The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient’s assessment, plan of care and care plan, as may be directed by supervisors.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. High school diploma or equivalent
2. Current Certified Nursing Assistant Certification issued by state where services are provided or due within three (3) months of hire.
WV: Nurse's Aide Certification through the West Virginia Department of Health and Human Resources
3. State criminal background check and Federal (if applicable), as required for regulated areas
4. Obtain certification in Basic Life Support within 30 days of hire date.
PREFERRED QUALIFICATIONS:
EXPERIENCE:
1. One (1) year nursing assistant experience in long term care.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an exhaustive list of all responsibilities and duties. Other duties may be assigned.
1. Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner per Policy/Procedures and assures they are correct.
2. Measure and record temperatures, pulse, and respirations per Policy/Procedures.
3. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, communication, safety, education level and related criteria for the adult geriatric patient per the established age-specific standard.
4. Demonstrates the ability to use the following equipment according to manual: vital signs instrumentation, k-pad, scales, oxygen tanks, drainage systems/suction, SCDs, CPM machine, and Bladder Scanner.
5. Assist patients with daily hygiene (dental care, mouth care, bath, hair care, backrubs, nail care, and shaving, etc.
6. Assist patients with nail care, which includes cleaning and clipping, except diabetic patients per policy/procedure.
7. Turn and position bedfast patient every 2 hours and as needed. Assist patients with bowel and/or bladder functions. Assist patients in Bladder maintenance and bladder or bowel retraining programs.
8. Answer call bells promptly per policy /Procedure.
9. Assist with the care of the dying patient. Provide postmortem care per Policy/Procedure.
10. Provide perineal and indwelling catheter care on your shift and as necessary per Policy/Procedure.
11. Assist patients with (ADL) activities of daily living skills with and without adaptive equipment as per instructions of therapist or nurse.
12. Serve food trays per standard. Assist with feeding as indicated. Record food and fluid intake. Assist patients with identifying food arrangements with sight problem of foods or his/her tray.
13. Serve between meal and bedtime snacks.
14. Assist with lifting, turning, moving, positioning, and transporting patients into and out of bed, chairs, bath, wheelchairs, etc. utilizing proper body mechanics.
15. Report all allegations of patient abuse and/or misappropriation of patient property as per federal requirements and per policy/procedure.
16. Report all accidents and incidents you observe on the shift that they occur per Policy/Procedure.
17. Assists patient with specialized feeding techniques as per recommendations of the speech therapist in accordance with policy and procedure.
18. Assist with ambulation, gait training and CPM administration as instructed by the physical therapist.
19. Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist.
20. Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients who requires more than one person assistance.

1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shift.
2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
3. Hearing within normal range is required. (i.e.: to assess breath sounds, bowel sounds, apical pulse, monitors, etc.)
4. Visual acuity must be within normal range.
5. Must be able to perform heavy work: Exerting in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects

11. Maintains a neat and orderly nursing unit, hallway and utility room.

This job performs various laboratory functions which support the department which may include phlebotomy; specimen receiving and processing into the LIS; centrifugation; aliquoting, labeling and distribution of specimens to the appropriate test sites. Major Responsibilities:
- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
- Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.
- Assists the physician/provider with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.
- Administers medications, under physician/provider orders, including: immunizations, antibiotics, pain medications, vitamins, topical agents, and insulin. With limitations, initiates, maintains and discontinues IV therapy; administering selected fluids with pre-mixed medications.
- Provides basic patient education on the performance of tasks, skills, treatment and self-care.
- Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services.
- Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.

The Radiology Technologist assistant provides support to imaging technologists in performing various diagnostic imaging procedures. - Obtains authorization for surgeries/hospital procedures by researching coverage and obtain prior auth for them. Verifies eligibility and benefits. Coordinates referral for patients that will go out of network. Point of contact for surgery authorization questions. Obtain proper CPT code from provider for authorization.
- Proposes iCentra requirements to create a pre-surgical FIN# and to create required surgical scheduling card for the Hospital scheduler to pull the case information, where applicable.
- Coordinate with the provider to ensures all necessary surgical/procedure equipment is requested for any special item that needs to be pulled from Hospital supplies or special equipment that needs to be brought in by surgical supply Reps. - Meets with surgery/procedure patients to explain the process. This includes pre-op labs, tests, or any pre-requisite that needs prior attention. Coordinates with Providers and Medical Staff any red flag pre-operative health concerns for patient safety.

We are seeking a skilled and motivated Podiatrist to support a multidisciplinary limb preservation program focused on coordinated wound care, limb salvage, and prevention of avoidable amputations. In addition, the candidate can anticipate to:
- Serve as an active member of the Enterprise Limb Preservation Steering Committee by reporting program outcomes, advancing best practices and systemwide standards, and escalating issues that require enterprise-level action to improve limb salvage and reduce preventable amputations.
- In collaboration with operational and nursing leadership; support complex case review, promote adherence to evidence-based clinical pathways, address barriers to care, and strengthen transitions from hospitalization to ambulatory follow-up.

This job ensures availability of cleaned, prepared, sterilized equipment and supplies for surgical procedures; functions as a resource for clinical knowledge; and performs other duties as assigned by the sterile processing leadership. Receives, cleans, distributes, and stores medical instruments and equipment. Supports direct patient care in clinical and satellite operations as well as operative suites.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education
Required - High school diploma or equivalent.
Work Experience
Preferred - 2 years of sterile processing experience or graduation from accepted course in sterile processing or surgical technology.
Certifications
Preferred - Certification through one of the following:
Certified Registered Central Service Technician (CRCST)International Association Healthcare Central Service Materiel Management (IAHCSMM).
Certified Sterile Processing Department Technican (CSPDT), Certification Board for Sterile Processing and Distribution(CBSPD).
Certified Flexible Endoscope Reprocessor (CFER), CBSPD.
Knowledge Skills and Abilities (KSAs)
Clinical knowledge.
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and ability to communicate pertinent information to patients or appropriate staff in a timely manner.
Good organizational and time management skills.
Conflict resolution skills and ability to demonstrate good judgement in performing duties.
Ability to adapt to change in a positive, constructive manner.
Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability) and travel throughout and between facilities.
Job Duties
Performs general and specialized instrument care duties.
Provides supplies as needed for surgical procedures.
Disassembles and cleans critical reusable medical equipment in accordance with Manufacturer’s Instructions For Use (MIFU).
Inspects, packages, and reassembles surgical instruments and trays according to established priority.
Practices High Level Disinfection, Pre-Vaccuum Sterilization, Gravity Displacement Sterilization, and Hydrogen Peroxide Plasma Sterilization techniques in accordance with MIFUs.
Practices record keeping and document control as defined in Standard Operating Procedures, policies, and recommendations from regulatory bodies.
Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs other related duties as required.
