RN Quality Analyst
32 Weekly Hours, Full Time, Dayshift
Job Summary:
Assist with the process to gather, compile, and summarize medical staff quality data utilized for the evaluation of care and to identify recommendations for improvement.
Facilitates the clinical evaluation process including the utilization and analysis of data and sensitive information to support medical staff recommendations and actions.
Facilitates process for focus and ongoing performance evaluation of physicians. Contributes to ensuring compliance with the accreditating body medical staff standards and participating in the survey process.
Assist with physician quality profiles necessary for reappointment and onboarding.
Assists with the coordination of the Medical Staff Patient Care Committees that evaluate the quality of care provided and patient outcomes within the organization.
Assists with additional Quality Improvement efforts as directed by the Quality Resource department.
Responsible for establishing effective and collaborative relationships with physician leadership and physician advisors.
Responsible for integrity of data submitted/reported.
Participation with quality committees and initiatives as determined.
Minimum Education, Training and Experience Required:
Active KY Registered nurse with a minimum of 3 years clinical experience as well demonstrated experience in clinical performance improvement.
BSN required
Experience as a facilitator in managing sensitive data and issues with staff and physicians

Patient Access Coordinators are imperative to the overall success of the entire revenue cycle. Most of the information submitted on a claim derives from an Access team member. They are primarily responsible for completing a thorough admissions process of all inpatients and outpatients. Exemplary customer service skills are crucial in this role, as the Patient Access Coordinator must ensure that accurate information is collected, that they are always sensitive to the confidentiality of this information, and that patients are aware of their rights as determined by HIPAA regulations and company policy. Extensive Insurance experience/training is vital to success within this role. Employee will maintain compliance with state and federal regulatory body requirements regarding access services. Verifying Insurance Eligibility, Compliance, Point of Service estimates and collections are part of their responsibility.

Job Description:
May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. Meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
- Provides enhanced patient care under the direction of the licensed nurse, which may include but is not limited to advanced dressing care, suctioning, removal of Foley Catheter and peripheral IV.
- May perform venipunctures and skin punctures to obtain blood specimens according to Clinical Laboratory Standards Institute guidelines.
- May perform daily, hands-on aspects of the storage, preparation and delivery of breast milk, infant and pediatric formulas.
- Documents in the appropriate sections to create a complete and accurate medical record.
- Identifies, observes, and documents the physical needs of the patient according to established standards. Reports any changes in patient condition to licensed personnel in a timely manner.
- Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
- Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
- Provides one to one patient safety watch as assigned following established policies and procedures.
- Accurately processes specimens and provides timely transport of the specimen to the laboratory.
- Performs point of care patient testing and point of care instrument maintenance and quality control.
- Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
- Serves as a preceptor and mentor for new staff and students in a positive manner.
- Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard.
- Releases and confirms ordered tests to appropriate departments.
- Coordinate's transportation of patients to multiple tests.
- Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consult, etc.
- Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies.
- Communicates with food services to ensure appropriate patient diets.
- Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc.
- Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit.
- Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed.
- Assists with transporting blood products to and from Blood Bank as needed.
- Discharge follow up phone calls / coordinating virtual discharge appointment team.
- Support / Assist staff with 5 min Drills and/or Quality Rounds - participate in regulatory readiness as needed.

Position Details:
HfAM7 Cardiac Intensive Care Unit
Schedule:Three 12-hour shifts weekly including Day/Evening hours (7:00am - 7:30pm/11:00am-11:30pm) and every other weekend and holiday requirements.
Required Orientation:At the start of employment, a two-week classroom orientation is required with a schedule of Monday - Friday, 7:00am - 3:30pm.
No experience needed. Comprehensive on the job training provided.

Job Duties:
- Provides technical expertise to operate CT Scanners, power injectors and computer systems to acquire images for use by the radiologist in the diagnosis and treatment of pathologies.
- CT technologist must maintain the knowledge to operate different CT Systems at multiple locations.
- Maintain protocols that are based on Image Gently Guidelines and CT scanner’s integration into a program that compiles dose data from each scan performed and notifies the user when a scan exceeds specific preset levels.
- Maintain vast knowledge of all clinical histories to match appropriate protocols and knowledge of all major organ systems and vascular systems for Thoracic, Abdominal, Pelvic, Neuro systems and Muscular Skeletal.
- Obtains CT images according to established protocols.
- In doing so, ensures the patient is properly prepared, positioned, immobilized, protective shielding used when applicable and correct technical factors are selected.

MINIMUM QUALIFICATIONS:

The Medical Emergency Response Interventional Team (MERIT) is staffed by highly experienced ICU nurses with diverse specialty backgrounds who use technology, innovation, and education to improve patient and quality outcomes. The MERIT nurse responds to and leads clinical bedside emergencies throughout the campus. Partners with the provider and nursing staff to optimize code response in terms of efficiency and to decrease incidence of inpatient hospital mortality. Proactively responds to elevated MEWS alerts and nursing concerns at the bedside to prevent patient decompensation. 12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.

15. Replenishes patient and unit supplies and equipment.

- Assesses patients regularly, monitors progress towards treatment goals, modifies goals, and plan or program as indicated. Proactively anticipates complications/limitations associated with certain procedures and addresses such for optimum outcomes.
- Coordinates, monitors and reports on various quality and safety metrics for the practice area and/or department. Develops and implements initiatives to improve the patient experience Proactively identifies opportunities for corrective action, leads process improvement providing recommendations, and contributes to meeting departmental and hospital targets for quality and safety.
- Oversees the work flow, caseload and patient processes and treatments for assigned team/area, ensuring quality outcomes and the safety of the patient, adjusting activities as applicable. Coordinates activities for the service line, shift, and/or clinics and as requested for the department.
FINANCE ESSENTIAL FUNCTIONS
- Monitors the accuracy and completion of documentation of patient education and procedures performed in a timely manner in the patient record. Accurately enters charges for services provided and performs charges reconciliation as applicable. Serves as resource to management for chargemaster item/CPT (Code and Procedure Terminology) code maintenance and development for physical therapy charges.
- Uses resources efficiently to maximize utilization of time and equipment. Self-motivated to independently manage time effectively, minimizing incidental overtime, and prioritize daily tasks for self and area(s) of responsibility/assigned team members to meet the labor productivity standards.
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
- Develops patient treatment protocols and best practices with research-based evidence and offers innovative solutions for patient population or practice area. through performance improvement projects.
- Leads department projects, programs and shared governance activities. Actively participates in and/or leads HM-related and approved hospital/community teaching opportunities, community services and/or functions.
- Initiates opportunities to identify self-development needs and expand learning beyond baseline competencies with a focus on continual development as a Physical Therapist, taking appropriate action. Assists management with lower level PTs and program development and/or clinical competencies. Ensures own career discussions occur with appropriate management. Completes and updates the My Development Plan on an on-going basis.
