Job Description:
Pediatric Care Coordinator:Coordinates care for pediatric patient populations in order to increase effectiveness of preventive health care measures. This includes collaborating with health care providers and conducting outreach to patient families for follow up, scheduling, and resourcing. Monitors and enrolls patients for various preventative healthcare campaigns.

Posting Specifics:
- Schedule: Shifts are scheduled Monday through Saturday between 8:00 a.m. and 7:00 p.m. Mountain Time.
- Hours: Full-time remote position, 40 hours per week.
- Scheduling: Individual schedules are determined based on business needs.
- Travel: Quarterly visits to clinic(s) if location permits
Essential Functions
- Daily monitoring and working of schedule queues to place outbound calls to schedule patient appointments and notify them of appointment information
- Receives inbound calls from patients/clinics and assists in resolution of concerns. Prepares, processes, and manages patient documentation in electronic medical record system
- Engages in pre-visit planning to surface important information to close gaps in patient care. Manages and updates patient information in electronic medical records system. Manages patient appointments and referrals throughout the system.
- Works closely and collaboratively with clinic teams. Leads and participates in Provider huddles to disseminate patient level data and receive instruction for next steps to improve patient outcomes
- Supports Providers and Care Managers in working at the top of their license.
- Acts as a liaison between the patient and the clinics by providing high levels of customer service and resolving outstanding issues/concerns. Supports patients to access of care and instruction about their condition(s). Supports patients through transitions of care and facilitate handoffs between care teams
- Establishes and maintains expertise in community resources and connect patients to these resources in order to help them overcome socioeconomic barriers.
- Assists caregivers and patients with escalated inquiries via telephone, email, and other technology-enabled avenues in a courteous manner. Accurately and efficiently processes transactions, answer questions, and resolve concerns for assigned specialty area and other specialty areas as assigned.
- Demonstrates knowledge of HIPAA regulations and maintain the confidentiality of patient information to be compliant with internal policies and procedures. Provides feedback to Knowledge Repository Content Owner (KRCO) to ensure appropriate direction is provided to caregivers.
- Works with other Care Coordinators, the Care Coordination Supervisor, and the Care Coordination Manager to develop standard work and best practices

Minimum Qualifications
- Experience in a customer service role requiring use of enterprise software systems.
- Demonstrated proficiency in computer software including word processing, spreadsheets, presentations, and calendaring.

- Experience in a role requiring effective verbal, written, interpersonal communication, and collaboration skills.

The Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing, and evaluating the care of patients. The staff RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members.
Shift Details: (Night Shift / 7p-7a position)
Essential Functions
- Assess: Performs appropriate assessment of physical, social, and psychological status (including cognitive, communicative, and developmental skills as appropriate). Seeks and evaluates information acquired from other members of interdisciplinary team: patient, family, physician, non-nursing disciplines, support staff, others.
- Plan: Uses assessment information and critical thinking skills to collaboratively develop individualized plan of care. Actively seeks patient, family, team, and physician involvement to develop plan of care including needed education. Collaboratively plans and prepares patients for further support needs once the current episode of care outcomes are met (i.e., teaching, referrals, and follow-up).
- Implement: Directs the interdisciplinary care team via delegation, coordination, and collaboration as appropriate. Provides or delegates care consistent with plan of care, guidelines of care and professional licensure provisions. Demonstrates the ability to set priorities and to coordinate and organize patient care delivery through effectively managing time, supplies, and resources.
- Evaluate: Evaluates patient response to interventions through review of achievement of goals, clinical outcomes, patient, and family satisfaction. Revises plan of care as indicated and reassesses changes as appropriate.
- Professionalism: Promotes nursing profession and participate in development of others. Minimum Qualifications
- Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
- RNs with less than 12 months of experience must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of their hire date.
- Basic Life Support Certification (BLS) for healthcare providers.
Preferred Qualifications
- Bachelor's degree in nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.

Physical Requirements
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
- May be expected to stand in a stationary position for an extended period of time.
- For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

As a Respiratory Therapist RRT - Homecare, you will provide advanced respiratory care services across a variety of settings, including Homecare facilities, hospital discharges and setups, physician-directed patient care, and in patients' homes. The Registered Respiratory Therapist (RRT) is responsible for assessing, treating, monitoring, and educating patients with cardiopulmonary conditions while serving as a clinical resource for respiratory caregivers and interdisciplinary teams. This position combines independent clinical decision-making, patient advocacy, equipment management, and exceptional customer service to ensure safe, effective respiratory care.

