Summary:
This role is responsible for overseeing the day-to-day operations and guest experience of Conine’s Clubhouse, the hospital’s on-site family hospitality house for families of critically ill children. This role manages lodging logistics, coordinates with hospital departments and vendors, and ensures the facility supports a healing, welcoming environment.The Coordinator facilitates the development and evaluation of clubhouse services and programs, manages volunteers, and serves as the key liaison for community partners and donors supporting the clubhouse through special events and contributions.
Responsibilities:
Documentation and Data Management:-Maintains accurate and confidential guest records in accordance with HIPAA and organizational policies.-Utilizes electronic systems (e.g., Excel, databases) to manage lodging information and track guest feedback.-Documents and reports facility issues, safety incidents, or special guest needs to the appropriate teams.Collaboration and Communication:-Works closely with case managers, social workers, volunteer services, and community partners to coordinate referrals and support for lodging.-Contributes to experience improvement efforts, attends staff meetings, and participates in performance reviews.-Assists with onboarding and coordination of volunteers involved in family lodging services.Key Responsibilities:-Greets and orients families and patients, offering information on lodging, hospital services, and community resources.-Manages room assignments, check-ins/check-outs, and maintains accurate occupancy records.-Ensures the cleanliness, safety, and readiness of the clubhouse in partnership with Environmental Services.-Addresses guest concerns, resolves service issues, and coordinates necessary accommodations.-Works collaboratively with clinical and support teams to ensure smooth referrals and continuous guest support.Lodging Space Utilization and Reporting:-Tracks and maintains accurate records of room occupancy, length of stay, and turnover rates.-Produces regular reports on utilization metrics, guest demographics, and referral sources.-Monitors trends in demand and collaborates with leadership to prepare for peak periods or space constraints.Donation Tracking & Supply Management:-Records and tracks in-kind and monetary donations (e.g., linens, toiletries, snacks, gift cards).-Ensures proper inventory, storage, and distribution of donated items aligned with donor intent policy.-Supports donor acknowledgment by providing documentation and coordinating with development or foundation teams.

Atrium Health Outreach Athletic Training partners with area middle schools, high schools, and community athletic programs to provide comprehensive sports medicine services focused on injury prevention, evaluation, treatment, and return-to-play management. Athletic trainers work closely with physicians, coaches, athletes, and families to deliver high-quality care both on-site and in clinical settings while promoting athlete health, safety, and performance throughout the season.
Job Description:
The outreach athletic trainer is responsible for the day-to-day medical coverage in secondary school settings (middle school and high school). Uses preventative measures appropriately; provide standard immediate care procedures used in emergency situations; clinical evaluation, treatment, rehabilitation, and reconditioning. Employ appropriate assessment outcomes measures to determine effectiveness of the outreach program; determines appropriate return to play of athletes and active population. Management of head injuries and concussions; referral to appropriate medical professionals to ensure optimal level of care; all duties and responsibilities performed under the supervision of a directing physician. Also works collaboratively with the Sports Medicine Team to ensure service excellence for all customers of the athletic training outreach program.
Major Responsibilities
- Provide athletic training services to secondary schools and community sporting events as determined necessary to attain departmental goals of community service.
- Works within the Sports Medicine Team concept and assists with event coverage as necessary to attain departmental goals of community service.
- Provides acute medical care to athletes 'on the field' during practices or sporting events.
- Provides on-site recognition, evaluation, and immediate care procedures of illness and injury including emergency situations, independent of setting.
- Perform clinical injury evaluation and assessment of patient's level of function and follows standardized clinical practice in the area of diagnostic reasoning and medical decision making.
- Determine appropriate therapeutic treatment and provides rehabilitation, and/or reconditioning strategies, and taping / wrapping and bracing for preventative and protective treatment.
- Implement treatment program objectives including long- and short-term goals and an appraisal of those which the patient can realistically be expected to achieve from the program. Assessment measures to determine the effectiveness of the program are incorporated into the program.
- Collaborate with their directing physician, recommend discontinuation of the athletic training service when patient has received optimal benefit of the rehabilitation and treatment program, and notes final assessment of patient status.
- Determine return to play standards of injuries for athletes and the active population.
- Maintain daily records of treatment logs, injury reports, doctor's referrals, and medical inventory management used for the care of athletes according to clinic standards.
- Complete set-up of all practice and games including medical supplies, fracture management equipment, hydration, cryotherapy, and any other supplies needed for adequate coverage.
- Assist with planning, organizing, implementing and compliance of departmental and policies.
- Ensures an Emergency Action Plan for each venue be put in place and discussed with the school's Athletic Director, coaches, and athletes, including local EMS. Exhibit ability to perform essential duties efficiently and accurately with or without reasonable accommodations and without endangering incumbent or other employees.
- Assists with the preparation of lectures, sports medicine programs, seminars and serves as a presenter in these programs when needed.
- Mentor and manage on site education of students enrolled in university-based athletic training education programs or any other healthcare professionals at outreach events (if applicable).
- Ensure all athletic training room sites have appropriate supplies and equipment; recommends new equipment for acquisition and use by the organization.
- Develop physician referral network including family practice, orthopedic, physical therapy & other specialties as needed.
- Facilitates patient entry into the healthcare system and properly track and document these referrals.
- Maintain necessary training on concussion education, management, and care, including acute management of the cervical spine.
- Adheres to departmental dress code when providing on-site Athletic Training services.
- Perform other related clinical duties as required incumbent to State Board of Medicine/ Board of Athletic Training Statue and Rules.
Minimum Job Requirements
Education:
Preferred:Bachelor’s, Master’s, or equivalent degree from a CAATE-accredited athletic training program.
Minimum requirement is a Bachelors' degree in early childhood development, early childhood education, child and family development, nursing, speech and language pathology, occupational therapy, physical therapy or a related human services field.
Certification / License:Current athletic training certification by the Board of Certification for the Athletic Trainer (BOC). Currently licensed or eligible for a State of applicable state License in Athletic Training within 30 days of hire. Ability to complete American Heart Association Healthcare Provider (BLS), which includes CPR and AED training within 30 days of hire.

