This role will have a focus on rural health strategy, partnering with the community and community partners, and activating programs.
Pay Range:
$30.70 - $46.05You will support the development, alignment, execution, and evaluation of community engagement strategies from conception to implementation. You will work in partnership with leaders in designing and implementing Advocate Health community engagement strategy and action plan to achieve defined goals across the assigned division. You'll cultivate manages long-term relationships that influence Advocate’s Health’s reputation, awareness, and social impact with teammates, patients, and community partners. In this role you will implement and activate plans in line with best practices and Advocate Health’s core values, mission, priorities, and overall business strategy.
Major Responsibilities:
Support the development and execution from creation, distribution, management, and implementation of defined community engagement programs.
Ability to manage a wide range of relationships with community partners, internal and external stakeholders, and track interactions with these groups.
Maintain and utilize community engagement tracking tools to provide full record of activities, to compile data, and to develop reports.
Participates in meetings, events, symposiums, or initiatives, in community engagement involvement by providing regular updates, identifying challenges, and reporting on a project plan.
Work collaboratively with other internal departments. Serving as an Advocate Health representative on committees (internal and external community coalitions).
Build and maintain strategic partnerships with community stakeholders including local elected officials, nonprofits, corporate partners, public health officials, and foundations while continually assessing and making recommendations for appropriate level of funding and mutually beneficial engagement opportunities.
Communicate, advance, emphasize, prioritize programs and makes recommendations regarding community investment opportunities.
Identify healthcare and community engagement trends and support management in the developing the Advocate Health Community Engagement strategy.
Support the development of an execution model from creation, distribution, management, and implementation.
Participate in reporting on related and respective programs using defined key performance indicators. Retrieve, test, measure, evaluate, and recommend insight research to ensure effectiveness of community engagement campaign and social impact results.
Reenforce the organization’s brand identity and guidelines through community engagement efforts.
Develop key performance indicators that are measurable in design and execution in collaboration with leadership.
Level of Education:
Bachelor's Degree in Public Relations, Public Health, Business or related field.
Years of Experience:
Typically requires 3 years of experience required in community relations, non-profit management, corporate giving, or corporate responsibility. Familiarity with corporate social responsibility programs, local community organizations, programs, and nonprofit groups.

The Registered Nurse Surgical Admit & Recovery is responsible for providing high-quality nursing care to patients before and after surgical procedures, ensuring safe transitions between preoperative and post-anesthesia care phases.
What you will do:
Assess and prepare patients for surgery, including reviewing medical history and administering preoperative medications. Monitor patients in the Pre-Op area, ensuring they are stable and ready for surgery.
Provide immediate post-anesthesia care, monitoring vital signs and assessing for complications.
Educate patients and families on post-operative care, discharge instructions, and recovery expectations.
Strong assessment, critical thinking, and patient advocacy skills along with the ability to work in a fast-paced environment and respond quickly to changing patient conditions.
Minimum Qualifications:
Education: Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred.
Licensure: Current Licensure as a Registered Nurse RN in New York.

$22.90 - $34.35The Lead supports leadership by providing onsite guidance and support to the team responsible for inpatient care for individuals who have experienced recent trauma. This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team.
Major Responsibilities:
Team Navigation: Assists the leadership with daily operations and team performance by guiding the team to meet individual and team goals, high standards of productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment.
Patient Support: Provides immediate emotional support and resources to hospitalized patients following a trauma, including connecting them with the clinical team when appropriate, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. Also, complete assessments and facilitate warm-handoffs to providers for ongoing care.
Resources Navigation: Responds to trauma alerts, safety calls, pre-escalation needs and collaborates with street outreach teams and facilitates mediation to mitigate retaliation risks.
Outreach: Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources.
Continuity of Care: Schedules patient appointments with the TRC, other internal Advocate Health resources, or community partners before discharge and provides brief follow-up calls after discharge when necessary, to ensure ongoing care which requires strong collaboration with other providers.
Assessments: Conducts intakes and needs assessments, linking patients to appropriate resources and outpatient services. It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital.
Record Keeping: Maintains accurate records of patient interactions and action plans.
Collaboration: Works with the hospital treatment team and community partners to ensure comprehensive treatment, discharge planning, and continuity of care.

