Position Summary
The Patient Services Representative (PSR) is an excellent opportunity at MultiCare to support our Lab Services. In this role, you will be responsible for patient registration and order entry, phlebotomy, and specimen processing. You may also be responsible for specimen pick-up in offices, report sorting and delivery, supply delivery, and other tasks related to the operation of the specific location.
Requirements
- High school graduate or equivalent preferred
- Current Medical Assistant-Certified, Medical Assistant-Phlebotomy Certification, or Medical Assistant-Phlebotomist eligible - must complete the training and submit application within 90 days of hire
- Completion of phlebotomy course or training preferred
- Phlebotomy experience preferred
- Previous experience dealing with the public preferred

Pay and Benefit Expectations

Banner Estrella is a full-service medical campus. The 353 all-private bed hospital offers an array of medical services, including heart care, emergency services, general surgery, weight loss surgery, medical imaging, orthopedic surgery and a comprehensive maternity services program. 3A is a 24 bed Cardiac Progressive Care Unit (PCU) that serves patients with medically complex cardiac needs. This unit sees a variety of cardiac related diagnosis, cardiac drips, pre/post procedures including TAVR recovery and recovery of open hearts post ICU downgrade, may also see pulmonary, renal and vascular patients. The vast majority of the patients in the PCU require continuous cardiac monitoring. Ratio flex between 1:4-5, however most of the time ratios are 1:4. The Progressive Care Unit at Banner Estrella Medical Center is currently seeking experienced PCU Nurses, but at this time we are accepting application for those that have Telemetry, ICU, Home Health, Skilled Nursing, Medical/Surgical or any other acute or critical care background. This is a full time day shift, working three 12 hour shifts per week, 7:00am to 7:30pm, with rotating weekends/holidays. Enjoy a a flat rate $3/hour weekend shift differential.

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.
9. Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
PREFERRED QUALIFICATIONS
Bachelor’s degree preferred. Professional certification preferred.

The Behavioral Health Case Manager is responsible to provide care/service safely and efficiently for a full range of services to patients of all ages and their families. Assumes responsibility for risk and safety issues associated with the position. Takes call as required by the department expectations. Performs job specific responsibilities and demonstrates accountability for own actions and decisions.
- Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope. Brings ideas and concerns to supervisor, participates in department decision making. Maintains current licensure.
- Initiates and participates in family conferences to determine psychosocial and discharge planning needs.
- Completes psychosocial assessments of patient/family situations including social, psychological, emotional, financial and other related factors to facilitate patients return to the community.
- Utilizes social work assessment and input from other team members to formulate realistic recommendations for social work action plan while patient is hospitalized and formulate and execute an appropriate discharge plan.
- Locates community resources and connects patient with resources to assist in their living situation
- Assists in determining financial concerns including insurance and assists in application for social security benefits (such as SSD, SSI or any other applicable social security benefits) as well as application for Medicaid/Medicare if appropriate.
- Responsible for understanding various State Waivers, qualifications for each waiver and how to assist patients in determining eligibility and obtainment of waiver services.
- Connects patient with Community Mental Health or other agencies or private providers to provide:Psychiatry follow-up appointments

POSITION SUMMARY

10DE Progressive Care is a 19 bed, high acuity progressive care unit serving a diverse patient population with complex medical, surgical, cardiac, neurological, and trauma needs. - This full-time position is 5 days/week (Monday through Friday) and will require working 2 evenings per week until 7:30 p.m. (Mondays and Thursdays).

Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
May supervise physical therapy assistants following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. May supervise physical therapy extenders (rehab aides/techs) in administering treatments to patients and will teach them appropriate techniques to perform with specific patients. May supervise physical therapy students after completing one year of clinical experience.
Minimum Job Requirements
Education
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License
Licensed Physical Therapist in the State upon which you work. Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)

Position Purpose:
In accordance with the Mission and Guiding Behaviors; the Paramedic will provide patient care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. Functions within the standards, policies, procedures and guidelines of the Organization.
This Paramedic position will be working in our Emergency Department at Mount Carmel Grove City on nights three 12-hour shifts per week from 7pm - 7:30am with rotating weekends and holidays.
What You Will Do:
- Initiates respiratory interventions including oxygen therapy and assistance with preparation for intubation per physician order or protocol
- Initiates and discontinues intravenous lines and intraosseous (IO) access as requested by the physician, including use of the ultrasound machine
- Interprets cardiac rhythms and initiates appropriate treatment (CPR/defibrillation)
- Transports and monitors stable telemetry and non-telemetry patients
- Maintains professional radio communication with incoming EMS units
- Performs patient care activities as appropriately delegated by the RN in a timely manner
- Performs treatments and procedures according to approved standards, policies, procedures, and guidelines.
- Verifies and implements physician/other licensed independent practitioner’s orders.
- Administers medications, according to the organization’s standards, policies, procedures and/or guidelines.

Under general supervision, serves as expert resource for performing and teaching in CV/Cath/EP labs. Utilizes professional cardiovascular theory, technology and processes to implement direct patient care on adult patients that require diagnostic or therapeutic procedures within the cardiovascular service. Demonstrates competence in administering these procedures including, but not limited to PTCA, stent placement, IABP placement and EP procedures.
Essential Functions
- Plans, provides, and documents care of cardiovascular invasive labs patients in accordance with physician orders and established policies and procedures; uses professional clinical judgment in assisting the physician to plan the care for the patient based on the assessment of the patient's diagnosis, needs and response to care.
- Operate, monitors, records, evaluates, and maintains patient information.
- Meets challenges of case volumes and patient care issues to ensure optimal care and outcomes; works in an organized and timely fashion by prioritizing tasks to ensure efficient and efficacious patient care.
- Ensures quality and accuracy of reported laboratory data; reports variances to physician and ensures that variances have been addressed; ensures all recorded data is accurate and makes corrections as needed.
- Operates, inspects, troubleshoots and performs routine maintenance for variety of invasive lab equipment; effectively operates all devices utilized in procedures; initiates maintenance or service request for equipment that is damaged or malfunctioning.
- Performs daily room QCs and notifies appropriate people of variances; prepares procedure room and equipment for daily use and each procedure; ensures that stock on hand is sufficient to accommodate routine daily case volumes; conducts in-room inventory and supply management; is aware of and stores all patient care products according to established regulations and standards.
- Documents, retrieves and verify data necessary for patient records.
