Our structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies – a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in-office team of QI specialists, telehealth resources, clinical manager and in office triage support. As a Home Health Registered Nurse, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community. You will enjoy an autonomous practice focusing on medical intervention within the patient’s actual home environment. The Home Health space allows for the time to create one on one relationships with your patients and their caregivers and have some flexibility over your schedule that is not possible in the hospital setting. You will enjoy working in a sub-assigned geographic territory with a smaller travel distance than other valley agencies! You will enjoy an autonomous but supportive work environment. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).

POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral or BUMG, BUMCT in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. An Arizona Criminal History Affidavit must be signed upon hire. Employees working at BUMCS in a Behavioral Health clinical setting must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working in any Banner Staffing (BSS) Behavioral Health role in Arizona must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Bachelor's degree preferred. Professional certification preferred.
Additional related education and/or experience preferred.
Estimated Pay Range:
$33.11 - $55.18 / hour

This range is based on possible base salaries and does not include the value of our total rewards package. This position selects and places surgical instruments, supplies and equipment during surgery. Scrubs and provides instruments, sutures, and other sterile supplies during surgical procedure. Participants in the training and orientation of surgical technology staff and students, as assigned. Is proficient to scrub on a minimum of two surgical specialties.
The Cardiovascular Operating Room Surgical Technologist is responsible for assisting the physician with advanced tasks during surgical procedures which include cardiopulmonary bypass/heart-lung machine use, cardiac assistive devices, and vascular access and interventions. The CVOR ST must be able to work under high pressure in a fast paced environment.
CORE FUNCTIONS
1. Prepares for surgical procedures. Pulls appropriate instruments and supplies and ensures good working order of instruments. Reviews case carts for correct instruments and supplies. Troubleshoots malfunctioning equipment and makes minor adjustments or calls biomedical equipment staff. Checks schedule and plans ahead for upcoming surgical case needs.
2. Anticipates the surgeon's needs. Passes instruments and retracts tissue. Maintains competency to scrub in a minimum of two surgical specialties.
3. Demonstrates the principles of aseptic technique and universal precautions throughout the procedure. Disposes of sharps and trash and places used supplies into case cart for processing. Participates in counting sharps, instruments and sponges. Prepares instruments for cleaning following each case.
4. Ensures that all surgeon preference cards and pick lists are accurate.
5. Assists with OR turnover between cases. Participates in stocking, replenishing supplies and cleaning operating rooms.
6. Participates in or leads the training and orientation of surgical technology staff and students, as assigned.
7. May provide business support to the department including administrative tasks.
8. This position follows safety procedures in regard to medication labeling, appropriate handling of specimens, appropriate handling of sharps, time-out procedure, radiation and laser safety, infection control, assembling and disassembling instruments. Participates in emergency call rotation. Is proficient to scrub on at least two surgical specialties. Internal customers are patients, surgeons, and nursing staff. External customers may include practicing physicians, patient families and vendors or sales reps.
MINIMUM QUALIFICATIONS
High school diploma/GED and current basic life support certification.
Requires certification as a surgical technologist and one to two years of experience in a surgical technologist role OR three to four years in a surgical technologist role. Where applicable must be registered with State Regulatory Agency.
This position must be able to scrub proficiently on a minimum of two surgical specialties. The position requires manual dexterity and the ability to maintain professional decorum in highly stressful emergent situations. The position requires the ability to be conscientious, organized, and orderly and the ability to respond quickly to the needs of the surgeon and patient. Must stay abreast of new developments in the field.

POSITION SUMMARY

1. Processes and facilitates the timely discharge/transfer of clients from hospital care to identified post-acute setting. Notifies care coordination team member(s) if patient or caregiver demonstrate or verbalize any inability/concern to be able to manage their post-acute plan or responsibilities.

