West Virginia University School of Medicine and the Department of Medical Oncology seek a Hematologist/Oncologist (ranks available: Assistant Professor, Associate Professor, or Professor). The successful candidate will be expected to practice in Charleston, West Virginia. and Jackson General Hospital, Ripley, West Virginia.
Duties: The successful candidate will practice in the areas of hematology/oncology. In addition to providing excellent patient care, the successful candidate will also be actively involved in teaching medical students, residents, and fellows.
Qualifications: Candidate must have an MD or DO degree or foreign equivalent and be eligible for state medical license. Successful candidate must have completed an accredited internal medicine residency and hematology/oncology fellowship program. Successful candidates must be board certified/eligible in hematology/oncology. All qualifications must be met by the time of appointment.
As a full member of the WVU Health System, Thomas Memorial Hospital is part of a broad, integrated network of doctors, hospitals, clinics, and specialized institutes across West Virginia, Southwestern Pennsylvania, Western Maryland, and Ohio. The WVU Cancer Institute is one of the country’s premier cancer treatment centers. We have a very active clinical trials program, pharmaceutical studies, and emerging statewide clinical trials network.
The WVU Cancer Institute which was recently named one of the top cancer hospitals by Newsweek, provides care to over 125,000 patients annually. West Virginia University, the WVU Health System, and the state of WV are engaged and committed to moving the WVU CI toward obtaining NCI Designation. In addition, there is commitment to build a new state of the art, inclusive of inpatient and outpatient services, cancer hospital attached to the main JW Ruby Memorial Hospital. Wheeling Hospital: As a full member of the health system, Wheeling Hospital is part of a broad, integrated network of doctors, hospitals, clinics, and specialized institutes across West Virginia, Southwestern Pennsylvania, Western Maryland, and Ohio. Wheeling Hospital has served the Northern Panhandle of West Virginia, Eastern Ohio, and parts of Pennsylvania longer than any other hospital in the state. It has 247 beds, and the medical staff totals 230 primary care and specialty physicians. Two additional system hospitals provide service to this region. Weirton Medical Center in Weirton, WV has 238 beds while Reynolds Memorial Hospital has 94 beds and is in Glen Dale, WV. Other services at Wheeling Hospital include the Continuous Care Center, the Wheeling Renal Care, and the Howard Long Wellness Center. Connected to the hospital are four medical office buildings where 80 physicians have their private offices

About this role
As a General Podiatry Physician, you will work alongside a well-established group of orthopedic fellowship trained trauma and foot/ankle surgeons managing patients in the clinic, the hospital, and perform exceptional quality care as a Podiatrist. You will also work alongside the wound team with inpatient and outpatient care, the team consisting of a wound physician, physician assistant, and wound nurses.
In addition, you will provide clinical oversight for multidisciplinary program meetings and build strategic relationships across inpatient, outpatient, specialty, and community partners to promote program engagement and timely referrals.

- Take general podiatry emergency room call coverage of 7 days per month; potentially as consecutive days or one week per month
- Work clinic schedule consisting of 2 podiatry clinic days, 1 surgical day, and 2 wound care clinic days per week

Position Overview
The Integrated Case Manager, RN is responsible for providing comprehensive case management services within the scope of nursing licensure, focusing on members with behavioral health needs, complex medical conditions, intellectual and developmental disabilities, and those receiving Medicaid waiver services. The Case Manager develops, implements, evaluates, and revises individualized care plans designed to improve health outcomes, promote independence, and enhance quality of life.
This role conducts telephonic and face-to-face assessments to identify, coordinate, and manage members' physical health, behavioral health, social determinants of health, long-term services and supports (LTSS), and community resource needs.
Key Responsibilities
- Perform comprehensive assessments of members with complex medical, behavioral health, and social needs.

