Job Description:
The Social Work Care Manager I utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties.

Essential Functions
- Assessment & Screening: Evaluates patients for transition planning, mental health, substance use, and goals of care.
- Care Coordination: Develops and monitors care plans, addressing social determinants of health and community resources.
- Behavioral Health Support: Uses motivational interviewing and therapeutic techniques to promote mental health care including women’s services, behavioral change, trauma informed care, and substance use disorders.
- Therapeutic Intervention: Provides brief individual, group, and family therapy, plus psychosocial assessments.
- Diagnosis & Referrals: Identifies mental, emotional, and behavioral disorders and connect patients to services.
- Education & Advocacy: Trains staff, educates patients, and advocates for rights and care access through facilitating safe transitions of care to the community.
- Team Collaboration: Works with healthcare teams, insurers, and community providers for quality care.
- Quality & Compliance: Leads improvement initiatives, tracks key metrics, and ensures policy adherence.
- Advanced Care Planning: Facilitates clinical goals of care discussions with patients, families, and teams.
Minimum Qualifications
- Master of Social Work (MSW) from an accredited institution (degree verification required).
- Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours. (CSW/LCSW)
- Basic computer proficiency, including familiarity with word processing and spreadsheet software.
- Strong written and verbal communication skills.
- Demonstrated ability to apply critical thinking skills.
- Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation.
Preferred Qualifications
- Case Management Certification.
- Experience in clinical care management, social work, or working with third-party payers.
- Demonstrated understanding of care management principles and practices.
- Demonstrated understanding of health insurance products and related processes.
- Demonstrated understanding of coding, episode of care, and length of stay guidelines.
- Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment.
Physical Requirements
- Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
- Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
- May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts.
- For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

Minimum Job Requirements
Education
Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing services

Experience
Typically requires 1 year of experience in clinical nursing.

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.
- Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

$20.80 - $31.20Major Responsibilities:
- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and
- Nurse Aide in good standing as indicated with the State Registry in which the team member practices.

Job Description:
A nursing student who has completed a minimum of one medical/surgical rotation in an accredited nursing program and is pending nursing licensure. All nursing students hired into this position are expected to obtain nursing licensure as soon as possible and transition into a new graduate nurse resident position, as available, for the next available cohort after licensure. Physical Requirements and Working Conditions:
- Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways. Must be able to provide/arrange own transportation to and from patients homes and branch offices. Ability to work rotating weekends and holidays. Requires adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned. Ability to attend mandatory informational/education/branch meetings. Work environment includes patient or family homes, or assisted living facilities. Requires full range of body motion including: a. Bending, sitting and standing b. Experience

Physical Requirements and Working Conditions
Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
Maneuver foot pedals on carts or machines.
Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
Must be able to respond quickly to changes in patient and/or unit conditions.
Physical Abilities Testing may be required.
Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

Job Description:
The Social Work Care Manager I utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties.
Shift coverage weekdays, weekends, and holidays for inpatient and ED units.
ED - 10 AM - 6:30 PM
Inpatient - 8 AM - 4:30 PM
Essential Functions
- Assessment & Screening: Evaluates patients for transition planning, mental health, substance use, and goals of care.
- Care Coordination: Develops and monitors care plans, addressing social determinants of health and community resources.
- Behavioral Health Support: Uses motivational interviewing and therapeutic techniques to promote mental health care including women’s services, behavioral change, trauma informed care, and substance use disorders.
- Therapeutic Intervention: Provides brief individual, group, and family therapy, plus psychosocial assessments.
- Diagnosis & Referrals: Identifies mental, emotional, and behavioral disorders and connect patients to services.
- Education & Advocacy: Trains staff, educates patients, and advocates for rights and care access through facilitating safe transitions of care to the community.
- Team Collaboration: Works with healthcare teams, insurers, and community providers for quality care.
- Quality & Compliance: Leads improvement initiatives, tracks key metrics, and ensures policy adherence.

Minimum Qualifications
- Master of Social Work (MSW) from an accredited institution (degree verification required).
- Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours.

- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
- May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts.
- For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
