*Job Description:*

Home care Nursing Supervisor - Branch Specific - Case Management - responsible to assist in the coordination of staff and to ensure quality and safe delivery of home health care services.

*Duties include but not limited to :*

Patient assessment

Care plan development

MD orders

Compliance, evaluation and supervision of non-professional staff in the delivery of in-home patient care.

*REQUIREMENTS:*

\*Must have valid NYS RN License

\* 1 year home care experience required, 2 years preferred.

\* Bilingual in English/Spanish (a plus, but not necessary).

\* Local daily Travel is required and reimbursed
