Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Mankato, MN.
Position Overview:
Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
Ability to design and manage your schedule
Develop and implement care plans appropriate to patients and their desires
Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
Access to diagnostics such as labs and x-rays as needed
Collaborate with community partners within home care, hospice, and other resources as appropriate
Qualifications:
Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
Board Certified/Board Prepared through AANP, ANCC, or NCCPA
Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
Ability to obtain and /or maintain DEA certification for the State of required of work
Proven ability to communicate effectively with strong verbal skills
Excellent interpersonal and customer service skills
Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required
Preferred Qualifications:
2+ years of clinical experience as a Nurse Practitioner or Physician Assistant

THEMA Health Services is seeking a Nurse Practitioner to provide community-based palliative care for patients living with serious illness. This role provides care in private homes, assisted living and other community settings, as well as through telehealth.
The NP will complete comprehensive palliative assessments, manage symptoms and medications, develop patient-centered care plans, facilitate goals-of-care and advance care planning discussions, and coordinate care across THEMA Home Health, Hospice, community providers, facilities, and other members of the patient’s care team.
Key Responsibilities
* Complete initial and follow-up palliative care assessments
* Evaluate symptom burden, disease trajectory, functional and cognitive status, caregiver needs, safety concerns, and goals of care
* Diagnose and manage palliative symptoms within NP scope and competency
* Order appropriate medications, labs, diagnostics, DME, referrals, home health services, and hospice evaluations
* Perform medication reconciliation, deprescribing review, opioid-safety assessment, and high-risk medication monitoring
* Conduct goals-of-care discussions, advance care planning, family meetings, and care-plan updates
* Identify changes in condition requiring escalation
* Recognize hospice eligibility triggers and initiate appropriate hospice consultation
* Participate in care-management workflows and maintain documentation supporting compliant billing and reimbursement
* Collaborate closely with patients, caregivers, clinicians, facilities, and community partners
Required Qualifications
* Active Arizona RN license and Arizona RNP certification
* National certification appropriate to the population served, or ability to obtain within 15 months of hire
* Prescriptive authority and DEA registration when controlled-substance prescribing is part of the role
* Medicare NPI/PECOS enrollment and payer credentialing before independent billing
* Experience caring for patients with complex chronic or serious illness
* Strong skills in symptom management, medication reconciliation, family communication, and care coordination

JOB SUMMARY: We are seeking a passionate and skilled Therapist to join our Family Trauma Intervention Program (FTIP). In this role, you will provide home-based therapeutic services to children and families experiencing significant trauma, including abuse, neglect, and domestic violence. This position focuses on strengthening family relationships, supporting child well-being, and preventing foster care placement through evidence-based, trauma-informed practices grounded in Attachment Theory and Psychodynamic frameworks.
RESPONSIBILITIES INCLUDE:
Provide home-based therapeutic services to children and caregivers, including individual and family therapy, caregiver support, crisis intervention, and case coordination.
Complete comprehensive assessments, treatment plans, safety plans, and timely progress notes in applicable record systems
Maintain accurate and current case records and complete required monthly reporting and statistics.
Coordinate services with the Monroe County Department of Human Services, Child Protective Services, Preventive Unit, and other community providers.
Facilitate referrals and connect children and families with appropriate community resources and support.
Collaborate with community partners to strengthen referral networks and promote positive outcomes for families.
Participate actively in individual and group supervision, reflective practice, team meetings, and required trainings.
Provide community education and outreach when requested.
Transport or accompany clients to services when appropriate.
Support the training and supervision of interns and volunteers.
Requirements:
COMPETENCIES/SKILLS:
Strong knowledge of child development, attachment theory, trauma, child maltreatment, family systems, and culturally responsive practice.
Demonstrated ability to provide ethical, strengths-based, and trauma-informed clinical services to children and caregivers.
Clinical psychotherapy skills necessary to support children and families impacted by trauma, loss, poverty, and/or child maltreatment or abuse.
Professional reflective capacity and an understanding of trauma informed care, equity work, and ethical practices.
Willingness to engage in building reflective practice (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.)
Detail-oriented, organized and strong communication skills (both verbal and in writing)
QUALIFICATIONS:
Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, Psychology, Creative Arts Therapy, or another license-eligible behavioral health field.
Minimum of two (2) years of experience providing services to children and families impacted by trauma, abuse, neglect, or other adverse experiences.
Must be currently licensed in New York State in an appropriate mental health discipline or hold an active New York State limited permit at the time of hire or hold a limited permit or licensure within 6 weeks of hire. Continued employment is contingent upon maintaining active licensure or limited permit status in good standing throughout the duration of employment.
Experience with Child-Parent Psychotherapy (CPP) and/or other trauma-focused evidence-based practices preferred.
Interest in or experience working with infants and young children (ages 0–6) and their families from an Infant and Early Childhood Mental Health (IECMH) perspective preferred.

