As a Resident Assistant at Hawthorne Ridge, you'll play a vital role in enhancing the lives of our Resident’s. This is an incredibly rewarding position where you can make a real difference in the lives of those who need it most.
The team provides care in a residential setting, thoughtfully designed for individuals with memory loss, Alzheimer’s disease and other related dementias. Eddy Hawthorne Ridge is a 66-bed assisted living community on 14 beautiful acres in East Greenbush, New York.

Position Summary: The Resident Assistant, or RA, is a caregiver providing assistance to residents under the supervision of the LPN and/or RN. Resident Assistants provide compassionate care that is respectful of each resident's needs, values and wishes.
Job Specific Competencies
Principal Responsibilities:

- Supports the Trinity/SPHP Vision, Mission, Values in all encounters with residents and families
- Adheres to the SPHP Code of Conduct
- Demonstrates accountability as an individual and team member in meeting basic resident and family needs
Clinical Competencies:
Resident Assistants provide the following nursing services under the supervision and direction of an LPN or RN across the continuum of care at SPHP.
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting and mobility in a respectful, compassionate manner.
- Assist residents in taking medications as needed.
- Encourage and support resident independence and dignity.
- Monitor resident safety and well-being, reporting any change to the LPN. Consult with the LPN/RN on any changes of health statuses of residents.
- Build meaningful relationships with residents, understanding their personal histories, preferences and routines and anticipate the needs of residents.
- Help with light housekeeping and meal service as needed to maintain a safe and clean-living space.
- Participate in team meetings and communicate/collaborate with coworkers and supervisors.
- Promote a warm, friendly, and respectful environment.
Education Requirements:
- Must possess patience, empathy and a genuine desire to help others
- High school diploma or GED preferred.
- Previous experience in senior care, health care or assisted living preferred, but not required
Must be able to:
- Ability to follow care plan and safety protocols
- Working knowledge of computers including electronic health record, checking email, and preforming required in-servicing, along with use of other required applications
- Have the physical, visual and auditory ability adequate to perform resident care
- Maintain composure and positivity in a high stress environment
- Promote physical, cognitive, and psychosocial well-being of each resident
- Work with residents in a highly personal manner during stages of acute and chronic illness
Pay Range: :$16.70 - $20.36 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

- Greets patients arriving for their appointments. Monitors patient flow to ensure patients are cared for in the most efficient and courteous manner.
- Ensures all patient demographic and insurance information is complete and accurate
- Completes the registration process on walk-in patients, verifies and / or updates patient demographic and insurance information if changes or additions have occurred

- Performs visit closure, including but not limited to checking out patients, scheduling follow-up appointment(s), collecting additional patient responsibility (when applicable) and providing patient with appropriate documents.
- Maintains knowledge of and reference materials of the following: Medicare, Medicaid and third-party payer requirements, guidelines and policies, insurance plans requiring pre-authorization/referral and a list of current accepted insurance plans.
- Proactively communicates issues involving customer service and process improvement opportunities to management
- Meets productivity requirements to ensure excellent service is provided to customers
- Meets or exceeds performance expectations of 98% accuracy rate and established department productivity measurements.
- Maintains excellent public relations with patients, families, and clinical staff as well as demonstrates a willingness and ability to work collaboratively with others for concise and timely flow of information

- Ability to identify and understand issues and problems. Examines data and draws logical conclusions based on information available
- Knowledge and ability to articulate explanations of Medicare, HIPAA, and EMTALA rules and regulations and comply with updates on insurance pre-certification requirements
- Mathematical aptitude, effective oral and written communication skills and critical thinking skills
- Understanding of basic human anatomy, medical terminology and procedures for application in the patient referral, pre-certification and authorization processes.
- Ability to speak effectively to customers or employees of the organization; presents a pleasant, professional demeanor and image during telephone conversation
- Ability to handle sensitive and confidential information according to internal policies
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
- Experience with Microsoft Outlook, Word and Excel and ADT software
- Ability to write routine correspondence, calculate figures and amounts such as discounts and percentages
- Must be able to work with minimal supervision, to problem solve in a high profile and high stress area and interact positively with all internal and external customers while possessing the ability to determine priority of work

Resident Assistants provide the following nursing services under the supervision and direction of an LPN or RN across the continuum of care at SPHP.
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting and mobility in a respectful, compassionate manner.
- Assist residents in taking medications as needed.
- Encourage and support resident independence and dignity.
- Monitor resident safety and well- being, reporting any change to the LPN. Consult with the LPN/RN on any changes of health statuses of residents.
- Build meaningful relationships with residents, understanding their personal histories, preferences and routines and anticipate needs of residents.
- Help with light housekeeping and meal service as needed to maintain a safe and clean living space.
- Participate in team meetings and communicate/collaborate with coworkers and supervisors.
- Promote a warm, friendly, and respectful environment.
Education Requirements:
- Must possess patience, empathy and a genuine desire to help others
- High school diploma or GED preferred.
- Previous experience in senior care, health care or assisted living preferred, but not required

Pay Range: :$16.70 - $20.36 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

The Physical Therapist will be working in our Inpatient Acute Care Unit collaborating with providers to treating patients in hospital setting. The PT will conduct evaluations, perform treatments, and provide recommendations to improve mobility and prepare for discharge.

