The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient’s assessment, plan of care and care plan, as may be directed by supervisors.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. High school diploma or equivalent
2. Current Certified Nursing Assistant Certification issued by state where services are provided or due within three (3) months of hire.
WV: Nurse's Aide Certification through the West Virginia Department of Health and Human Resources
3. State criminal background check and Federal (if applicable), as required for regulated areas
4. Obtain certification in Basic Life Support within 30 days of hire date.
PREFERRED QUALIFICATIONS:
EXPERIENCE:
1. One (1) year nursing assistant experience in long term care.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an exhaustive list of all responsibilities and duties. Other duties may be assigned.
1. Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner per Policy/Procedures and assures they are correct.
2. Measure and record temperatures, pulse, and respirations per Policy/Procedures.
3. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, communication, safety, education level and related criteria for the adult geriatric patient per the established age-specific standard.
4. Demonstrates the ability to use the following equipment according to manual: vital signs instrumentation, k-pad, scales, oxygen tanks, drainage systems/suction, SCDs, CPM machine, and Bladder Scanner.
5. Assist patients with daily hygiene (dental care, mouth care, bath, hair care, backrubs, nail care, and shaving, etc.
6. Assist patients with nail care, which includes cleaning and clipping, except diabetic patients per policy/procedure.
7. Turn and position bedfast patient every 2 hours and as needed. Assist patients with bowel and/or bladder functions. Assist patients in Bladder maintenance and bladder or bowel retraining programs.
8. Answer call bells promptly per policy /Procedure.
9. Assist with the care of the dying patient. Provide postmortem care per Policy/Procedure.
10. Provide perineal and indwelling catheter care on your shift and as necessary per Policy/Procedure.
11. Assist patients with (ADL) activities of daily living skills with and without adaptive equipment as per instructions of therapist or nurse.
12. Serve food trays per standard. Assist with feeding as indicated. Record food and fluid intake. Assist patients with identifying food arrangements with sight problem of foods or his/her tray.
13. Serve between meal and bedtime snacks.
14. Assist with lifting, turning, moving, positioning, and transporting patients into and out of bed, chairs, bath, wheelchairs, etc. utilizing proper body mechanics.
15. Report all allegations of patient abuse and/or misappropriation of patient property as per federal requirements and per policy/procedure.
16. Report all accidents and incidents you observe on the shift that they occur per Policy/Procedure.
17. Assists patient with specialized feeding techniques as per recommendations of the speech therapist in accordance with policy and procedure.
18. Assist with ambulation, gait training and CPM administration as instructed by the physical therapist.
19. Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist.
20. Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients who requires more than one person assistance.
PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shift.
2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
3. Hearing within normal range is required. (i.e.: to assess breath sounds, bowel sounds, apical pulse, monitors, etc.)
4. Visual acuity must be within normal range.
5. Must be able to perform heavy work: Exerting in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects

Education

Physical and Environmental Demands

Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. Education

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated. Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately. Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices.
- Performs tasks related to nutritional needs of the patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection. Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. - Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required. Under direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care and perform bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system.
- Facilitates communications required to support patient care, including the multiple-line telephone, unit/site paging and the hospital information system. Screens incoming telephone calls. Monitors department office equipment and troubleshoots minor problems. Arranges repair of major malfunctions.
- Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. Verifies medical records contain complete and accurate patient demographic information, patient identification and registration in the medical record, including appropriate level of care.
- Establishes and maintains positive customer relations and contributes to a harmonious work environment. Quickly and courteously acknowledges requests, responds to call lights, demonstrates proper phone etiquette and fosters communication between the health care team.

- Phlebotomist II is responsible for accurately collecting patient specimens for diagnostic testing while working in a professional medical environment. This position functions as part of a dynamic and engaging team with a workload that ranges from fast-paced hospital settings and high/low volume clinics. Caregivers in this role will have valuable experience and knowledge that brings a higher level of expertise to laboratory services’ phlebotomy.
- The Phlebotomist II performs specimen collection using many different techniques such as venipuncture, capillary puncture, PIVO, and throat swab collections. These caregivers collect specimens from patients of various acuities, such as Outpatient, Emergency Department, and Inpatient – including newborns, patients in intensive care, and all other areas of the hospital. They are at the forefront of patient care and adhere to and enforce the standards for laboratory compliance and safety regulations.
- Adherence to personal protective requirements in inpatient and/or outpatient settings, including but not limited to gowns, gloves, lab coats, face shields, and masks.

The Mother baby unit at Logan Regional Hospital is a 29 bed unit. We primarily take care of the mother baby couplet after delivery through the rest of their stay. We also take care of GYN surgeries. We care for all acuities of postpartum patients and take care of newborns 35 weeks and above. We have an excellent lactation team that is here to support us and our patients in establishing breastfeeding. We strive to help each one of our patients, adult and newborn, to have the best experience possible. We are family oriented. The team here in Logan is invested in continually improving and we have great corroboration between the other women and newborn units here in the hospital.

- Phlebotomist is responsible for accurately collecting patient specimens for diagnostic testing while working in a professional medical environment. - The Phlebotomist performs specimen collection using many different techniques such as venipuncture, capillary puncture, PIVO, and throat swab collections. These caregivers collect specimens from patients of various acuities, such as Outpatient, Emergency Department, and Inpatient – including newborns, patients in intensive care, and all other areas of the hospital. They are at the forefront of patient care and adhere to and enforce the standards for laboratory compliance and safety regulations.
- Adherence to personal protective requirements in inpatient and/or outpatient settings, including but not limited to gowns, gloves, lab coats, face shields, and masks.

Essential Functions