The Behavioral Patient Navigator serves as a non-clinical member of the interdisciplinary care team supporting psychiatric and behavioral health patients throughout their healthcare journey. The role provides behavioral triage support, therapeutic communication, motivational interviewing, patient engagement, care coordination, behavioral and psychiatric resource navigation, and linkage to appropriate services. The Patient Navigator assists patients with overcoming barriers related to treatment adherence, appointments, medication routines, behavioral health follow-up, and engagement with their established plan of care while promoting individualized, patient-centered care and recovery.
The Behavioral Patient Navigator works closely with licensed behavioral health providers, psychiatrists, social workers, case managers, community partners, Nursing, and Utilization Management to facilitate timely access to care, support care transitions, and promote continuity of care. This is a non-clinical role and does not provide clinical assessments, diagnoses, therapy, or medical advice.
Key Responsibilities
- Conduct outreach and maintain regular contact with behavioral health and psychiatric patients.
- Provide non-clinical behavioral health triage and escalate urgent or high-risk concerns to licensed clinicians.
- Use therapeutic communication and motivational interviewing techniques to support patient engagement, treatment adherence, and behavioral goals.
- Assist patients with navigating appointments, referrals, behavioral health services, and community resources.
- Coordinate with internal care teams, behavioral health and primary care providers, and community organizations.
- Track referrals, follow up on missed appointments, and assist with transitions of care.
- Connect patients with appropriate behavioral health, social service, and community-based resources.
- Bachelor’s Degree in Psychology, Social Work, Behavioral Science, and Behavioral Science.
- 3 years or more in Behavioral health, patient navigation, case management support, care coordination, community health, and/or related healthcare setting.
- Demonstrated experience engaging patients with behavioral health psychosocial or complex healthcare needs.

Overview
As a key member of the Population Health Team, the Social Worker, MSW, will contribute to results as defined by the principles of value-based care.

Overview
The Manager, Behavioral Health Service Delivery provides operational leadership for behavioral health programs and practices across the Southeast market, with an initial focus on Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) services.
This position is responsible for the daily management and performance of assigned behavioral health practices while also supporting the development, implementation, and operationalization of new behavioral health programs and locations. As services expand, the Manager will provide oversight across multiple PHP/IOP practices and ensure consistent operations, quality, regulatory compliance, patient access, staffing, and financial performance across the market.
The Manager works closely with Enterprise Behavioral Health leadership, physicians, clinical leaders, hospital and ambulatory operations, finance, revenue cycle, compliance, and other partners to create a coordinated and sustainable behavioral health service delivery model.
Key Responsibilities
Provides operational leadership and oversight for assigned behavioral health programs and practices, including PHP and IOP services.
Oversees multiple behavioral health locations as programs expand across the Southeast market, ensuring consistent operational standards and performance.
Leads or supports the development and implementation of new PHP/IOP practices, including operational planning, staffing, workflows, readiness, and transition to ongoing operations.
Manages day-to-day practice operations, including staffing, scheduling, patient flow, clinical support processes, resource utilization, and operational problem solving.
Partners with clinical leadership to coordinate interdisciplinary care delivery, therapeutic programming, caseload distribution, and patient scheduling.
Develops and monitors operational, access, productivity, quality, financial, and patient experience metrics and implements corrective actions when performance does not meet expectations.
Ensures effective patient access and financial clearance processes, including coordination of insurance authorization, eligibility, scheduling, and revenue-cycle workflows.
Ensures compliance with applicable regulatory, accreditation, licensing, safety, documentation, and organizational requirements.
Recruits, develops, and manages staff; establishes performance expectations; promotes accountability; and supports staff competency and professional development.
Standardizes workflows, policies, procedures, and operating practices across behavioral health locations to support consistent service delivery.
Collaborates with hospital, ambulatory, Enterprise Behavioral Health, Health Plan, and community partners to improve transitions of care and access to behavioral health services.
Supports annual planning, budgeting, resource allocation, and business performance for assigned programs.
Identifies operational barriers, capacity constraints, and opportunities for program growth and develops solutions in partnership with behavioral health leadership.
Serves as an operational escalation point for assigned programs and provides leadership during periods of significant operational change or program expansion.