The Registered Dietitian serves as a key member of the Trinity Health HEART Center team, directing the nutritional components of the HEART Center, Medical Weight Management Program, and Diabetes Education Program. Key Responsibilities
Clinical Nutrition Management
- Conduct comprehensive dietary assessments and develop individualized nutrition care plans.
- Implement nutrition treatment interventions and evaluate patient progress toward established goals.
- Collaborate with physicians, managers, and interdisciplinary care team members to ensure exceptional patient care and outcomes.
- Provide nutrition counseling and education for patients with cardiovascular disease, diabetes, obesity, pulmonary conditions, eating disorders, and other nutrition-related diagnoses.
Patient Education & Counseling
- Develop, implement, and evaluate educational materials and programs related to nutrition, weight management, diabetes prevention and management, and healthy lifestyle practices.
- Facilitate individual counseling sessions, support groups, nutrition classes, and patient group meetings.
- Educate patients and families on nutrition principles, disease management, behavior modification strategies, and self-management techniques.
- Serve as a nutrition resource for patients, providers, and community stakeholders.
Program Development & Leadership
- Coordinate and oversee nutrition-related programming within the HEART Center, Medical Weight Management Program, and Diabetes Education Program.
- Develop nutrition care processes and protocols for targeted patient populations.
- Participate in care conferences, discharge planning, and multidisciplinary treatment planning.
- Act as a nutrition liaison for the Trinity Health primary care network, supporting nutrition education programs and workshops for colleagues and patients.
Quality Improvement & Compliance
- Participate in Continuous Quality Improvement (CQI) initiatives and quality assurance activities.
- Maintain compliance with organizational policies, accreditation standards, and regulatory requirements.
- Accurately document patient assessments, treatment plans, progress notes, educational activities, and outcomes in the electronic medical record.
- Support resource utilization and evidence-based practice initiatives to improve patient outcomes.

Job Summary
Behavioral Health Services (BHS) is made up of a multidisciplinary team of Psychiatrists, Mental Health Nurse Practitioners, Physician Assistants, Mental Health Professionals, Medical Assistants, and Administrative Staff. These professionals work together to provide a gold standard of care regarding effective treatment to support Memorial Hermann patients in an effort to connect the patient to the most appropriate level of care. Our goal is to increase access to behavioral health treatment services, promote better health outcomes for patients with chronic or urgent mental health treatment needs by utilizing empirically validated interventions, decrease unnecessary ER visits, and decrease incarcerations.This role will function as the lead clinician by coordinating the day to day functioning of the BHS Case Management Team. What You'll Do
The Director Cardiovascular Service Line provides strategic, operational, and financial leadership for the full continuum of cardiovascular care across the health system, including inpatient and outpatient cardiology, cardiac surgery, vascular surgery and vascular medicine, interventional and structural cardiac programs, electrophysiology, peripheral vascular interventions, cardiac and vascular imaging, and cardiac rehabilitation. This role partners closely with physician leadership (Service Line Medical Director, often representing both cardiology and vascular/cardiac surgery), nursing leadership, and hospital administration in a dyad model to drive clinical quality, patient experience, growth, and financial performance across all sites of care. The Director is accountable for translating system strategy into an integrated service line plan that spans the inpatient, ambulatory, and post-acute cardiovascular care journey across cardiology and surgical/vascular disciplines.
Responsibilities:
Serve as the operating leader for the cardiology service line.
Partner in a physician-administrator dyad with the Service Line Medical Director to set and execute service line strategy.
Develop and execute multi-year service line growth plans, including program development (e.g., structural heart, advanced heart failure, electrophysiology, complex peripheral and vascular surgery), market expansion, and outreach clinic strategy.
Own the service line's annual operating and capital budgets; monitor volumes, revenue, expense, and productivity, and lead corrective action plans to meet financial targets.
Oversee daily operations of cardiac catheterization, electrophysiology, and vascular labs, non-invasive cardiac and vascular imaging and diagnostics, cardiology and vascular surgery clinics, and cardiac rehabilitation programs.
Lead service line quality, safety, and patient experience initiatives in collaboration with clinical leaders, including registry participation, accreditation readiness, and core measure performance.
Drive physician engagement, recruitment planning, and alignment strategies across cardiology and cardiac/vascular surgery, in partnership with medical staff leadership.
Manage service line staffing strategy, workforce planning, and leadership development for direct and matrixed reports across multiple departments and sites.
Lead or support service line contracting strategy, including supply chain/value analysis for cardiovascular devices and implants, in partnership with supply chain and finance.
Represent the service line in payer strategy discussions, bundled payment and value-based care initiatives, and managed care contract negotiations as needed.
Ensure regulatory compliance across all cardiology sites, including CMS Conditions of Participation, and applicable state regulations.
Build and maintain relationships with referring physicians, community partners, and regional outreach sites to support network integration and patient access.
Lead change management and performance improvement initiatives.
Report service line performance, risks, and strategic initiatives to system leadership and the board as required.
Requirements
