review of pricing, process monthly billing for internal and external customers, manage funds to ensure revenue meets expenditures.

Oversight of the clinical trials start-up process from receipt of protocols through the Scientific Review Committee and IRB submissions, and site activation activities.
Responsible for data reporting and management, collection of source documents, use and development of case report forms, adverse events reporting, filing, and archiving of study records, and resolution of data queries.
May prepare and or complete regulatory related reports and IRB submissions. Maintain and organize study participant/subject trial binder and regulatory binders, enter all required study data on an ongoing basis and as applicable.

The department cares for a wide range of conditions, from non-emergent complaints to life-threatening illnesses and injuries. A dedicated split-flow area supports efficient evaluation and treatment of fast-track patients with lower acuity needs. Additionally, the ED includes a behavioral health intake area designed to safely assess and manage patients presenting with mental health and behavioral health concerns.

Description:
Assist with patient admission, orientation to room and safety features, and securing valuables.
Obtain vital signs, apply cardiac monitors, and perform EKG testing with proper lead placement.

Job Duties:
Responsible and accountable for performing a wide range of clerical duties and direct patient care activities, depending on the need of the unit. The performance of these tasks in an efficient and timely manner is essential to enhance the care team's ability to provide quality care. Meets the qualifications to provide care for patients in the specific age range in their assigned clinical area.
- Provides patient care as directed, within the nursing assistant scope of practice, in meeting the multidisciplinary care plan established for each patient.
- Identifies, observes, and documents the physical needs of the patient according to established standards. Reports ant changes in patient condition to licensed personnel in a timely manner.
- Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
- Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
- Provides one to one patient safety watch as assigned following established policies and procedures.
- Accurately processes specimens and provides timely transport of the specimen to the laboratory.
- Performs point of care patient testing and point of care instrument maintenance and quality control.
- Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
- Serves as a preceptor and mentor for new staff and students in a positive manner.
