Job Description:
A Medical assistant has accountability for providing care to patients in an ambulatory clinic setting. This will involve caring for patients with acute and chronic needs. A Medical Assistant will be responsible for communicating clearly with patients seeking care through different modalities including telephone and in-person interactions.

As a Medical Assistant at Intermountain Health, you will play a vital role in supporting our clinical team, ensuring our patients receive the best care by:
- Providing Patient Care: Under the direction of the Clinician, providing care to patients by assisting the provider to gather information that is relative to the patient’s visit and condition.
- Effective Communication: Communicating clearly with patients seeking care through different modalities including telephone and in-person interactions.
- Team Collaboration: Working with cross functional teams to collaborate on patient care. Attention to detail and follow-through are key components in helping to provide the care that is needed.
Minimum Qualifications
- Successful completion of a Medical Assistant Program or one year of Medical Assistant Experience or a current RN/LPN license
- Current Basic Life Support Certification (BLS) for healthcare providers. Must be obtained prior to start date
- Demonstrated basic computer skills involving word processing and data entry.
- Professional manner and strong interpersonal and communication skills.
- Ability to work collaboratively with patients and fellow caregivers to deliver the highest level of customer/patient satisfaction.
- Ability to protect privacy, confidentiality, and Protected Health Information (PHI) of patients, members, and caregivers.
Preferred Qualifications
- Experience in an outpatient clinic or other healthcare setting.
- Computer literacy in using electronic medical records (EMR) systems and other relevant software.
- Current national MA certification from one of the following national MA certifications or registrations: Registered Medical Assistant (RMA) of AMT, National Certified Medical Assistant (NCMA), Certified Clinical Medical Assistant (CCMA) or Certified Medical Assistant (CMA) of the AAMA
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items.
Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
May be expected to stand in a stationary position for an extended period of time.
For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Job Description:
Performs a variety of food and guest service duties under general supervision. This position includes the following roles, which will vary by facility need: patient meal tray assembly and delivery, cashier, food assembly, food serving, and cleaning.

Job Description:
The Clinical Program Coordinator assesses, plans, implements, and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.
2 part time positions, 20 hours a week, day shift (Monday - Friday 0730-17:00)
No nights/weekends/holidays. One day per month we work 7:30PM-9:30PM to prep for conference with the provider on Teams.
Essential Functions
- Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers
- Provides patient and community education
- Ensures optimal patient care using evidence-based, clinical best practice models, appropriate staffing and technologies and continuous process improvement measures
- Involved in patient engagement efforts and monitors patient perception of the quality of care delivered
- Coordinates care across the continuum, including planning and implementing clinical protocols and practice management guidelines, monitoring care, and serving as an expert resource for clinical practice.
- Monitors clinical outcomes and system issues related to quality of care
- Stabilizes the complex network of many disciplines that work in concert to provide high quality care, serving as an internal resource for staff in all departments

This is an on-the-job training role designed for Imaging team members who are pursuing a specialty within one of five modalities: CT, MR, US, NM, or Interventional Radiology. The position is responsible for performing high quality procedures using independent decision making and problem-solving skills to obtain the best possible images. The incumbent will become proficient in all aspects of patient care and the use of ionizing radiation, if applicable, for diagnostic purposes.

