This job functions autonomously to deliver physical therapy treatment, either through referral or direct to consumer. Evaluates patients, develops treatment plans, assesses patients, and monitors progress toward treatment goals. Ensures patient needs are met through access to care, patient experience and quality of care metrics. Assists in program development and enhancement to meet patient and community needs.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education
Required - Bachelor's Degree in physical therapy.
Work Experience
Required - None.
Certifications
Required - Current physical therapy license in the state of practice

Basic Life Support (BLS) from the American Heart Association.
Knowledge Skills and Abilities (KSAs)
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
Excellent judgment and decision-making skills in order to deal with a variety of patients and diagnoses.
Strong interpersonal skills.
Ability to remain flexible with fluctuating schedules based on patient demand.
Job Duties
Performs evaluations and provides treatment programs to facilitate patient specific goals.
Assesses the patent’s response to treatment and recommends treatment plan changes as appropriate.
Maintains patient records according to documentation standards, policies, and procedures.
Attains established productivity levels set forth by administration, including but not limited to access to care, patient experience and quality.
Assists in program development and enhancement to meet patient and community needs.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Physical and Environmental Demands

Very Heavy Work - Exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Heavy Work. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. This job works in a collaborative model including but not limited to: residents, fellows, attendings, and staff who care for outpatient clinic patients of the organization. This job works under the supervision of the Medical Director or approved staff physician and in association with the PCCs, staff nurses, and discharge coordinator in the planning, implementation and evaluation of medical and nursing care; and assumes responsibility for primary and urgent medical management, emergency stabilization of all critically ill patients, teaching of residents and medical students, staff development and education, patient education and discharge planning.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education
Required - Master’s degree in nursing.
Required - Bachelor’s degree in Physician Assistant medicine or Health Sciences, or graduate of an approved Physician Assistant Training Program.
Work Experience

Preferred - 3 years of related working experience.
Certifications
Required - Current License as an Advanced Practice Registered Nurse (APRN) in the state of practice.
Family Nurse Practitioner or Adult/Geriatric Nurse Practitioner ANCC or AANPCP Certified for Family Nurse Practitioner or Pediatric Nurse Practitioner ANCC or AANPCP Certified.
Required - License as a Physician Assistant in the state of practice, NCCPA Certified, Basic Cardiac Life Support (BCLS) / Advanced Cardiovascular Life Support (ACLS) Certification from the American Heart Association.
Eligible for Prescriptive Authority in the state of practice as outlined by the State Board of Medical Examiners.

7. Assures the efficient operation of workflow of the department. Monitors and maintains an adequate inventory of supplies and material to ensure non-interruption of services.
8. Independently performs prescribed procedures in accordance with department/facility policies, procedures and protocols to patients. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, patients and family members.
MINIMUM QUALIFICATIONS
Certificate or diploma from an approved/accredited Radiologic Technology programs. Requires national certification from the American Registry of Radiologic Technologists (ARRT), Nuclear Medicine Technology Certification Board (NMTCB), or American Registry for Diagnostic Medical Sonography (ARDMS) based on modalities in which they will be conducting work and licensure by the state regulatory agency as applicable. As of August 1, 2019, all Multi-Modality Technologist's hired on or after August 1,2019 will be required to have an advance certification by an accrediting body in a specialty if applicable. BLS certification required.
This position requires clinical knowledge typically achieved with 2 years of Radiologic Technologist experience. Must be actively working in each certified modality, in accordance with the needs of the facility. The Multi-Modality Technologist I will be required to rotate and cover department scheduling as needed and required in areas where obtained certifications apply. Multi-Modality Technologist I requires certification in two (2) or more of the following modalities: Radiologic (X-ray), Computed Tomography (CT), Bone Densitometry (DEXA), Mammography, Breast Sonography, Angiography.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, and BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. An Arizona Criminal History Affidavit must be signed upon hire.

$43.77 - $72.95 / hour
