*About the Position*

We're looking for an experienced, dedicated Occupational Therapist to support our growing home health patient population in Marin County. This is a part time position. You'll evaluate patients in their homes, help build the plan of care, and deliver the therapy that gets them back to the daily activities that matter to them. Our ideal candidate is compassionate, driven, and knowledgeable, with excellent time management, interpersonal, and communication skills.

*Coverage Area:* Marin County, CA. Candidates must be able to visit patients in their homes throughout the Marin County area.

*Compensation:* $45.00 - $60.00 per hour based on experience

*What You'll Do*

* Assess each patient's rehabilitative status and develop an appropriate treatment plan with goals that are reasonable and necessary for the patient's needs
* Provide therapy services in accordance with the plan of treatment and plan of care ordered by the patient's physician
* Observe and evaluate the effect of treatment and the patient's physical and emotional response to it
* Update the treatment plan on an ongoing basis and as new orders are received
* Contact the physician to clarify orders, report significant changes, discuss the patient's condition and course of treatment, and recommend changes as needed
* Observe, record, and report information on the patient's condition to the physician, the care team, and the medical record
* Document treatment and patient response accurately, following established guidelines
* Participate in interdisciplinary planning, case conferences, and staff meetings as required

*What You Bring*

* Current CA Occupational Therapist license
* Graduate of an accredited occupational therapy program
* 1 year of clinical experience
* Home health experience preferred but not required
* Valid driver's license and auto insurance
* Ability to pass a background check

We're looking for a highly skilled Physical Therapist to join our home health team in Marin County. This is a part time position. You'll provide exceptional care to patients in their homes, helping them regain their strength and independence. You'll work closely with physicians, nurses, and other healthcare professionals to develop and carry out individualized treatment plans that meet the unique needs of each patient. The goal is to help every patient reach their maximum potential and improve their quality of life.

*Coverage Area:* Marin County, CA. Candidates must be able to visit patients in their homes throughout the Marin County area.

*Compensation:* $55.00 - $60.00 per hour based on experience

*What You'll Do*

* Conduct patient evaluations and develop individualized treatment plans
* Provide hands-on therapy to patients in their homes
* Educate patients and their families on proper techniques and exercises
* Collaborate with physicians, nurses, and other healthcare professionals to achieve the best possible patient outcomes
* Maintain accurate, up to date patient records

*What You Bring*

* Current California Physical Therapist license
* At least 1 year of home health experience
* Valid driver's license and auto insurance
* Ability to pass a background check
* Strong problem solving and critical thinking skills
* Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and families
* Ability to work independently and as part of a team
* Proficiency with electronic medical records
* Bilingual English and Spanish a plus
