Performs diagnostic sleep studies related to the evaluation and treatment of patients with sleep disorders.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Must have completed or have current enrollment in one of the following education programs:
Board of Registered Polysomnographic Technologists (BRPT) approved Sleep Technology Approved Resource (STAR) designated focused education program
Accredited Sleep Technologist Education Programs (A-STEP) on-line self-study modules
A-STEP 80-hour Introductory Course
Polysom program through the Commission on Accreditation for Respiratory Care (CoARC) or Commission on Accreditation of Allied Health Education Programs (CAAHEP)
2. Registered Polysomnography Technician (RPSGT) through the Board of Polysomnographic Technologists or must obtain within 30 months of hire.
3. Obtain certification in Basic Life Support within 30 days of hire date
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Associate degree in an Allied Health Field
2. Certified Polysomnographic Technician (CPSGT) upon hire
3. ACLS Certification within 90 days of hire
EXPERIENCE:
1. One (1) year of health care experience
2. One (1) year of Sleep Lab experience
CORE JOB DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Coordinates and prioritizes scheduling of both inpatient and outpatient procedures while ensuring proper document flow and storage according to established departmental guidelines.
2. Assures the completion of all patient testing procedures within established time frames, as observed by supervision.
3. Documents procedures performed in the patient chart within established guidelines, as observed by supervision.
4. Files and completes necessary departmental documentation in an accurate and timely fashion to provide necessary historical data, as monitored by supervision.
5. Submits accurate and timely patient charges, as monitored by supervision.
6. Obtains necessary information from the clinical record, requisition, or by directly questioning the patient to enable the technologist to be aware of any special circumstances involved and to achieve a quality study to facilitate the completion of a final report, as monitored by supervision.
7. Performs sleep studies in accordance with the policy and procedure guidelines.
8. Assures the accurate recording of the electrical impulses of the neurological system
9. Prepares initial data for use by the physician in the formulation of sleep reports according to established departmental guidelines as monitored by supervision.
10. Informs the patient and family regarding the nature and significance of the procedure to be performed, as observed by supervision.
11. Accurately observes, records, and reports all patient concerns, symptoms, reactions in a timely manner, as monitored by supervision.
12. Demonstrates the knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs, and to provide the care needed as described in the department policies and procedures.
13. Maintains orderliness and cleanliness of the equipment, procedure rooms, and work areas of the Sleep Evaluation Center.
14. Assists in maintaining a daily stock of clean linen and necessary supplies for the patient testing rooms of the Sleep Evaluation Center, as monitored by supervision.
15. Participates in the cleaning and maintenance of all equipment, detecting instrument malfunctions, making minor adjustments, and reporting the need for any repairs in accordance with policy and procedure guidelines as monitored by supervision.
16. Cleans and prepares necessary items for the proper sterilization techniques strictly adhering to policy and procedure guidelines and infection control standards 100% of the time as monitored by supervision.
17. Attends educational inservices and workshops on topics related to the services of the Sleep Evaluation Center providing information upon their return so as to share the knowledge gained with other related personnel, as observed by supervision, and to continuously work towards increasing the professional level of patient care rendered and acquiring a better understanding of the services and functions of the Sleep Evaluation Center.
18. Reviews educational materials on hand on a self-paced basis and meeting other standards of academic performance determined by the department, as monitored by supervision.
19. Communicates with co-workers and staff to answer any questions as they arise, as monitored by supervision.
20. Maintains Continuing Education (CEU) requirements as determined by the department, as monitored by supervision.
21. Assists physicians with related research activities.
22. Performs study or assists with the performance of the research activity as per specific protocol guidelines maintaining proper documentation, as monitored by supervision.
23. Assists in providing inservices regarding the principles of sleep procedures and equipment operation to members of the professional medical community.
24. Assists in the instruction and hands-on technical training of trainees, house-staff, and medical staff from both within the institution and from the outside professional medical community providing inservices on the principles of sleep procedures and equipment operation.
25. Provides information to other healthcare workers and the general public, as monitored by supervision.