Essential Functions
Clinical Care & Patient Management
- Perform respiratory care assessments, treatments, and interventions in accordance with physician orders, departmental protocols, and regulatory standards.
- Evaluate respiratory status through physical assessment, review of medical history, diagnostic data, and patient response to therapy.
- Develop, implement, and adjust patient care plans based on clinical outcomes and changing patient needs.
- Independently perform advanced respiratory procedures, assessments, and interventions within scope of practice.
- Monitor patient progress and communicate findings to physicians, nursing staff, patients, families, and care team members.
- Participate in clinical initiatives, quality improvement activities, and specialty care programs.
Homecare & Equipment Management
- Conduct respiratory equipment setup, troubleshooting, maintenance, and education in hospitals, Homecare locations, and patient homes.
- Ensure safe and effective use of oxygen therapy, PAP therapy, ventilators, aerosol medications, and other respiratory equipment.
- Collaborate with physician offices to coordinate orders, documentation, authorizations, and continuity of care.
- Recommend appropriate respiratory technologies to maximize patient safety, comfort, and clinical outcomes.
Patient & Family Education
- Develop and implement individualized education plans for patients and caregivers.
- Teach proper use, maintenance, and safety considerations for respiratory equipment.
- Provide disease-specific education to support long-term respiratory health and successful home management.
Leadership & Collaboration
- Serve as a clinical mentor and resource for Certified Respiratory Therapists (CRTs) and other caregivers.
- Support implementation of department protocols, procedures, and evidence-based practice initiatives.
- Assist with special projects, program development, and process improvement activities.
- Handle operational concerns and escalate issues appropriately to leadership.
Minimum Qualifications
- Graduate of a NBRC approved Respiratory Therapist program. Education must be obtained from an accredited institution. Degree will be verified.
- Current State License as a Respiratory Care Practitioner.
- Registered by the National Board for Respiratory Care (RRT).
- Basic Life Support (BLS) for healthcare providers.
- Specific certifications as required (i.e. PALS, ACLS, NRP).
- Experience working in a customer service or home healthcare requiring exceptional customer service
- Current driver’s license insured and reliable vehicle, and acceptable driving record.
- Demonstrated strong organizational and prioritization skills and the ability to work independently and be self-motivated
- Strong communication skills.
- This position must be willing to help serve our community, referral sources, system, and team; all things larger than themselves. MINIMUM QUALIFICATIONS
Certificate or diploma from an approved/accredited Radiologic Technology programs. Requires national certification from the American Registry of Radiologic Technologists (ARRT), Nuclear Medicine Technology Certification Board (NMTCB), or American Registry for Diagnostic Medical Sonography (ARDMS) based on modalities in which they will be conducting work and licensure by the state regulatory agency as applicable. As of August 1, 2019, all Multi-Modality Technologist's hired on or after August 1,2019 will be required to have an advance certification by an accrediting body in a specialty if applicable. BLS certification required.
This position requires clinical knowledge typically achieved with 2 years of Radiologic Technologist experience. Must be actively working in each certified modality, in accordance with the needs of the facility. The Multi-Modality Technologist I will be required to rotate and cover department scheduling as needed and required in areas where obtained certifications apply. Multi-Modality Technologist I requires certification in two (2) or more of the following modalities: Radiologic (X-ray), Computed Tomography (CT), Bone Densitometry (DEXA), Mammography, Breast Sonography, Angiography.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, and BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Job Description:
The Clinical Assistant provides clinical patient care support in a caring, safe, and efficient manner, under the supervision and delegation of the Registered Nurse. - Proficiently operates ultrasound equipment to perform two of the following: general, vascular, and/or cardiac procedures/studies.
- Reviews patient's medical record to obtain necessary clinical information including patient history, lab results, and physician orders, obtains necessary clinical information from patients, and compares requisitions to physicians' orders to ensure exam accuracy.
- Selects proper transducer and technical factors for accurate ultrasound procedures. Performs scans of various anatomical areas. Identifies cross-sectional human anatomy, distinguishing normal from abnormal, to establish initial evaluation of sonograms. Post-processes images as directed and accurately identifies and labels images identifying patient name, medical record unit (MRU) number, and location or position of scan on all images.
- Oversees the maintenance of records in accordance with various regulatory agencies and enters patient and study information into the radiology information system, and documents billing codes of procedures performed to ensure accurate patient charges.
- Identifies equipment performance issues and ensures and/or performs equipment quality assurance tests and calibration procedures. Reports malfunctioning equipment to appropriate staff and evaluates new products and equipment.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

Essential Functions

$35.50 - $53.25Major Responsibilities:
- Engages in unit councils, professional governance, and quality initiatives to improve care processes and apply evidence-based practices.
- Utilizes the nursing process to assess, plan, diagnose, implement, and evaluate nursing care, engaging patients and families through the continuum of care.
- Monitors patient conditions, adjusts care plans, mobilizes resources, and collaborates with the care team to influence care outcomes.
- Upholds and promotes a culture of safety.
- Continuously evaluates patient, team, and unit outcomes, taking action as needed.
- May administer medications, treatments, and therapies safely and according to clinical protocols and procedures.
- Demonstrates effective communication, feedback, and conflict resolution, fostering team collaboration and appropriate delegation.
- Pursues professional development, completes required education, and maintains certifications.
- Adhere to the ANA Code of Ethics and practices ethical decision-making, respects interdisciplinary roles, and contributes to integrated, unbiased patient care.
- Appropriate delegation to other Registered Nurses, Licensed Practical Nurses, and unlicensed assistive personnel (i.e., nurse aides, credentialed medical assistants, etc.).
- Maintains accurate, timely EHR documentation.
- May be required to float to other units, departments, care areas, or facilities within the designated service area to meet patient care needs.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Minimum Job Requirements
Education
Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing services
Note: Licensed nurses practicing via telehealth/telenursing/virtual modalities are required to be licensed or hold the privilege to practice in the state(s) where the client(s) is/are located. Licensed nurses are responsible and accountable for knowing, understanding, and practicing in compliance with the laws, rules, regulations, and standards of practice of the state(s) where the client(s) is/are located.
Experience
Typically requires 1 year of experience in clinical nursing.

- Ability to work in a fast-paced, dynamic environment.
- Proficiency in operating computer functions (e.g., E-mail, electronic records, digital platforms etc.) clinical, aviation, maintenance, communications, and administrative departments.
Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.
- Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