- Evaluates, directs, plans, and treats to help clients achieve a maximum level of functional independence.
- Work in a fast-paced environment with ongoing collaboration and communication with staff, patients, and families.
- Develops and carries out a variety of techniques and modalities to provide individualized treatment appropriate to established goals.
- Modifies treatment plans and goals due to changes in client status and capabilities.
- Provides appropriate information regarding client status to patient, family, treatment team and other professionals in a clear and concise manner.
Requirements:
- Graduate of an accredited program in Physical Therapy
- Current State of CT license.
- Must have the ability to communicate effectively both orally and in writing. This may include explaining clinical, technical and diagnostic procedures to clients and their families, counseling patients and their families and enforcing policies and regulations in difficult situations.

Performs laboratory testing of biological specimens in an acute setting in any/all specialty areas using manual or automated techniques. Uses independent judgment and cognitive skills to report timely and accurate laboratory results.
Major Responsibilities:
- Performs, evaluates, and monitors laboratory tests as authorized within established protocols, selecting and performing additional procedures when indicated. Monitors workflow and makes necessary adjustments to complete work in an appropriate timeframe.
- Evaluates results for clinical significance, validity and appropriateness. Follows prescribed policy in the reporting and notification of test results.
- Performs and documents calibration and maintenance of instruments and equipment. Performs and documents quality control; evaluates quality control results and takes corrective action when indicated.
- Recognizes indicators of test and/or instrument malfunction and makes basic corrections by using prescribed strategies, or notifies the Medical Technologist, site coordinator or supervisor. Performs internal and external proficiency testing in the same manner as patient samples.

- Provides direct support and timely, accurate laboratory results for critical care departments such as the ED, ICU and OR during a high acuity patient event. This includes transfusion services, attending codes, performing testing on irretrievable samples, tissue grinding and gram stains, mass casualty procedures, and mass transfusion procedures.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

Position Purpose

Required:

Operational Integration & Performance
- Directs clinical and administrative operations across assigned care locations to ensure consistent execution of service line, divisional, and enterprise priorities.
- Exercises direct operational authority, in partnership with the service line leadership team, to establish operational plans, set performance expectations, and implement management routines across practices.
- Accountable for adherence to approved operating and labor budgets, monitoring operational performance, identifying risks and variances, and directing corrective actions to achieve financial and operational targets.
- Drives integration across care settings and specialties to improve access, capacity, growth, and operational consistency.
- Monitors key performance indicators, including throughput, quality, experience, and financial metrics, and leads operational response to performance trends and location-based initiatives.
Leadership of Frontline Management
- Provides direct leadership to managers and supervisors, including coaching, development, and performance management, in partnership with Advocate Health Acute, Ambulatory, and Virtual Nursing and other groups as needed.
- Establishes clear leadership expectations and holds frontline leaders accountable for operational performance, quality, teammate engagement, and patient experience.
- Directs recruitment, onboarding, development, and resolution of teammate concerns through frontline leaders to maintain a strong and effective work environment.
- Ensures compliance with organizational policies, regulatory requirements, and standards of performance across assigned areas.
Clinician Partnership & Engagement Support
- Partners with clinician leadership and the service line leadership team to align operational execution with clinical priorities and care delivery needs. Exercises direct operational authority to remove barriers, coordinate stakeholders, and implement workflow, scheduling, and resource decisions that support clinician effectiveness and patient care.
- Supports clinician recruitment, onboarding, retention, and engagement through operational leadership and administrative coordination, while clinician leadership retains accountability for clinician practice and professional standards.
- Optimizes coverage models, scheduling, and resource allocation across sites in partnership with enterprise, divisional, local, and clinician leaders.
Quality, Improvement & Workforce Oversight
- Leads continuous improvement efforts focused on quality, safety, access, efficiency, and patient experience, including root cause analysis, corrective action planning, and follow-through on performance gaps.
- Uses data, benchmarking, and performance reviews to identify risks, improvement opportunities, and operational priorities across assigned locations.
- Directs workforce planning, staffing models, recruitment support, and performance expectations through frontline leaders using Advocate Health and Clinical Services operating standards.
- Designs and directs the work of assigned areas, including talent development, succession planning, feedback, recognition, and performance improvement.
- Ensures understanding of and compliance with the organization’s Code of Ethical Conduct and applicable policies, regulations, and laws.