- Conducts comprehensive assessments, diagnosis, treatment planning, treatment interventions and measurable outcomes to patients with a history of sexual trauma. Develops aftercare plans as appropriate.
- Provides case management for complex patients/clients.
- Makes appropriate referrals to and works in collaboration with abuse response advocates for advocacy services and service coordination.
- Provides trauma-informed response and best practices to support victims in immediate and long-term recovery.
- Communicates effectively with patients, family, other members of the health care team and community agencies toward the resolution of issues which could impact the continuity of care.
- Provides community and professional education and outreach services in order to enhance the services of the center. Acts as a liaison to appropriate community partners (i.e. colleges, partner SA agencies, etc.).
- Provides expert witness testimony on sexual assault cases in court as needed. Tracks caseload of social work referrals and interventions.

- Partner with local facility leaders, clinical teams, operational stakeholders, and enterprise Community Health & Impact leaders to align local community health priorities with broader organizational strategies, service line priorities, and population health goals.
- Lead the Community Health Needs Assessment cycle and related Implementation Strategies and governance responsibilities for assigned markets or facilities, ensuring identified priorities are translated into actionable plans that reflect local needs, division/facility priorities, and enterprise Community Health & Impact direction.
- Develop, implement, and refine community health programs that address priority social drivers of health and support measurable improvements in social and clinical outcomes for patients, communities, and targeted populations.
- Establish performance measures, evaluation approaches, and reporting processes to assess program effectiveness, demonstrate impact, and inform continuous improvement of community health strategies and investments.
- Source, assess, and advance local community projects for potential low-interest loan investment, including coordination with internal partners to evaluate alignment, readiness, impact potential, and strategic fit.
- Support Community Engagement strategies to build and maintain productive relationships with community-based organizations, public agencies, civic leaders, philanthropic partners, and other local stakeholders to support shared priorities and strengthen Advocate Health’s role as a trusted community partner.
- Oversee timely and accurate community benefit data entry, documentation, and validation processes to support internal reporting, regulatory expectations, and consistent enterprise standards.
- Translate community data, partner insights, and implementation progress into clear recommendations, presentations, and updates for local leaders, enterprise stakeholders, and community partners.
- Support cross-functional collaboration and project execution by convening internal and external partners, monitoring progress against agreed-upon goals, and addressing barriers that may affect successful implementation of community health initiatives.

The Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of patients. The staff RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members.
Scope
1.Provides direct patient care and education to those with altered/impaired skin integrity,ostomies and incontinence congruent with defined standards of practice. Evaluates patent care with emphasis on quality improvement.
2. Provides in-service education, staff development instruction and educational opportunities to medical and nursing staff and nursing students. Initiates and applies new clinical modalities based upon expert knowledge, technical competency, new research and literature. Provides input through product evaluation regarding the responsible use of present resources as well as purchasing cost-effective supplies and equipment relating to wounds, ostomies and incontinence.
3. Partners with medical staff, interdepartmental staff and community resources to provide comprehensive inpatient care. Assists with referrals of clients along the care continuum.
4. Assists with the development and coordination of job specific nursing competency validation.
5. Assists with the development, implementation and evaluation of policies and procedures. Consults in area of specialty within the care site and with community partners.
6. Demonstrates and promotes the use of the quality improvement process for the assigned patient population.

