*Overview*
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have a strong background in various therapeutic settings, including home health, pediatrics, geriatrics, and inpatient care. This role involves assessing patient needs, developing individualized care plans, and implementing effective treatment strategies to enhance mobility and improve overall quality of life.

*Responsibilities*

* Conduct comprehensive evaluations of patients to determine their physical therapy needs.
* Develop and implement personalized treatment plans based on patient assessments.
* Utilize various therapeutic techniques including trigger point therapy and orthopedic rehabilitation.
* Collaborate with healthcare professionals to ensure cohesive patient care and effective discharge planning.
* Educate patients and their families on treatment protocols and home exercises to promote recovery.
* Maintain accurate documentation of patient progress and treatment outcomes in compliance with healthcare regulations.
* Stay current with advancements in physical therapy practices and participate in ongoing professional development.

*Experience*

* Proven experience in physical therapy settings such as home health, hospital, inpatient, or outpatient facilities is preferred.
* Familiarity with specialized areas including pediatrics, geriatrics, orthopedics, and anatomy knowledge is highly desirable.
* Experience in developing care plans tailored to individual patient needs is essential.
* Strong communication skills to effectively interact with patients, families, and interdisciplinary teams are required.
* A valid state license to practice physical therapy is mandatory.
