We use Open Dental, an Itero Scanner, a CBCT 3D Bone Scans for full mouth treatments. We have ultrasonic and laser for hygienist and dentist. We also love using our portable Xray unit.
*We are seeking a part time hygienist* 2-3 days/wk. Days are negotiable. Hours for our office are 8am-5pm but we are willing to be flexible and have options for earlier start and release times. We are also open to hygienist seeking assisted hygiene but do NOT require it of our hygienist. Our work schedule is currently rotating weeks of M-Th, T-F (8am-5pm or 7am-4pm) with an hour lunch.

What You'll Do
Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria's Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
Your Qualifications
Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty. New graduates who are board-eligible and actively pursuing licensing are encouraged to apply. · Graduate from an AMA/NCCPA/AAPA accredited Physician Assistant Program, or, an Accredited Nurse Practitioner Program.
· Current licensure with New York State Department of Education. Maintain appropriate AAPA/ANA certification.
· Must work and communicate effectively and collaboratively with: patients, families, Family Health Network staff, hospital personnel, EMS, police, fire department, community schools and other community groups and organizations.
*Primary Function: *The Physician Assistant (P.A.) / Nurse Practitioner (N.P.) provides primary diagnostic and therapeutic care to patients in multiple settings.
*General Summary of Duties:*
Provide evidence-based, empathetic, and patient-centered primary care to patients. Provision of care within accordance with Bylaws, Rules, Regulations, and policies/procedures of the health centers; including all applicable state and federal laws, rules, regulations, codes, directive and memoranda; conditions of participation in Medicare, Medicaid and other financial reimbursement or grant providers; the directions of the administrator, senior physician or staff physicians. Shall provide such care in accordance with the current ethical and professional standards of the AMA, AAPA, AAFP and AAP.
* Though care will primarily be provided in the clinic setting, patient needs may require care setting to be hospital or home based at times.

As part of the patient care team, the PA/NP will engage patients in their own health care. This will include supporting patients and their families in self-management and positive behavior change. This will require good communication which is both open and supportive.
· This position requires compliance with Health Center’s written standards, including its Standards of Conduct and policies and procedures pertaining to all operational aspects, furthermore this position is required to report any suspected instances of noncompliance.
· This position will function as a contributing member of the FHN Dramatic Performance Improvement Model of Care to ensure that FHN’s Team-Based Care is the most sustainable, efficient method for providing high-quality, satisfying, and equitable health care to our patient population.
· This position will function as a contributing member of the emergency preparedness team under the incident command system.