This position has the authority, responsibility, and shift accountability for the delivery of nursing care through the use of the nursing process. The incumbent accepts accountability, applies competent clinical knowledge skills, and uses independent nursing judgment for care provided by self and by health care providers to whom care is delegated to achieve quality patient care outcomes. Performs the duties and responsibilities within the scope of a Registered Nurse (RN) - Level I, as defined within the job description.

MINIMUM QUALIFICATIONS:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

A. POSITION SUMMARY
This individual is principally responsible for intervening on behalf of children, adolescents, and adults of the Desert Sage Health Centers. (DSHC) patient mix, who have suffered neglect, physical, sexual, and/or emotional abuse, or suffer from emotional disturbances or mental illness. The Licensed Mental Health Provider is responsible for assessing, and if appropriate, treating and connecting these individuals in need of services to appropriate DSHC departments, programs, and/or to other agencies and sources around the community which could meet the individual needs of the identified client.

Reports and works under the Behavioral Health Director
C. PRIMARY DUTIES AND RESPONSIBILITIES
1. Conduct a psychosocial assessment of the client and determine if the client meets criteria for Behavioral Health services.
2. Responsible for learning and implementing the DSHC electronic medical records (EMR) system following heath center policies.
3. Diagnose client using DSM-VTR criteria, and if appropriate, psychometric tools and records.
4. Develop a treatment plan that identifies both short and long term need of the client.
5. Provide individual, couple, family, and group treatment services, when appropriate.
6. Develop and implement group counseling/interactive sessions, as appropriate.
7. Conduct crisis intervention and coordinate follow up care, including hospitalization referral, if needed.
8. Triage with nurse and providers in coordinating patient care.
9. Serve as a consultant to clinicians though Behavioral Health integration.
10. Advocate for the client or family in obtaining needed services from appropriate agencies.
11. Make appropriate client referral to community agencies. Coordinate the delivery of these services, assess client progress and evaluate outcomes, as appropriate.
12. Network with community service providers to help assure the availability of services.
13. Awareness/acceptance of cultural competency aspects and sensitivity.
14. Maintain appropriate records, execute proper forms, and protect client confidentiality.
15. Provides quality service by informing the client of rules, regulations, and legal requirements related to care and by documenting the client’s situation and own actions.
16. Responsible for following and implementing HIPAA, OSHA, and corporate compliance policies and procedures.

D. MARGINAL DUTIES
1. Participate in coalitions, groups and organizations that are working toward goals that are beneficial to the rehabilitation of the mentally ill, when appropriate
2. Provide community education and consultation services to increase awareness of mental illness and to make clients aware of services available
3. Provide consultation/education or limited supervision to students, entry-level case managers, or volunteers

E. PERFORMANCE CRITERIA
1. Complete required documentation in accordance with legal and Medicaid requirements and DSHC policy and procedures
2. All documentation indicates the client’s progress, services provided, attainment of client goals and discharge referral information
3. Records progress notes pertaining to individual and group counseling sessions in a legible, coherent and timely fashion, using the electronic health record (EHR) system following health center policies
4. Reports to work punctually during assigned working hours; when a problem necessitates absenteeism or tardiness, the supervisor is notified immediately
5. Complies with all DSHC policies and procedures as well as Federal and State legal requirements by studying existing and new legislation; enforcing adherence to requirements; informing management of needed actions
6. Adheres to a professional code of conduct and advocates for professional values
7. Complete 20 hours of ongoing training per year, or as required by state licensing board. (Including meeting participation and conferences.)
8. Familiarity/adoption of Meaningful Use (MU) criteria, completing accurate charting, data collection, orders/tracking/flowsheet/template documentation. Responsible for implementing PCMH Model of Care including Standard Elements/Factors.
G. MINIMUM QUALIFICATIONS
1. Licensed to practice in the State of Idaho
2. LMSW (Licensed Master’s of Social Work)
3. LPC (Licensed Professional Counselor)
4. Previous preferred experience in individual assessments/interventions and family/group therapy.
H. PREFERRED QUALIFICIATIONS