Required:
Graduation from an accredited School of Nursing is required.
A Registered Nurse with an ASN/ADN degree must complete a BSN degree within four (4) years from the Registered Nurse’s date of hire unless the Registered Nurse has a minimum of fifteen (15) years of proven continuous Registered Nurse experience in an acute care setting. Failure to obtain a BSN degree within that time frame may result in suspension or termination of employment. Any Registered Nurse who takes legally protected leave during the Four (4) year completion period may have the period extended by a comparable amount of time.
Registered Nurses employed by St Vincent, Good Samaritan, St. Joseph, or Lutheran Medical Centers as of September 1, 2014 with only an ASN/ADN will be exempt from this requirement. Registered Nurses employed by Holy Rosary, Platte Valley, St. Mary’s, Home Health, and TSFP as of December 31, 2017 with only an ASN/ADN will be exempt from this requirement.

About this role
As a General Podiatry Physician, you will work alongside a well-established group of orthopedic fellowship trained trauma and foot/ankle surgeons managing patients in the clinic, the hospital, and perform exceptional quality care as a Podiatrist. You will also work alongside the wound team with inpatient and outpatient care, the team consisting of a wound physician, physician assistant, and wound nurses.

- Practice at Intermountain Health McKay-Dee Hospital and Orthopedic Podiatry Clinic in Ogden, Utah
- Take general podiatry emergency room call coverage of 7 days per month; potentially as consecutive days or one week per month
- Work clinic schedule consisting of 2 podiatry clinic days, 1 surgical day, and 2 wound care clinic days per week
- Have nurse practitioner support available in the podiatry clinic
An ideal candidate will have
- Board-qualified or board-certified Doctor of Podiatric Medicine with wound care or limb preservation experience

About the Tennessee Memory Care Assessment Network (MCAN)
Tennessee has an estimated 129,000 residents living with Alzheimer's disease and related dementias. Rural Tennesseans face three persistent barriers to timely diagnosis and care: a shortage of dementia specialists outside major urban centers, time constraints that limit how thoroughly primary care providers can evaluate cognitive concerns, and provider discomfort with disclosing a dementia diagnosis. The Tennessee Memory Care Assessment Network (MCAN) is a Vanderbilt Health-led hub-and-spoke model that establishes a standardized statewide dementia protocol connecting rural Tennesseans with suspected dementia to expert diagnosis, evidence-based treatment access, caregiver support, and provider education. Vanderbilt serves as the statewide Hub, supporting five regional spokes and a statewide Dementia Care Navigator workforce.
MCAN draws on Vanderbilt's existing infrastructure at scale: the Vanderbilt Health Affiliated Network (67 hospitals, more than 7,500 providers, and 1,789 adult PCPs across Tennessee); the NIA-designated Vanderbilt Alzheimer's Disease Research Center; the Division of Behavioral and Cognitive Neurology's subspecialty depth; Epic and REDCap informatics infrastructure extended to non-Epic sites via EpicCare Link; and a formal partnership with the Alzheimer's Association. An eleven-member External Advisory Board of clinical, community, and policy leaders provides independent accountability alongside Tennessee Department of Health oversight.
Position Summary
The Executive Program Director provides strategic and operational leadership for the Tennessee Memory Care Assessment Network, coordinating statewide partners, standardizing assessment and care pathways, and ensuring high-quality, data-driven program implementation and sustainability. This is the senior full-time leadership role on the MCAN team. This role will report to the Director and Co-Director, who will retain the clinical and scientific direction of the program.
The Executive Program Director translates the network's clinical, educational, and technical vision into a running statewide operation: standing up spoke sites, building and supervising the program management team and the statewide Dementia Care Navigator workforce, managing contractual milestones and Tennessee Department of Health reporting, and coordinating the clinical, IT, financial, and community stakeholders that make the hub-and-spoke model work. The role reports to and partners closely with the Directors, Ryan Darby, MD (Principal Investigator/Medical Director) and Co-Director, Ciaran Considine, PhD, ABPP-CN, while owning day-to-day program management, operations, compliance, and stakeholder relations across the network's five-year term.