The successful candidate will be expected to practice in Princeton, West Virginia.

The Peer Recovery Coach (PRC) is a vital member of the Morton Hospital care team and provides non‑clinical, recovery‑oriented peer-to-peer support to individuals with substance use disorders and co‑occurring health or behavioral health needs. The Peer Recovery Coach uses lived experience, recovery coaching principles, and a trauma‑informed approach to engage patients, promote hope, reduce stigma, and support sustained recovery.
Peer Recovery Coaches focus on addressing recovery needs and social drivers of health by supporting patients in navigating healthcare systems, recovery supports, and community‑based services. The PRC works collaboratively with physicians, nurses, social workers, case managers, and community partners while maintaining a clearly defined non‑clinical peer support role within the interdisciplinary care team.

Responsibilities
Peer Recovery Coaches must understand and apply Massachusetts BSAS peer recovery standards, trauma‑informed care, and motivational engagement strategies while maintaining ethical peer boundaries.
Specifically, the Peer Recovery Coach will:
- Work collaboratively on an interdisciplinary hospital care team using an integrated, recovery‑oriented model.
- Engage patients through rapport building, peer recovery coaching conversations, and motivational interviewing techniques.
- Provide non‑clinical education on substance use disorder treatment options, harm reduction strategies, overdose prevention, and recovery supports.
- Support patients during Emergency Department, inpatient, and care transition encounters, including initiation or continuation of medications for opioid use disorder (MOUD), without providing clinical services.
- Advocate for patient goals and preferences during care planning and discharge processes.
- Assist patients in navigating community‑based recovery supports, housing resources, insurance, transportation, food access, and related social services.
- Facilitate warm hand‑offs and referrals to treatment providers and recovery community organizations.
- Use professional judgment when responding to patient crises such as relapse, housing loss, or emotional distress, and escalate concerns using a team‑based approach and established hospital protocols.
- Maintain accurate and timely documentation of peer recovery services in the electronic medical record.
- Participate in supervision, team meetings, and ongoing training required to maintain Massachusetts peer certification.
Essential Duties
Under the supervision of the designated program supervisor and in collaboration with hospital leadership, the Peer Recovery Coach will:
- Conduct recovery‑oriented intake conversations to identify strengths, needs, and recovery goals.
- Support patients in developing and maintaining recovery and self‑management strategies.
- Provide follow‑up and outreach to assigned patients after discharge or Emergency Department visits.
- Coordinate care efforts with existing providers, case managers, and recovery partners.

Minimum Qualifications

- Massachusetts‑recognized Peer Recovery Coach or Peer Specialist certification, or ability to obtain within one (1) year of hire through a BSAS‑recognized program.
- Lived experience with substance use recovery, including a minimum of two (2) years of sustained recovery and active engagement in personal recovery.
- High school diploma or GED required; must be at least 18 years of age.
Experience & Skills

The facilitator works collaboratively with primary care, specialty care, palliative care, care management, social work, and community partners to ensure patients' healthcare preferences are understood, documented, and accessible.

- Facilitate advance care planning conversations with patients and families.
- Assist patients with completion of advance directives and state-specific ACP documents.
- Facilitate POLST/POST/MOST discussions consistent with organizational policies and state regulations.

- Support centralized ACP referral processes and patient outreach activities.