Silver Regional Sexual Assault Support Services (SRSASS) is seeking a compassionate, trauma-informed Licensed Therapist to provide high-quality mental health services to survivors of sexual assault, child abuse, interpersonal violence, and other forms of trauma.

* Licensed as an LCSW for at least 2 of the past 5 years prior to the start of supervision.
* Completion of the BBS-required 15-hour clinical supervision training course, or willingness and ability to complete the course within 30 days of hire.

* Complete initial and follow-up palliative care assessments
* Evaluate symptom burden, disease trajectory, functional and cognitive status, caregiver needs, safety concerns, and goals of care
* Diagnose and manage palliative symptoms within NP scope and competency
* Order appropriate medications, labs, diagnostics, DME, referrals, home health services, and hospice evaluations

Summary:
This role is responsible for overseeing the day-to-day operations and guest experience of Conine’s Clubhouse, the hospital’s on-site family hospitality house for families of critically ill children. Responsibilities:

Atrium Health Outreach Athletic Training partners with area middle schools, high schools, and community athletic programs to provide comprehensive sports medicine services focused on injury prevention, evaluation, treatment, and return-to-play management. Athletic trainers work closely with physicians, coaches, athletes, and families to deliver high-quality care both on-site and in clinical settings while promoting athlete health, safety, and performance throughout the season.
Job Description:
The outreach athletic trainer is responsible for the day-to-day medical coverage in secondary school settings (middle school and high school). Uses preventative measures appropriately; provide standard immediate care procedures used in emergency situations; clinical evaluation, treatment, rehabilitation, and reconditioning. Employ appropriate assessment outcomes measures to determine effectiveness of the outreach program; determines appropriate return to play of athletes and active population. Management of head injuries and concussions; referral to appropriate medical professionals to ensure optimal level of care; all duties and responsibilities performed under the supervision of a directing physician. Also works collaboratively with the Sports Medicine Team to ensure service excellence for all customers of the athletic training outreach program.
Major Responsibilities
- Provide athletic training services to secondary schools and community sporting events as determined necessary to attain departmental goals of community service.
- Works within the Sports Medicine Team concept and assists with event coverage as necessary to attain departmental goals of community service.
- Provides acute medical care to athletes 'on the field' during practices or sporting events.
- Provides on-site recognition, evaluation, and immediate care procedures of illness and injury including emergency situations, independent of setting.
- Perform clinical injury evaluation and assessment of patient's level of function and follows standardized clinical practice in the area of diagnostic reasoning and medical decision making.
- Determine appropriate therapeutic treatment and provides rehabilitation, and/or reconditioning strategies, and taping / wrapping and bracing for preventative and protective treatment.
- Implement treatment program objectives including long- and short-term goals and an appraisal of those which the patient can realistically be expected to achieve from the program. Assessment measures to determine the effectiveness of the program are incorporated into the program.
- Collaborate with their directing physician, recommend discontinuation of the athletic training service when patient has received optimal benefit of the rehabilitation and treatment program, and notes final assessment of patient status.
- Determine return to play standards of injuries for athletes and the active population.
- Maintain daily records of treatment logs, injury reports, doctor's referrals, and medical inventory management used for the care of athletes according to clinic standards.
- Complete set-up of all practice and games including medical supplies, fracture management equipment, hydration, cryotherapy, and any other supplies needed for adequate coverage.
- Assist with planning, organizing, implementing and compliance of departmental and policies.
- Ensures an Emergency Action Plan for each venue be put in place and discussed with the school's Athletic Director, coaches, and athletes, including local EMS. Exhibit ability to perform essential duties efficiently and accurately with or without reasonable accommodations and without endangering incumbent or other employees.
- Assists with the preparation of lectures, sports medicine programs, seminars and serves as a presenter in these programs when needed.
- Mentor and manage on site education of students enrolled in university-based athletic training education programs or any other healthcare professionals at outreach events (if applicable).
- Ensure all athletic training room sites have appropriate supplies and equipment; recommends new equipment for acquisition and use by the organization.
- Develop physician referral network including family practice, orthopedic, physical therapy & other specialties as needed.
- Facilitates patient entry into the healthcare system and properly track and document these referrals.
- Maintain necessary training on concussion education, management, and care, including acute management of the cervical spine.
- Adheres to departmental dress code when providing on-site Athletic Training services.
- Perform other related clinical duties as required incumbent to State Board of Medicine/ Board of Athletic Training Statue and Rules.
Minimum Job Requirements
Education:
Preferred:Bachelor’s, Master’s, or equivalent degree from a CAATE-accredited athletic training program.
Minimum requirement is a Bachelors' degree in early childhood development, early childhood education, child and family development, nursing, speech and language pathology, occupational therapy, physical therapy or a related human services field.
Certification / License:Current athletic training certification by the Board of Certification for the Athletic Trainer (BOC). Currently licensed or eligible for a State of applicable state License in Athletic Training within 30 days of hire. Ability to complete American Heart Association Healthcare Provider (BLS), which includes CPR and AED training within 30 days of hire.
