Organization:
SMCS-Sutter Med Center - SacramentoPosition Overview:
Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. This position works in collaboration with the Physician, Utilization Manager, Medical Social Worker and bedside RN to assure the timely progression and transition of patients to the appropriate level of care to prevent unnecessary admissions or readmissions. The Care Management process encompasses communication and facilitates care across the continuum through effective resource coordination. The goals of this role are to include the achievement of optimal health, access to care, and appropriate utilization of resources balanced with the patients' self –determination while coordinating in a timely and integrated fashion. He/She collaborates with patients, families, physicians, the interdisciplinary team, nursing management, quality, ancillary services, third party payers and review agencies, claims and finance departments, Medical Directors, and contracted providers and community resources. If assigned to the Emergency Department, the Care Management process is to address complex clinical and social situations efficiently in order to avoid unnecessary admissions.Job Description:

EDUCATION:
Graduate of an accredited school of nursing
CERTIFICATION & LICENSURE:
RN-Registered Nurse of California Upon Hire
TYPICAL EXPERIENCE:
2 years recent relevant experience
OR graduate of an accredited Master's degree program in Nursing with a specialization in Case Management
SKILLS AND KNOWLEDGE:
Experience in acute care case management or health plan case management/utilization management.
A broad knowledge base of health care delivery and case management within a managed care environment.
Comprehensive knowledge of Utilization Review, levels of care, and observation status.
Awareness of healthcare reimbursement systems: HMO, PPO, PPS, CMS, value-based reimbursement models, and alternative payment systems
Working knowledge of laws, regulations, and professional standards affecting case management practice in an integrated delivery system: including but not limited to: CMS, Title 22, CHA Consent Manual, CDPH and TJC.
A broad knowledge base of post-acute levels of care and associated regulatory compliance requirements.
General understanding of coding and DRG assignment process preferred.
Must be able to effectively communicate with, and promote cooperation and collaboration between individuals including patients/families/caretakers, physicians, nurses and other ancillary partners.
Ability to work independently and exercise sound judgment in interactions with physicians, payers, and patients and their families.
Demonstrates commitment to service excellence in all patients, family and employee interactions and in performing all job responsibilities.
Functions in a manner to promote quality patient care and assure a positive patient experience.
Strong verbal and written communication skills and negotiation skills

Houston Methodist Willowbrook Hospital - Nights/CVICU
At Houston Methodist, the Registered Nurse (RN) III position a licensed staff nurse, who as an advanced clinician, functions at the Proficient level according to Benner's model of clinical practice. The RN III position provides professional direct patient nursing care to a diverse patient population and is considered an expert in a specialty. Besides conducting nursing assessments, assisting with exams and treatment and maintenance of medical records, this position assists in coaching and mentoring department staff in areas of clinical and professional practice. Role modeling compassionate communication with physicians and the interprofessional health care team, the RN III position assumes leadership responsibilities with high level contributions at the bedside, unit, service line, and across the institution. 2. Utilizes down time to meet departmental needs (serves in cafeteria, assists chefs, food prep cook, baker or salad worker, etc.)

When appropriate, higher-acuity cases are transferred to Good Samaritan Medical Center or Saint Joseph Hospital, where robotic surgery platforms are available. Robotics experience is not required for this role.
Call Responsibilities:
This position includes a 1:4 call rotation with a minimum of eight call days per month, including one full weekend. Weekend coverage runs from Friday at 7:00 PM to Monday at 7:00 AM. Additional vacation coverage may be required, with locum support available. After-hours clinic call volume is minimal.
While on call, surgeons are expected to:
- Handle trauma activations within the scope of a Level III Trauma Center
- Provide surgical coverage for the hospital and emergency department
- Independently address urgent surgical needs
- Support trauma handoffs and inpatient care
To ensure a timely response, surgeons must live within approximately 20 minutes of Platte Valley Hospital.
What you’ll bring:

Act as a liaison to food service for the patient, family, and staff. Tasks as liaison: explaining the meal selection process; assist menu selection of patients according to prescribed diet order using HealthTouch. To assure patients are fed within 45 minutes of ordering the meal.Major Responsibilities:
1. Communicates on the telephone in an efficient, pleasant, and respectful manner, even during periods of high call volume, to assist patients and guests with their meal service selections. At peak time, a call center operator answers the phone and immediately enters a complete order into the HealthTouch system. A call center operator’s average call time will range between 1 ½ - 2 minutes.
2. Reviews therapeutic diet orders and modify the patient’s order to meet the diet order parameters while attempting to honor the patient’s request and be aesthetically pleasing. This includes nourishments, supplements, and tube feeding.
3. Speaks with the patient, family member(s) or nurse to explain needed modifications regarding meal orders. To communicate changes and if possible, find a satisfactory substitution. To ensure a nutritionally balanced meal that meets the dietary restrictions of the patient’s diet order.
4. Communicate on the phone to patients, staff, and guests according to department scripting policy and procedures.
5. Communicates patient diet order issues to the Supervisor or clinical dietitian when policy mandates.

EDUCATION

ESSENTIAL FUNCTIONS

- Coordinates with Treasury Management and Financial Accounting to ensure that all debits and credits are accurately reflected in the daily activity.
- Coordinates with Single Billing Office (SBO) and/or AR representatives to resolve cash application discrepancies, as needed.
- Assists with knowledge sharing, payor and department training, and provides support to other team members as advised by the manager and/or supervisor.

- Works correspondence received in lockbox daily.
- Ensures protection of private health and personal information. Adheres to all Health Insurance Portability and Accountability Act (HIPAA) and Payment Card Industry (PCI) compliance regulations.
- Monitors the appropriate software tools and reports to ensure that assigned electronic remits are accurately posted.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Reviews remittance advices or explanation of benefits received in lockbox correspondence to apply correct contractual adjustment and enters appropriate account status.
- Researches transactions appropriately to ensure that payments are correctly applied. Resolves any pending electronic transactions upon management’s request.
- Identifies and resolves unmatched cash transactions in a timely manner. Thoroughly utilizes all available resources to research any potential refund to ensure accuracy and prepares refund requests when appropriate; consistently follows the Levels of Authority (LOA). (EF)
- Completes reconciliation of unidentifiable/unmatched cash receipts. Processes insurance refunds as applicable.
FINANCE ESSENTIAL FUNCTIONS
- Reviews and enters all transactions from work driver rejections/exceptions.
- Resolves assigned reconciling items to ensure the proper statement of cash in financial reports.
- Plays a key role in month end by clearing any assigned tasks for month end and assists with additional research and/or posting as needed to close month properly.
