Job Description:
Provides nutrition services to patients utilizing standardized guidelines. This position interacts with clinical caregivers, patients, and patient’s family members to explain the meal process and modify meal selections according to provider orders.
Posting Specifics
Pay Rate: Based on Experience
Shift Details: Part-Time (30hrs/wk). Every other weekend and PM shifts only 10:30am-7pm or 11:15a-7:45pm or 3pm-7pm or 3:30pm-7:30pm or 3:45pm-7:45pm
Additional Details: Not located in the new main hospital campus, but in an office building on the old campus.
Essential Functions
- Takes patient meal selections and modifies them using system standards to meet provider orders.
- Checks trays for accuracy during meal assembly.
- Communicates clearly to both clinical and culinary caregivers.
- Collects and inputs nutrition screening information
- May complete calorie count and nutrition analysis as dictated by facility
- Utilizes a computer to run reports and take orders.
- Delivers and presents trays to patients using the standard process.
- Performs food preparation functions according to system standards (following standard recipe, assembling ingredients) for example, prepares salads, sandwiches, vegetables, and fruit.
- Follows standardized practices relating to Nutrition Services (e.g. dining experience, meal delivery and maintaining required stock levels)
- Performs accurate credit transactions according to system standards and independently resolves basic customer service issues.

Qualifications
- Food Handler Permit or ServSafe Certification is required by first day of work (required ONLY in the states of Utah and Idaho). Remote and off-site caregivers are exempt.
- Demonstrated ability to work with modified diets (preferred)
- Demonstrated ability to provide exceptional customer service (preferred)
Physical Requirements
- Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess customer needs.
- Frequent interactions with customers that require employee to communicate as well as understand spoken information, alarms, needs, and issues quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer, phone, and cable set-up and use.
- Expected to lift and utilize full range of movement to transport, pull, and push equipment. Will also work on hands and knees and bend to set-up, troubleshoot, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
- Remain standing for long periods of time to perform work.
- Tolerate extremes in temperature such as performing work at a grill or in a refrigerator and tolerate exposure to cleaning chemicals.

Position Purpose:
The Rehabilitation Tech is responsible forpreparing treatment areas, cleaning treatment areas, obtainment of supplies, transporting, positioning and inassistingpatients/clients,therapistsand therapist assistants in the completion of the therapy treatment plan. This may include vitals.
Collaborates with clinic staff and assists with the safe transporting and transferring of patients.
Contributes to complete and accurate patient records performed under the direct supervision of a licensed clinician.
Completes and records vitals and L-Dex Lymphedema Test (Lymphedema Clinic) for patient care.

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated. Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately. Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices.
- Performs tasks related to nutritional needs of the patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection. Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required. Under direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care and perform bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system.
- Facilitates communications required to support patient care, including the multiple-line telephone, unit/site paging and the hospital information system. Screens incoming telephone calls. Monitors department office equipment and troubleshoots minor problems. Arranges repair of major malfunctions.
- Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. Post-processes images as directed and accurately identifies and labels images identifying patient name, medical record unit (MRU) number, and location or position of scan on all images.
- Oversees the maintenance of records in accordance with various regulatory agencies and enters patient and study information into the radiology information system, and documents billing codes of procedures performed to ensure accurate patient charges.
- Identifies equipment performance issues and ensures and/or performs equipment quality assurance tests and calibration procedures. Reports malfunctioning equipment to appropriate staff and evaluates new products and equipment.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

Sets the standard for the department regarding following standards, maintaining regulatory requirements, and modeling expected behaviors within the department. Duties include obtaining patient specimens via venipuncture or micro method.
Major Responsibilities:
• Collects and/or processes patient blood obtained by venipuncture and/or capillary puncture in a timely and accurate fashion.
• Use of vacutainer tube, butterflies, syringes, and needle for drawing blood. Use of micro lancets and capillary pipettes in obtaining blood specimens by heel stick.
• Performs Positive Patient identification by verifying the patient’s name and date of birth against lab orders and accurately record it onto the specimen.
• Determines proper container type.
• Promptly draws and delivers blood specimens to the appropriate laboratory department.

Job Description:
30 Hours Per Week Position - Sunset Instacare in St. George, Utah.
The Registered Nurse (RN) is a licensed professional caregiver who assumes responsibility and accountability for assessing, planning, implementation and evaluating care of patients. The staff RN utilizes the nursing process by use of technology, therapeutic intervention, and coordination of care with other health team members.

Minimum Qualifications
- Current RN License in state of practice.
- Basic Life Support Certification (BLS) for healthcare providers.
- Practice specific certification as required by the clinical setting in which they work (i.e., ACLS, PALS, etc.).
- Ability to communicate effectively both verbally and in writing.
- Basic computer skills.
RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within five years of their start date.
Preferred Qualifications
- Bachelor's Degree in Nursing (BSN) from an accredited institution.
Scope
Staff RNs are pivotal members of the care delivery team headed by the physician and have specialized knowledge, skills and ability pertinent to ambulatory care patient needs. Scope of performance includes prioritization and triage of care delivery, implementing established standing orders and protocols, and performing procedures and therapies consistent with training and licensure. This position requires specialized knowledge and increased accountability, specific to high acuity and/or specialized patient populations (i.e. Instacares, Obstetrics, Infusion services, Sedation monitoring, etc).
Job Essentials
Assessment - Performs appropriate assessment of physical, social, and psychological status (including cognitive, communicative and developmental skills as appropriate); seeks and evaluates information acquired from other members of interdisciplinary team: patient (e.g., family, physician, non-nursing disciplines, support staff).
Planning - Uses assessment information and critical thinking skills to collaboratively develop individualized plan of care; actively seeks patient and family involvement to develop plan of care; collaboratively plans and prepares patients for further support; prepares patients for further support needs once the current episode of care outcomes are met (i.e., teaching, referrals, and follow-up).
Implementation - Directs the interdisciplinary care team via delegation, coordination, and collaboration.
Evaluation - Evaluates patient response to interventions through review of achievement of goals, clinical outcomes, patient and family satisfaction; revises plan of care as indicated, and reassesses changes as appropriate.
Professional Development - Promotes the nursing profession and participates in development of others; promotes an appropriate professional relationship between the nursing staff and physicians; integrates legal and ethical standards into practice: comply with regulatory standards and Intermountain policy, practice within scope of licensure, provide accurate & timely documentation, and understand legal implications of care delivery; meets initial, annual and ongoing competency requirements for job functions that are outside of basic RN training (i.e. clinical procedures, pertinent laboratory tests, use of equipment and supplies).
Actively participates in quality improvement initiatives (i.e. CQI, TQM, Lean, 100% participation, etc.).
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