Key Responsibilities
Program Leadership & Network Operations
- Serve as the day-to-day operational leader of MCAN, overseeing execution of all five program aims: standardized diagnostic protocol, phenotype-specific specialist diagnosis, navigator-facilitated care connection, interoperable IT infrastructure, and workforce training.
- Working closely with the education / training team lead and the social work lead, standardize assessment and care pathways across the network, ensuring the diagnostic protocol is applied consistently from community awareness and PCP screening through navigator-assisted intake, specialist evaluation, disclosure and care planning, and structured follow-up.
- Manage the phased statewide rollout: select and onboard regional spoke sites in coordination with the Vanderbilt Health Affiliated Network (VHAN), and drive the network toward its contractual milestones - a live IT platform by Month 12, three operational spoke sites by Month 24, all five sites and a finalized statewide protocol by Month 36, and full network operation through Month 60.
- Coordinate statewide partners (spoke health systems, VHAN, the Alzheimer's Association, and community organizations) to keep the network's many stakeholders aligned around shared timelines and deliverables.
- Oversee program implementation quality: build the data-driven monitoring practices needed to track fidelity to the protocol and course-correct as the network scales.
- Lead sustainability planning so the network can continue generating clinical revenue, a trained workforce, and durable community partnerships beyond the state grant period.
Team Leadership
- Interview, hire, and directly supervise the MCAN program administrative management team: Program Manager, Hub-Spoke Clinic Coordinator, Communications Program Manager, Education / Outreach Program Manager. Provide operational oversight of the statewide Patient Service Specialist team, who support patients and caregivers with scheduling, navigation, and resource linkage, and of the broader Dementia Care Navigator workforce as it is built out across spoke sites.
- Set annual goals, manage performance, and support professional development for direct reports, building a program management team capable of operating at statewide scale.
Cross-Functional Coordination
- Serve as the central point of coordination between the program management team and the MCAN clinical and technical leadership, including the Co-Directors/Medical Director, Education and Community Outreach Lead, Rural Health and Dementia Equity Lead, Health IT Infrastructure Lead, Data, Reporting, and Biostatistics Core Team Lead, Anti-Amyloid Infusion Director, Lead Neuropsychologist, Social Work Lead, Geriatric Psychiatry lead, Geriatric Medicine Lead, Nurse Lead. .Partner with the Health IT Project Manager and Data/Biostatistics Core Lead to ensure the interoperable data system and reporting infrastructure are delivered on schedule and support quarterly program metrics.
- Work with the Education/Training Team Lead to align navigator training, PCP engagement, and Tennessee Dementia ECHO Program curriculum with network operational needs.
- Coordinate with VHAN leadership on spoke-site selection, provider engagement, and use of VHAN's population-health reporting and quality-measurement infrastructure.
Contract, Compliance & Reporting
- Own the MCAN relationship with the Tennessee Department of Health, ensuring the network meets contractual deliverables, quarterly reporting requirements, and grant-funded milestones.
- Work with Vanderbilt's Office of Sponsored Programs (OSP) on subcontracting, invoicing, subrecipient monitoring, and audit readiness for the life of the award.
- Prepare and present regular progress updates to the MCAN Co-Directors and the External Advisory Board, which meets quarterly.
- Monitor program budget execution against the approved grant budget and narrative, flagging risks and driving corrective action.
External & Community Relations
- Engage with the External Advisory Board, to cultivate and maintain the network's partnerships with the Alzheimer's Association, area agencies on aging, local health departments, patient advocacy organizations, and other community stakeholders.
- Represent MCAN to external partners, state officials, and the provider community, and support engagement with the External Advisory Board's clinical, community, and policy members.
- Support the recruitment of new provider spoke sites and rural PCP participation, working from VHAN's existing footprint of contractual relationships.
Sustainability & Growth
- Develop and execute strategies to generate the clinical revenue, trained workforce, and durable community partnerships needed to sustain MCAN beyond the state grant period.
- Identify opportunities to expand network capacity, including spoke-site infusion delivery capability and additional funding or partnership sources.
Qualifications
Required
- Bachelor’s degree
- 5+ years of progressively responsible experience in healthcare program management, clinical research, clinical operations, or population health, ideally including multi-site or statewide network operations.
- Demonstrated experience managing complex, multi-stakeholder programs involving clinical, IT, financial, and community partners.
- Experience with grant- or state-contract-funded programs, including milestone tracking, budget oversight, and regulatory/reporting compliance.
- Strong understanding of, or ability to quickly learn, dementia care pathways, rural healthcare delivery challenges, and hub-and-spoke care models.
- Direct people-management experience, with a track record of building and leading a multi-function team (e.g., program/project managers, community outreach, communications, clinical operations); this role is budgeted to directly supervise five to six program managers plus coordination oversight of a larger statewide navigator/patient-service workforce.
- Excellent written and verbal communication skills, including experience presenting to boards, funders, or senior health-system leadership.
- Familiarity with EHR-based care coordination workflows (Epic preferred) and clinical research data platforms (REDCap preferred).
Preferred
- Experience within an academic medical center or large physician-led clinically integrated network (e.g., a health system affiliated network similar to VHAN).
- Master's degree in health administration, public health, business administration, nursing, or a related field (or equivalent experience), or advanced clinical or doctoral degree
- Prior experience with dementia, neurology, geriatrics, or memory-care programs.
- Experience building or scaling a statewide or regional care-navigation workforce.
- Familiarity with Tennessee's rural health landscape and community aging-services infrastructure.
