As a patient care tech at MercyOne, you will participate in the provision of safe, quality patient care as delegated by the Registered Nurse through support of standards and philosophies of Genesis Medical Center. Provides assistance to nursing staff and physicians in various patient care and non-patient care activities. Performs tasks in a caring and efficient manner that supports customer service and assists with department related housekeeping, and supply/equipment/facility maintenance tasks.

The Mother Baby Unit is a 20 bed, all private post- partum unit offering 24-hour rooming in and individualized teaching. We are a Baby Friendly designated hospital with specially trained nurses that provide lactation support, and a lactation specialist is available seven days a week. In addition, we provide surgical procedures and triage for GYN patients. St. Joseph's Health is one of only two hospitals in New York State to be recognized in Newsweek's 2021 list of Best Maternity Hospitals. Nationally, only 217 facilities met Leapfrog safety standards for excellence in maternity care by following important protocols to protect moms and babies and maintaining low rates of C-section, episiotomy and early elective deliveries. Specialized training is provided for all aspects of obstetrical and GYN care.

Responsibilities:
Responsible for collaboration, communication & facilitation of optimal family centered provision of care, family/caregiver education, goal setting, and critical thinking.
Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
Provide quality patient monitoring, med/surge nursing interventions and hands on skills including monitoring mother & infant, post-surgical care, respiratory support and more.
Collaborative team approach: Our interdisciplinary team includes, OB providers, certified nurse midwives, registered nurses, lactation consultants, social workers, and spiritual care.
Ensures quality nursing care is rendered to all patients in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards of care and practice.

Education, Training, Experience, Certification and Licensure:

What you will do:

What you will do:

- Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Position Purpose:
The PCA, under the direction of a registered nurse, is responsible for direct patient care considering age specific, developmental, cultural and spiritual needs. They assist in establishing and maintaining a patient care environment that is clean, safe and conducive to patient/family wellbeing including concepts of relationship-based care.
Training is provided to prepare our colleagues for success. The PCA assists patient in completing ADLs including: Bathing, showering, toileting, positioning, turning, transferring, ambulation using assistive devices and with feeding and menu selection. They are responsible to record patient information and required data in appropriate areas in order to meet documentation requirements.
The Inpatient Rehab units at Chelsea Hospital care for patients requiring comprehensive inpatient rehabilitation after traumatic brain injuries, cerebral vascular accidents, amputations, debility, poly trauma or spinal cord injuries. Inpatient Rehab is a collaborative program between Chelsea Hospital and University of Michigan Health, staffed by a team of award-winning U-M Health rehab physicians and Chelsea Hospital nurses, therapists, and specialists.
Our Inpatient Rehabilitation unit has recently expanded to a 40-bed unit and is now accepting higher acuity patients. As part of our commitment to excellence in patient care, we are proud to be participating in the Spinal Cord Injury (SCI) Model System.
Shift Available: Days – 6:30am-7pm
Status Available: Full time, benefit eligible (72 hours per pay period – every two weeks)

This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team.
Major Responsibilities:
Team Navigation: Assists the leadership with daily operations and team performance by guiding the team to meet individual and team goals, high standards of productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment.
Patient Support: Provides immediate emotional support and resources to hospitalized patients following a trauma, including connecting them with the clinical team when appropriate, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. Also, complete assessments and facilitate warm-handoffs to providers for ongoing care.
Resources Navigation: Responds to trauma alerts, safety calls, pre-escalation needs and collaborates with street outreach teams and facilitates mediation to mitigate retaliation risks.
Outreach: Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources.
Continuity of Care: Schedules patient appointments with the TRC, other internal Advocate Health resources, or community partners before discharge and provides brief follow-up calls after discharge when necessary, to ensure ongoing care which requires strong collaboration with other providers.
Assessments: Conducts intakes and needs assessments, linking patients to appropriate resources and outpatient services. It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital.

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated. - Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately. - Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. - Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system.
- Facilitates communications required to support patient care, including the multiple-line telephone, unit/site paging and the hospital information system. Screens incoming telephone calls. Monitors department office equipment and troubleshoots minor problems. Arranges repair of major malfunctions.
- Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. Verifies medical records contain complete and accurate patient demographic information, patient identification and registration in the medical record, including appropriate level of care.
- Establishes and maintains positive customer relations and contributes to a harmonious work environment. Quickly and courteously acknowledges requests, responds to call lights, demonstrates proper phone etiquette and fosters communication between the health care team.

