Job Description:
The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.
Scope
Provides care appropriate to assigned patient population as established with the plan of care/treatment plan and in accordance with professional licensure. This care may take place at a hospital, facility, clinic, inpatient, outpatient or home care setting.
Job Essentials
Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility. Completes documentation as required.
Performs social work consultation for patients and caregivers. Conducts psychosocial assessments. Under supervision of a licensed clinical social worker (LCSW), provides supportive, crisis, bereavement and other social work interventions as necessary.
Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues.
Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.
May provide psychotherapeutic modalities appropriate to the level of care under the supervision of a licensed clinical social worker.
Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement standards.
May ensure that productivity standards and expectations are met.
Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.

May provide support and debriefing under the supervision of a licensed clinical social worker, in conjunction with the Employee Assistance Program, for staff members following adverse events.
May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.
Provides support and education to patients, families and staff members on Advance Directives.
May maintain an active role in denial prevention. Proactively intervenes with payers to prevent inpatient denials. Communicates any necessary information to payers to help appeal existing denials.
May coordinate the discharge planning process from patient admission to departure. Screens for and assess patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.
Works under identified LCSW and actively participates in required supervision through scheduled meetings, consultations, and chart reviews as indicated.
Note: The Certified Social Worker Intern License can be utilized for up to 24 weeks (6 months) while employees are working toward preparing, testing, and passing the State Boards examination (obtaining an CSW license). After that 24 week period, or if the CSW Graduate fails the State Board exam a second time, the employee can no longer remain in the CSW-I License. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e. SSW, psych tec, etc.). Failure to either pass boards or to be hired into another job will result in termination.
Minimum Qualifications
Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.
Current State Licensure as a Certified Social Worker.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

Overview of the Acute Care Services Summer Internship
The Acute Care Services Administrative Summer Internship is designed to augment classroom learning and to prepare future healthcare leaders for opportunities upon graduation.
The incumbent will work directly with members of the leadership team on a variety of short-term strategic and operational improvement projects. In addition, the intern will partner with an executive mentor, attend leadership meetings and participate in development workshops.

The Nursing Graduate works under the direct line-of-sight supervision of the Registered Nurse (RN) in the clinical setting and participates appropriately in the patient care delivery process. The incumbent also orients to the RN assignment, excluding independent nursing care. This position may participate in medication administration under the direction and supervision of the licensed RN.
The Nursing Graduate job can be utilized for up to 12 weeks while employees are in the Intermountain Nurse Residency program and are working toward preparing, testing, and passing the State Boards examination (obtaining an RN license). After that 12-week period, or if the Nursing Graduate fails the State Board exam a second time, the employee can no longer remain in the Nurse Graduate job. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e., CNA, etc.). Failure to either pass boards or to be hired into another job will result in termination.
Registered Nurse graduates must be enrolled and have started coursework in the Intermountain Nurse Residency program before providing direct patient care (including direct patient care supervised by a licensed RN).

Essential Functions
- Demonstrates understanding of the steps in the nursing process and how the patient plan of care is developed, implemented, and revised to meet the needs of the patient (under direct supervision of an RN).
- Assessment: Collects patient data (e.g., vital signs) under the direct supervision of the RN and reports significant changes in patient condition. Completes accurate documentation also under the direct supervision of the RN.
- Planning: Observes and discusses with the RN the prioritization of patient needs among all patients assigned to their care during any one shift, and observes the development and modification of the interdisciplinary plan of care used to care for individual patients (under the direct supervision of the RN).
- Implementation: Observes the implementation of the patient plan of care, giving consideration to specialized needs of patients such as their physical, emotional, and social needs, age, ethnic origin, and other factors (under the direct supervision of the RN).
- Evaluation: Reports patient responses to treatments and collaborates with the RN regarding changes requiring modification of nursing interventions to meet patient needs.
- Participates in patient/family education needs, under the supervision of the RN, as determined by initial and ongoing assessments, patient diagnosis, level of learning readiness, and patient/family/SO requests using other disciplines as appropriate.
- Demonstrates responsibility and accountability for patient care delivery within their defined role (under the direct supervision of the RN), and routinely observes and discusses all aspects of care with the RN regarding patient care delivery.
- Recognizes and demonstrates legal/ethical implications of care delivery by practicing as a Nursing Resident under the supervision of the RN, complying with regulatory standards, completing accurate documentation, and maintaining confidentiality.

Sterile Processing Department (SPD) Techs are asked to perform the four basic departmental functions (decontamination, prep and pack, sterilization, and dispatch) with efficiency, expertise, and accuracy. • Assembles supplies and places into case carts for scheduled and non-scheduled procedures

10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.

Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.

- Performs the duties associated with the operation of the nuclear pharmacy as defined in the Radiotherapy Policy and Procedure.
- Provides care for the physical needs of patients by executing various nuclear and CT imaging scan concepts to maximize performance. This includes: positioning, physiology, procedure preparation, and technical factors. Modifies technical factors to achieve desired imaging results and recognizes complications / limitations associated with certain procedures. Proactively identifies how to address these for optimum outcomes.
- Schedules nuclear & CT imaging procedures appropriately and performs other administrative tasks related to the development of new diagnostic procedures and/or imaging areas as assigned. Moves between modalities as needed for the most efficient and effective service and outcomes.

Major Responsibilities:
- Under the observation of a licensed respiratory therapist, may perform routine and basic level respiratory therapy procedures including oxygen therapy, pulse oximetry, aerosol therapy and incentive spirometry.
- Organizes and maintains supply of respiratory therapy equipment and supplies as needed by department specific standards.
- Collects equipment from all the areas of the hospital and cleans and reassembles equipment according to proper procedure observing standard precautions. Processes disposable/non-disposable equipment based on infection prevention guidelines.
- Completes annual hospital competencies. Learns new equipment/supplies, procedures, and processes as needed to perform their job as appropriate.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique (if applicable).
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Major Responsibilities:
- Uses Respiratory therapy processes to support assessment of patient utilizing a variety of skills such as pulmonary diagnostics, chest radiograph, hemodynamic values, laboratory values, and correlates blood gases and electrolytes. Major Responsibilities:
- Uses Respiratory therapy processes to support assessment of patient utilizing a variety of skills such as pulmonary diagnostics, chest radiograph, hemodynamic values, laboratory values, and correlates blood gases and electrolytes. Can identify and address the special needs of patient population specific to department services.
- Operates specialized ventilators in critical and non-critical care venues. Assists physician with bedside or other procedures as applicable to facility.
- Provides medication treatments and therapies per physician order and/or patient driven respiratory protocols as prescribed.
- Cleans, maintains, and ensures proper functionality of equipment. Troubleshoots equipment as necessary and identifies the need for service and/or repair.
- Educates and instructs patients in purpose, technique, self-care, goal of therapy, and disease management.
- Facilitates clinical problem solving and collaborates with other departments to provide education on managing patients.
- Under physician direction, the respiratory therapist must be able to adapt and comply to policies and procedures for a variety of different service areas, perform tasks and/or special duties as required by assignment and maintain performance standards and organizational behaviors.
- Demonstrates effective communication, feedback, and conflict resolution skills. Provides clear, concise, timely, and accurate patient handoff.
- Promotes a culture of safety by recognizing and applying proper safety measures to prevent patient harm. Reports patient safety events and near misses in a timely manner. Seeks to identify potential safety issues and assists in the implementation of corrective actions.
- Teaches, educates, and precepts new teammates, orientees, and students.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.
Licensure, Registration, and/or Certification Required:
- Respiratory Care Practitioner (RCP) license issued by the state in which the team member practices.
- Active Registered Respiratory Therapist (RRT) credential issued by the National Board for Respiratory Care (NBRC) within 12 months OR active Certified Respiratory Therapist (CRT) credential issued by the NBRC if credential received prior to January 1, 2023.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).

Education Required:
Graduate from an accredited respiratory care program, with a minimum of an associate degree.
Experience Required:
No experience required.
Knowledge, Skills & Abilities Required:
- Effective organizational, interpersonal and problem-solving skills.
- Problem solving and decision making for patient care and equipment use.
- Proficient computer skills, use of e-mail and electronic medical record systems.
- Excellent communication (written and verbal), listening and interpersonal skills.
- Demonstrates human relation skills with the ability to develop rapport and maintain positive, professional relationships with a variety of patients, team members, and physicians. Ability to take initiative and work collaboratively with others.
- Demonstrates time management skills with an ability to multi-task and prioritize in an environment with frequent interruptions.
Physical Requirements and Working Conditions:
- Operates all equipment necessary to perform the job.
- Must be able to lift 50 lb object from knee to waist.
- Must be able to lift 50 lb from floor to waist height occasionally.
- Must be able to lift 40 lb at waist height and carry it across various distances.
- Must be able to lift 20 lb from waist to overhead.
- Must be able to push/pull while utilizing 30# of force occasionally for long distances.
- Must demonstrate stand-pivot transfer from bed to chair.
- Must perform sliding transfer of 150 lbs. person on a sheet across 2 feet with a second person.
- Must utilize safe body mechanics at all times.