Understand sterile procedure and assists with setting up sterile trays, where applicable. Adheres to radiation safety policies. Escorts patients back to the patient exam rooms, takes vitals and patient history; may assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by the assigned facility (completion of Banner’s peripheral IV insertion course and validation of skill must be completed prior to any peripheral IV insertion and discontinuation performed. Must be able to organize, lead and direct work teams and serve in a leadership role. Must demonstrate effective communication and teamwork skills. This position requires making decisions related to patient care coordination throughout various facilities and with various specialty practices. Decision making required for treatment and standardization of care of patients. This position requires complex problem solving related to plan of care for evaluation, treatment, and discharge planning for patients.
MINIMUM QUALIFICATIONS
Must possess knowledge as normally obtained through the completion of a Bachelor's degree in nursing or related field.
Requires a current RN license in the state of practice. BLS certification required.
Requires a proficiency level typically achieved with 5 years clinical experience. Requires excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area. Requires effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Requires the ability to teach both clinical and non-clinical personnel regarding care and diagnostics services. Also requires a good understanding of process improvement.
PREFERRED QUALIFICATIONS
Current certification in specialty area preferred. Prior Case Management experience preferred.
Additional related education and/or experience preferred.

As a Genetic Counselor in Oncology at Banner Health, you will play a meaningful role in guiding patients and their families through the complex and often emotional journey of understanding their inherited cancer risk. In this position, you will obtain detailed personal and family medical histories, construct and analyze pedigrees, and determine the appropriateness of genetic testing to develop individualized care plans and treatment options. You will interpret complex genetic principles and laboratory results while providing compassionate counseling and psychosocial support to help patients and families navigate difficult decisions with clarity and confidence. Working collaboratively within a multidisciplinary oncology team, you will contribute to medical direction, support research initiatives, and provide education to healthcare professionals and the broader community. Schedule: Anticipated schedule is Monday -Friday 8:00am-4:30pm (though there is some flexibility on start time). No weekend or call rotations will be needed. The position is divided up between the BUMCP and Banner Scottsdale campuses, but the specifics of which days will be at which sites have not been determined.
POSITION SUMMARY
This position is responsible for working with patients who have a possible or established personal or family history of an inherited risk for disease. The counselor obtains personal and family medical history, develops and analyzes pedigrees, determines if genetic testing is appropriate, arranges genetic testing, analyzes and interprets test results in order to provide patient care plans and treatment options. Counselors provide counseling and psychosocial support to patients and families. Works collaboratively to provide medical direction for the program.
CORE FUNCTIONS
1. Provides counseling and genetic assessment services to patients with personal or family history or are at risk for genetic disorders. Provides education and counseling on genetic principles and tests to patients and family members.
2. Obtains and reviews patient’s personal and family medical history. Retrieves and reviews medical records. Documents family history and constructs pedigrees to assess the pattern of genetic disorders in the family and risk of disease and to determine whether genetics testing is appropriate.
3. Works in collaboration with program (s) regarding clinical care and research to provide medical direction.
4. Interprets complex genetics principles, laboratory data and test results. Provides counseling recommendations for care and psychosocial support for patients and families regarding risk of an inherited disorder. Establishes a plan of care based on genetic assessment.
5. Assists with research protocol enrollment, collection of samples, data management, and patient consent in accordance with the Institutional Review Board.
6. Provides education of genetic counseling and genetics to health care professionals, including residents, physicians, program staff, as well as organizations and individual members of the community.
7. May assist with facility program development.
8. Position works independently along with medical guidance from clinicians, working within protocols and interpreting genetic risk. Position provides care planning and counseling for patients and at-risk individuals. May work with various sub-specialties in the program.
MINIMUM QUALIFICATIONS
A Master’s Degree in Genetic Counseling, obtained through a genetic counseling program accredited by the ACGC (Accrediting Council for Genetic Counseling), is required. Must be eligible for Board Certification as a Genetic Counselor. Genetic Counselor certification must be obtained within 24 months of hire.
Position requires excellent communication, interpersonal, counseling and psychosocial assessment skills in establishing patient rapport and in responding to issues of client or family. Requires critical thinking skills to complete complex research and analysis.
PREFERRED QUALIFICATIONS
Board Certified Genetic Counselor with Master's Degree in Genetic Counseling.
Additional related education and/or experience preferred.