Pay Range:
$35.50 - $53.25Essential Functions
Reviews physician requisition orders and instructions concerning patient preparation for ultrasound procedure.
Communicates to OB/GYN nursing team status of patient condition pre and post procedure.
Performs ultrasound imaging for fetal diagnosis and anomaly detection for patients with high-risk indications.
Performs ultrasound guidance for gynecological sonography including doppler and obstetrical studies.
Performs ultrasound studies for diagnosis including but not limited to assessment of fetal growth, development and age, evaluation of fetal anomalies and abnormal AFP screening.
Documents procedure performed on patient medical record.
Assists with administrative duties as requested.
Physical Requirements
Requires standing, stretching, bending, pulling and pushing patients, supplies and moving ultrasound machine. Must have the ability to grasp necessary transducers to use for sonography. Digital dexterity to enter data on ultrasound keyboard is necessary. Ability to work in a fast-paced setting and ability to handle stressful situations with empathy and professionalism is a must. Must speak English fluently. Exposure to communicable diseases.
Education, Experience and Certifications
Current license Registered Diagnostic Medical Sonographer required with a specialty in OB-GYN. 2 years' experience in OB-GYN required. Competency in Nuchal Translucency and Nasal Bone Imaging may be required based on department.

Position Purpose:
The PCT, under the direction of a registered nurse, is responsible for direct patient care considering age specific, developmental, cultural and spiritual needs. They assist in establishing and maintaining a patient care environment that is clean, safe and conducive to patient/family wellbeing including concepts of relationship-based care.
Shifts Available: 6:30 am-3:00pm; 2:30pm-11:00pm, 10:30pm-7:00am
Status Available: Full-time, Part-time
Every Other Weekend and Holiday per year required for Full-time and Part-time team members.
REQUIRED EDUCATION / LICENSURE
- High school diploma or GED
- Minimum of one year of previous direct care experience (acute or long term) or successful completion of CENA, PCT, MA, or EMT course.
- Student nurses must have completed their nursing fundamentals class of a BSN program.
What you will do:
- Training is provided to prepare our colleagues for success.
- The PCT assists patient in completing ADLs including: Bathing, showering, toileting, positioning, turning, transferring, ambulation using assistive devices and with feeding and menu selection.
- A PCT will collects and send specimens including blood and blood cultures and inserts and removes peripheral IV catheters. They are responsible to record patient information and required data in appropriate areas in order to meet documentation requirements.
- Critically ill patients recovering from acute MI, Cardiogenic shock, CHF, cardiac arrest, and various percutaneous interventions; Cardiac patients suffering from Heart attack, heart failure, arrythmias, and various percutaneous interventions, in a progressive care setting.

- Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
- Checks in and registers patients; obtains and verifies complete demographic, guarantor, and insurance information; discusses and collects co-pays and other out-of-pocket patient responsibilities. Obtaining accurate information at the point of registration helps ensure timely payment to the organization and prevents billing issues and patient complaints. Maintains complete confidentiality regarding patient personal/financial information and medical records in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
- Knows insurance basics and recognizes commercial and government plans. Understands which plans Advocate Health contracts with and when a statement of financial responsibility is needed. - Operates x-ray equipment to produce radiographs of designated portions of the body as ordered by a physician
- Accountable for the performance and documentation of all diagnostic procedures utilizing proper technical parameters to produce quality studies.

- Proficient in the performance of general diagnostic procedures. If applicable to home site, is also proficient in general fluoroscopy procedures, including GI and Pain procedures.
- Adheres to infection prevention, isolation and sterile techniques as it applies in patient handling and delivery of care
- Adheres to ALARA principles in performance of exams to reduce exposure to self, the patient and others
- Utilizes proper patient identification to ensure the correct exam is performing on the correct patient at the correct time.
- Obtains and records pertinent patient history, explains the procedure to patient in a manner to ensure understanding, ensures patient comfort during performance of exam.
- Adherence to department standards of radiographic quality.
- Reviews radiographs to ensure optimum quality.
- Reports environmental/equipment problems to appropriate resource personnel.
- Performs required computer function for RIS/HIS systems.

Knowledge, Skills & Abilities Required:
- Ability to multi-task, route calls, greet patients, and maintain a schedule of appointments/meetings.
- Proficient in the use of Microsoft Office (Excel, Power Point, Access and Word) or similar products, including maintaining, tracking, and entering data in a database and/or spreadsheet software.
- Strong organizational skills and attention to detail.
- Ability to effectively communicate (written, and verbal), build rapport, and relate to all people.
- Ability to operate standard office equipment.
Physical Requirements and Working Conditions:
- Ability to sit, walk, stand, bend, and lift frequently throughout the workday.
- Must be able to occasionally lift items weighing up to 20 lbs.
- Must have functional speech, vision, and hearing.
- Operates all equipment necessary to perform the job.
- Exposed to a normal office environment.