SUMMARY:
The Mobile Integrated Health (MIH) Advanced Practice Provider (APP) serves as a key clinical provider within an innovative, community-based care delivery model designed to improve coordination and outcomes for medically complex patients. The APP provides comprehensive assessment, treatment, care coordination, and transitional care services in patients' homes, residential settings, and other community locations, while collaborating closely with physicians, nurses, care managers, social workers, paramedics, and community partners.
The APP delivers high-quality, patient-centered care that supports hospital avoidance, reduces unnecessary emergency department utilization and readmissions, and promotes patients' ability to safely remain in their homes and communities. The APP utilizes evidence-based practice, advanced clinical decision-making, telehealth technology, and interdisciplinary collaboration to manage acute, chronic, and transitional care needs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Clinical Care:
Conduct comprehensive health assessments in patients' homes and community settings, if needed.
Evaluate, diagnose, and treat acute and chronic medical conditions within the APP scope of practice.
Perform urgent and episodic visits to support Mobile Integrated Health program.
Communicate with paramedics involved with clinical care in MIH program.
Develop and implement individualized treatment plans in collaboration with patients, caregivers, and interdisciplinary team members.
Prescribe medications, therapies, and diagnostic testing in accordance with state regulations and organizational policies.
Provide symptom management, chronic disease management, preventive care, and patient education.
Monitor patients enrolled in the Mobile Integrated Health Program.
Utilize virtual care technologies and telehealth platforms to support ongoing patient monitoring and clinical decision-making.
Identify patients who are admitted to the hospital or in the emergency department that would benefit from MIH and help in enrolling appropriate patients for the same
Care Coordination:
Collaborate with physicians, nursing staff, social workers, pharmacists, behavioral health providers, paramedics, and care managers to ensure coordinated, holistic care.
Participate in multidisciplinary case reviews and care planning discussions.
Assist with transitions of care following hospitalization, emergency department visits, or skilled nursing facility stays.
Coordinate referrals to specialty care, home health services, community resources, and supportive services.
Address social determinants of health and barriers to care through collaboration with community partners and care coordination teams.
Engage family members and caregivers as integral members of the care team.
Program Operations and Quality:
Maintain accurate, timely, and complete documentation in the electronic medical record.
Participate in quality improvement initiatives designed to improve patient outcomes, safety, and operational efficiency.
Monitor program quality metrics, patient outcomes, and care management goals.
Participate in clinical case reviews and interdisciplinary rounds.
In conjunction with the Medical Director, assist in the development, implementation, and refinement of Mobile Integrated Health clinical workflows, protocols, and best practices.
Support regulatory compliance and accreditation standards applicable to community-based and home-based healthcare programs.
Participate in safety event reporting and quality investigations as required.
Professional Leadership:
Serve as a clinical resource and mentor for team members.
Promote a culture of collaboration, innovation, accountability, and patient-centered care.
Participate in provider meetings, education programs, committee work, and organizational initiatives.
Support program growth and innovation through active engagement in process improvement and strategic initiatives.
Represent the Mobile Integrated Health Program in interactions with healthcare partners, community agencies, and stakeholders as appropriate.

EDUCATION:
Current registration as a Physician Assistant or Nurse Practitioner in the State of Rhode Island.
Nurse Practitioner (NP):
Master's or Doctoral degree in Nursing from an accredited program.
Current Nurse Practitioner certification in Family Practice (FNP), Adult-Gerontology (AGNP), Adult-Gerontology Acute Care (AGACNP), or other relevant specialty.
Physician Assistant (PA):
Graduate of an accredited Physician Assistant program.
Current NCCPA certification.
Licensure and Certification:
Current unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
Current DEA registration and prescriptive authority as applicable.
Current Basic Life Support (BLS) certification.
Advanced Cardiac Life Support (ACLS) certification preferred.
Valid driver's license and reliable transportation.
EXPERIENCE:
Minimum of three (3) years of clinical experience as an Advanced Practice Provider required.
Experience in Hospital Medicine, Emergency Medicine, Internal Medicine, Family Medicine, Geriatrics, Critical Care, Palliative Care, Home-Based Primary Care, or Transitional Care preferred.
Experience providing care in home-based, community-based, mobile, virtual, or non-traditional care settings preferred.
Experience caring for medically complex, high-risk, or vulnerable patient populations strongly preferred.
Prior experience working within interdisciplinary care teams required.

Position Summary
