Under the direction of the appropriate licensed clinician, the Patient Care Technician I performs a variety of patient care activities. Receives workplace education and training to assist members of the healthcare team in the care of patients.I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning, and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures, and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

1. Minimum of a high school education or GED.
2. Basic Life Support certification within 30-days as a condition of employment.
Experience/Skills:

1. Ability to read, write and communicate in English in a clear and concise manner.

3. Basic Medical Terminology knowledge.
4. Must be able to remain calm when under stress or in emergency situations
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Position requires work indoors in a patient care environment.
Hand dexterity, hearing and vision ability to take direction from a patient or provider, adjust equipment, observe and describe patient observations. Must be able to lift, up to 50 pounds.
Work is of heavy demand, standing and walking most of the time with the possibility of pushing heavy equipment and bending frequently.
For TA Purposes:
PCA II -
Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team. In order to become a PCT II a caregiver must meet the full competencies outlined by the Nursing Professional Practice.
As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.

I. Major Responsibilities:
1. Provides clear, timely communication to the RN regarding observations and changes seen in patients. Examples, but not limited to changes in skin condition, vital signs, and/or mental status.
2. Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
3. Participates in the development and maintenance of a safe environment. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
4. Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
5. Assists in moving, lifting, positioning and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.
6. Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.
7. Takes and records vital signs (temperature, pulse, respirations, blood pressure and pulse oxymetry). Measures and records patients’ height and weight.
8. Performs EKG’s, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels and prepares specimens for transport to lab.
9. Phlebotomy in certain locations.
10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.
17. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
18. Has the ability to precept new Patient Care Technicians.

Performs laboratory testing of biological specimens in an acute setting in any/all specialty areas using manual or automated techniques. Uses independent judgment and cognitive skills to report timely and accurate laboratory results.
Major Responsibilities:
- Performs, evaluates, and monitors laboratory tests as authorized within established protocols, selecting and performing additional procedures when indicated. Monitors workflow and makes necessary adjustments to complete work in an appropriate timeframe.
- Evaluates results for clinical significance, validity and appropriateness. Follows prescribed policy in the reporting and notification of test results.
- Performs and documents calibration and maintenance of instruments and equipment. Performs and documents quality control; evaluates quality control results and takes corrective action when indicated.

This includes transfusion services, attending codes, performing testing on irretrievable samples, tissue grinding and gram stains, mass casualty procedures, and mass transfusion procedures.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. - Follows radiation safety standards and conducts all imaging procedures accordingly. Properly utilizes patient radiation protection and immobilization during diagnostic procedures. Maintains proper sterile techniques for procedures, regularly practices aseptic techniques, and ensures compliance with infection control procedures. Complies with job, departmental and hospital safety standards, practices and policies.
- Reports “near misses” and inputs errors promptly into documentation system to improve processes and identify trends. Contributes to identification of corrective actions and improvement activities and contributes to meeting departmental and hospital targets for quality and safety.
- Ensures all imaging equipment has had QC performed as required and is in safe working order prior to use. Reports any malfunctions to management and regularly practices preventive maintenance procedures; calls for service if needed.

As a Polysomnographic Technologist, you will work with the clinical director to provide comprehensive evaluation and treatment of sleep disorders. This may include polysomnography, diagnostic and therapeutic services or patient care and education.

2. Employs critical thinking skills and sound judgment in all aspects of care. Makes modifications in patient care based on evaluation and continuing assessments.
3. Performs all interventions/procedures within job scope and specific field of specialty in accordance with standards of care, policies & procedures, regulatory guidelines, and role specific competencies specifically: Routine, non-routine, and complex recordings such as CPAP, Bi-level, split night studies, seizure studies, REM behavior studies, MSLT testing, scoring all records, routine PSG recordings & Oxygen Titrations.
4. Follows proper chain of command when issues/problems arise and demonstrates appropriate follow-through. Always displays professionalism and maintains patient confidentiality.
5. Collaborates with all members of the health care team to promote a seamless patient care continuum.
6. Initiates emergency care as necessary and maintains BCLS. Stays current in field of specialty by actively participating in continuing education and maintaining certifications and annual competencies. Acquires 10 CEU’s per year or 50 CEU’s over a period of 5 years. Actively participates in orienting new staff and contributes to the growth of others.

Processes specimens for testing, including aliquoting, pre-analytical steps, and loading instruments, while maintaining proper specimen identification. Responds to additional specimen procurement requests via pager or other means.
- Orders and verifies test requests. Documents appropriate information for quality assurance/improvement according to established protocol. Runs quality control checks. Takes appropriate action to notify providers of stat, critical and other abnormal results as defined by policy and procedure.
- Performs laboratory result entry in the computer system.
- Performs internal and external proficiency testing in the same manner as patient samples. Reviews various pending and management reports. Generates and distributes reports.
- Instructs employees and students in phlebotomy, data entry and specimen processing functions.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. At Houston Methodist, the Registered Nurse (RN) II position is a licensed staff nurse, an experienced clinician, functions at the Competent to Proficient stage of Benner's model of clinical practice. The RN II position provides professional nursing care to a diverse patient population, conducts nursing assessments, assists with exams and treatment, patient education, and maintenance of medical records. The RN II position provides direct patient care effectively and efficiently which may include patients with varied and complex needs. This position communicates and collaborates with the physicians and interprofessional health care team to facilitate, coordinate and maintain compassionate, efficient, quality care and achievement of desired treatment outcomes.
