VytlOne (formerly Maxor National Pharmacy Services) is offering an exciting opportunity for an experienced Part Time Staff Pharmacist to work at McKeen Pharmacy in Sandusky, OH, a community healthcare center based, in-house pharmacy. This is a unique and rewarding opportunity for a pharmacist with a retail pharmacy background and an active OH pharmacist license. APhA or ASHP Immunizations Certified is a plus.

Family Health Services mission is to provide high quality care to those who may be struggling to make ends meet. They help you overcome health concerns through compassionate care with the most experienced, skilled, and emphatic team of health professionals. They accept Medicare, Medicaid, and most commercial insurances. Additionally, they use a sliding scale for those who qualify. The Sliding Fee Scale can help those with no insurance or to help supplement commercial insurance. Interpret and fill drug orders.
Provide patient information and counseling.
In the absence of store Pharmacist-In-Charge/Pharmacy Manager, may be required to act as the alternate store leader, responsible for managing ongoing functions of the pharmacy.
Contact and confer with physicians regarding questions or irregularities on prescriptions.
Dispense prescriptions, compound, and prepare I.V. admixtures (if applicable).
Monitor drug therapy for contraindications, interactions, allergies and appropriateness.
Maintain patients’ medication records.
Maintain inventory and records of controlled drugs and other drugs as required by law.
Supervise pharmacy technicians in routine performance of their duties.
Provide drug information to professional staff and patients as necessary.
Participate in inventory control programs such as ordering, inventorying and monitoring correct pricing and quantities.
Attend and participate in programs, committees, meetings, and functions as required by the facility or pharmacy.
Participate in educational programs for professional staff as necessary.
Participate in pharmacy orientation programs, pharmacy staff meetings and in-service educational programs.
Participate in activities resolving unsafe and unsanitary practices.
Exemplify excellence in customer communications and service.
Work cooperatively with other staff members to enhance the team concept.
Participate in the Performance Improvement Program.
Maintains regular attendance in accordance with established policies.
Promptly report any allegations of impropriety to the Compliance Department.
Comply with Maxor’s Ethical Business Conduct policy and Maxor’s Compliance Program.
Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
Complete required training and ensure the departmental staff training is completed as required.
Must be able to cope with the mental and emotional stress of the position.
Ensure staff is informed of product upgrades or enhancements in a timely manner.
May assist management with hiring and training competent staff.
On a biweekly basis, verify departmental timesheets for departmental employees ensuring the timesheets are accurate based on the work-schedule and necessary corrections are done prior to Payroll submission.
Develop and mentor departmental staff by providing career coaching; motivate departmental staff, individually and as a group; understand and support diversity; and establish an effective team.
May assist with conducting initial and annual performance evaluations.
Promote teamwork; discipline staff when warranted.
Education:
Bachelor’s degree, or Pharm. D., from an approved school of pharmacy and registration by the Ohio state board of pharmacy
One-year experience as a practicing pharmacist is preferred.
Knowledge, Skills and Abilities:
Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
Ability to communicate technical information to non-technical personnel.
Knowledge of polices and regulations pertaining to the disbursement of pharmaceutical drugs.
Ability to maintain quality, safety, and/or infection control standards.
Ability to supervise and train assigned staff.
Knowledge of related accreditation and certification requirements (if applicable).
Skill in the use of personal computers and related software applications.
Ability to assume essential responsibilities and duties in the absence of the Pharmacy Manager

POSITION SUMMARY: As a key member of the Northpoint team, the Adolescent Therapist/Counselor will oversee the therapeutic services provided to a caseload of clients. This individual will act as the treatment plan coordinator for the multi-disciplinary team in the development and implementation of the master treatment plan and the discharge plan for each client. The Adolescent Therapist/Counselor will provide individual, group and family therapy for assigned clients, as required by contractual and licensure obligations. This person will actively communicate with support staff, treatment teams, and outside agencies on treatment and safety issues pertaining to clients and their family members. The Adolescent Therapist/Counselor will complete all documentation associated with these duties in a professional and timely manner.
ESSENTIAL RESPONSIBILITIES AND DUTIES:
Provide counseling, assessments and group facilitation to individuals and families
Coordinate all aspects of direct clinical care from admission to discharge
Consult with the multidisciplinary treatment team to provide quality client care
Provide orientation and education to clients, families and others to aid in the therapeutic process
Collaborate with assigned clients in the development of written treatment plans; include client and family involvement in assessment, treatment, education and discharge planning
Monitor clients on an ongoing basis to continually assess needs and facilitate treatment and interventions
Document clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality client care
Actively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with employees and peers
Maintain professional boundaries with clients at all times
Oversee and participate in the quality assurance and compliance activities of the facility
Provide clinical updates to utilization review and representatives of managed care to best coordinate reimbursement of treatment services

*RESPONSIBILITIES: *The Clinical Therapist provides professional guidance and direction to maintain and improve the quality of the service delivery in effectively meeting the needs of the participants in placement at New Day group homes. This position requires the ability to manage time and work schedules effectively. Maintain the goals and objectives of New Day, Inc. and communicate to the Administrator while ensuring compliance with state licensing codes and federal standards. It may be necessary to work occasionally evenings and weekends. Additional duties may be assigned.

*MAJOR FUNCTIONS:*
1. INTENSIVE LEVEL OF CARE
A. Maintain case management of a caseload of up to 12 participants using active treatment principles.
B. Act as Treatment Team Leader for weekly internal staff meetings.
C. Ensure that each resident receives 75 minutes of therapy and 75 minutes of therapeutic intervention services per week. Individual therapy must be provided at least 50 minutes out of the required 75 minutes per week and family therapy must be provided to the youth and biological, adoptive, or foster family members with whom the youth previously resided or plans to reside with upon discharge. If family therapy is not appropriate based on the particular situation of the youth, the lead clinical staff shall document specific reasons why family therapy cannot be provided.
D. Follow Progress Notes Policy for documentation guidelines. Therapy sessions and therapeutic intervention must address the youth's treatment goals and objectives in the treatment plan and each session must be documented in the case record by the lead clinical staff.
2. Complete clinical assessment within 10 business days of the youth's admission.
3. Develop and complete Individual Case Plans/ Master Treatment Plans on each participant. Master Treatment Plans must be reviewed and updated at least every 30 days. A copy of the treatment plan must be provided to the youth's placing agency and custodial parent or guardian within 10 business days of plans development. Ensure Discharge Summaries are completed within 10 business days, ensuring a continuum of care is in place for participants and family. (See Individual Treatment Plan/ Master Treatment Plan and Discharge Summary Policies.)
4. Develop and submit in a timely manner “Continuation of Stays” for participants meeting the medical criteria for stay at the therapeutic group home level of care.
5. Oversee therapeutic infrastructure of group home as well as maintains organizational structure within the group homes and delegates responsibilities to assigned Program Managers, Lead Youth Care Specialists, and/or Youth Care Specialists. Serves as liaison between administration/business office and the group homes. Report directly to supervisor. Attend clinical meetings.
6. Complete quarterly chart audits under the direction of Quality Assurance or Clinical Director.
7. Other duties as assigned by New Day, Inc. administration.
*Qualifications:* The requirement listed below are representative of the knowledge, skill, and/or ability required.
*Education and/or Experience: *A minimum of a Master’s degree from an accredited college or university with major course work in social work, counseling or psychology, as well as management and extensive experience working with troubled youth, families, and the subsequent development of the emotional problems by displaced/dysfunctional youth. Ensure OSHA/HIPAA compliance.