Job Description:
The Nurse Care Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs. This position educates patients and families to help them manage their health care needs. The incumbent facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.
Scope
Nurse Care Manager-MG works in the ambulatory setting. Patient interactions may be in person, by telephone, or other electronic means.
Job Essentials
General Case Management
Responds to physician referrals and identifies patients who meet established criteria for care management (e.g. HgA1c > 8, elevated LDL and/or B/P, Mental Health Integration referral, complex resource needs). Works with discharge planners on inpatient units to get the appropriate appointments.
Patient Evaluation
Assesses family, social, cultural characteristics.
Understands communication needs (e.g., vision, hearing).
Assesses behavioral and family risk factors.
Assesses barriers.
Screens for chronic disease (e.g. depression).
Reviews patient understanding of medication treatment.
Coordination of Care
Coordinates with care managers in other settings as appropriate.
Provides information on enabling services (e.g., transportation).
Maintains list of key community services agencies with contact information.
Provides information about recommended or available services and contacts.
Personalized Primary Care.
Manage Populations, Disease Registries and Preventive Care
Establishes process to monitor patient adherence to medical plan of care.
Focuses on prevention measures consistent with established guidelines and care process models
Reviews and manages quality reports related to chronic disease and prevention
Supports clinicians in achieving quality incentives.
Team Based Care
Works collaboratively with referring physician and other members of care team
Personalized Primary Care:
Completes pre-visit planning (review chart before visit, notify patient of tests needed before the visit)
Facilitates advanced care planning (Advanced Directives). Establishes a process for reminder letters and phone calls.
Supports clinicians and team to achieve personalized primary care goals.
Facilitates transitions of care (e.g., unscheduled hospital admissions, emergency department visits, skilled nursing home).
Tracks status of critical adult and pediatric referrals.
Follows up to obtain report back from referral clinician.
In collaboration with clinician, establishes written care plan for patients transitioning from pediatrics to adult.
Provides information on health insurance resources.
Supervises and supports Health Advocates.
Attends clinic team meetings.
Assists with scheduling intake.
Minimum Qualifications
Current RN license for state in which the nurse practices.

BLS certification for healthcare providers.

RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.

Three years of clinical nursing experience.
Preferred Qualifications
Bachelor's degree in Nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.
Experience in case management, utilization review, or discharge planning.
Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

- Takes steps to correct errors in co-worker's delivery of patient care through direct action and communication to ensure patient safety and well-being.
- Supervision of others (i.e. police officers, fire personnel, bystanders, other EMT's, observers, physicians, nurses, and co-workers) at the scene or in the ambulance reflects sound decision making process.
- Consistently provides and documents daily duties, patient care provided and hospital charges.

Job Duties
Provides direct patient care, within the scope of their license, with supervision from the physician. This may include the following: medical treatment of wounds at the basic/intermediate/advanced level, would care, ability to administer medications by pre-established protocol which includes vasoactive medications, draw bloodwork, place splints/boots/etc., Foley insertion, use of glucometer, I-STAT, Kurin, vital signs (rectal temps/immunocompromised), and IV fluid administration. The position may administer preset protocols and standing orders, as ordered by a physician.

At Houston Methodist, the Registered Nurse (RN) II position is a licensed staff nurse, an experienced clinician, functions at the Competent to Proficient stage of Benner's model of clinical practice. The RN II position provides professional nursing care to a diverse patient population, conducts nursing assessments, assists with exams and treatment, patient education, and maintenance of medical records. The RN II position provides direct patient care effectively and efficiently which may include patients with varied and complex needs. - Provides patient care as directed, within the nursing assistant scope of practice, in meeting the multidisciplinary care plan established for each patient.
- Identifies, observes, and documents the physical needs of the patient according to established standards. Reports ant changes in patient condition to licensed personnel in a timely manner.
- Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
- Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
- Provides one to one patient safety watch as assigned following established policies and procedures.
- Accurately processes specimens and provides timely transport of the specimen to the laboratory.
- Performs point of care patient testing and point of care instrument maintenance and quality control.
- Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
- Serves as a preceptor and mentor for new staff and students in a positive manner.
- Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard.
- Releases and confirms ordered tests to appropriate departments.
- Coordinate's transportation of patients to multiple tests.
- Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consults, etc.
- Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies.
- Communicates with food services to ensure appropriate patient diets.

Job Duties:
- Obtains and documents, using the rooming tool in the electronic medical record, the patient’s reason for visit, weight, vital signs, visit screening information, medication review, and pertinent information for provider.
- Sets up examination and treatment rooms between cases.
- Assists patient as needed to prepare for visit with provider, takes appropriate measures to assure patient’s safety and privacy.
- Provides direct patient care as directed by provider based on competency and needs of clinic, such as performs EKGs, wound dressings, and functions as support to the provider during minor surgical procedures and ancillary examinations.
- Prepares and administers medications according to policy and procedure after validation of knowledge and demonstration of competency. May not administer intravenous medications.
