*Job Overview*

We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will possess a strong background in physiotherapy, with expertise in developing treatment plans, managing patient care, and collaborating with healthcare professionals. This role offers an opportunity to make a meaningful difference by providing high-quality outpatient and home health rehabilitation services to diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases.

*Duties*

* Assess patients' physical conditions and develop individualized treatment plans aimed at improving mobility, strength, and functional independence.
* Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs tailored to each patient's needs.
* Monitor patient progress regularly and adjust treatment strategies accordingly to optimize outcomes.
* Collaborate with physicians, nurses, and other healthcare professionals to coordinate care plans and discharge planning processes.
* Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
* Educate patients and their families on exercises, mobility techniques, and home care strategies to promote ongoing recovery outside clinical settings.
* Manage discharge planning effectively by ensuring patients are equipped with appropriate care instructions for home or community-based care environments.
* Maintain compliance with healthcare regulations and safety standards across inpatient, outpatient, hospital, and home health settings.

*Requirements*

* Valid licensure as a licensed Physical Therapist in the relevant jurisdiction with practicing experience in physiotherapy roles.
* Demonstrated experience working in outpatient clinics, inpatient hospital settings, or home health environments.
* Proficiency in clinical documentation standards and electronic health records (EHR/EMR) management systems.
* Ability to collaborate effectively with multidisciplinary healthcare teams and communicate clearly with patients of diverse backgrounds.
