Sentara is currently hiring a Patient Care Technician to work at Sentara Virginia Beach General Hospital on the Inpatient Rehab Unit.
Hours/Shift:
FLEXI (as needed), rotating shifts, 7am to 7pm and 7pm to 7am
Department/Position Overview:
Inpatient Rehab (IPR) is an 8-bed unit specializing in providing patients with additional rehabilitation needs for 2-3 weeks when they are not independent enough to go home but do not require a skilled nursing facility. We work closely with physical therapy, occupational therapy, & speech therapy teams while providing care to these patients. We become familiar with our patients and their routines over this longer stay. We enjoy watching our patients progress to going home and that is rewarding as a patient care technician. The patient population includes but is not limited to stroke, spinal cord injury, patients that required amputation or other orthopedic patients that require more therapy after being medically cleared from the hospital.
Patient Care Technicians are unlicensed staff members who are accountable to, and work under the direct supervision of a professional nurse to implement delegated aspects of nursing care. Provide patient care, support the admissions and discharge process, and comply with patient safety practices under the direction of the professional nurse. Observe patients' physical, mental, and emotional conditions and report any change to the nurse.
Education
High School Diploma or Equivalent
Certification/Licensure
- American Health Associate BLS Certification required within 90 days of hire.
- Minimum requirements: Must have one of the following
- Sentara Care Partner Certification Program, or
- Successful completion of the Certified Nursing Assistant (CNA), or
- National Certified Medical Assistant (NCMA), or
- National Patient Care Technician (NCPCT), or
- Completion of US Navy Hospital Corps School, or
- Emergency Services to include EMR, EMS, EMT, AEMT and Paramedic, or
- Current student enrolled in a clinical program (beyond prerequisites) having completed at least one semester of RN, LPN, RT or PT, or
- Current work experience within last three years as a PCT or Nursing Assistant.
ExperienceOne year of prior health care experience preferred.

The ambulatory clinical pharmacist is responsible for assuring the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. The pharmacist is embedded within a multidisciplinary clinical team, works under a collaborative practice agreement with the physician(s), and provides direct patient care, via either in-person or telephone interventions. The pharmacist will consult with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals. The pharmacist assesses each of the patient’s medications for indication, effectiveness, safety, tolerability, and adherence issues and provides care in a way that maximizes patient satisfaction. The pharmacist will titrate/manage/monitor the patient’s medication therapy to optimize clinical outcomes and will identify and address gaps in care.Job Duties:
- Assumes responsibility for the delivery of high-quality medication disease management services for the specific patient care area in which the pharmacist is assigned.
- Demonstrates proficiency in medication and disease management, as well as a thorough understanding of the medications, clinical conditions, and current treatment guidelines for the conditions in their assigned clinical areas.
- Gathers appropriate clinical, imaging, and laboratory data necessary to make appropriate decisions on the management of the patient’s medication therapy and related diseases and conditions.
- Assesses patient’s medication therapy regimen for appropriateness, effectiveness, tolerability, safety, and potential for adverse events.
- Make clinically appropriate changes to regimen which will maximize effectiveness and patient satisfaction, while working to minimize potential for both acute and chronic complications Provides in-services, in-depth drug information, and other educational and informational materials to members of the medical, nursing and other healthcare professionals.
- Provides patient with appropriate educational materials and information in order help them understand and actively participate in their care.
- Provides appropriate documentation of each encounter in the electronic health record and conveys that information to the referring physician and next provider of care as appropriate.
- Attends, participates, and upon request, presents at department educational programs.
- Teaches and trains pharmacy interns, pharmacy residents, students, pharmacy technicians and new pharmacists, regarding information in all appropriate areas of pharmacy practice, policies, and procedures.
- Assists the Ambulatory Clinical Pharmacy leadership team in implementation of new clinical programs and sites including building and organization of the new program, education and certification of all staff members Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered, and work performed are in accordance with departmental standards, state and federal regulations, and billed appropriately.
- Advises, assists, and supervises the work of clinic assistants and pharmacy interns; assumes final responsibility for accuracy of their work.
- Works with facility, medical and nursing staff, support staff, and clinic and region administration where necessary to resolve issues, incorporate clinic services seamlessly into site workflow, and to continually demonstrate the value of the ambulatory clinical pharmacy program.
- Assists with the review and revision of departmental policies and procedures, as requested.
- Applies departmental policies and procedures to complex situations and identifies areas for which policies and procedures need to be developed, improved, or revised.

Banner Estrella Medical Center is a 353 bed, 24 bed ICU and level 4 trauma center. This is full time night shift position. It is a 3 12-hour shift per week. The schedule is 6:00PM to 6:30AM.
As a Registered Respiratory Therapist Vascular Access Specialist at Banner Estrella Medical Center you have the opportunity to work with an excellent team with high acuity and diverse patient population. Our Respiratory Care team provides care 24/7 for patients requiring services ranging from simple oxygen therapy to advanced ventilator support. The department also performs diagnostic testing and emergency medical support including CPR. Four years of clinical experience as an registered respiratory therapist in critical care, advance practice experience is preferred. The shift is 6:00pm-6:30am with rotating weekends and holidays.

This position independently provides and maintains peripheral and central venous access. This position conducts patient assessments and provides recommendations regarding care, serves as a resource person and specialist and provides assistance with departmental activities, quality improvement and development of best practice guidelines.
CORE FUNCTIONS
1. Reviews labs, patient history and physician orders to determine appropriate line to be utilized for the procedure and/or identify potential issues that may arise or preclude the patient from being a viable candidate or require alternate form of vascular access. Collaborates with physicians to determine appropriate long-term access needs. Obtains patient’s authorization to perform the procedure. Inserts peripheral venous and/or arterial catheters to provide access or more complex vascular access as indicated.
2. Inserts and maintains peripherally inserted central catheter (PICC) lines according to department policy and procedures to include dressing changes, line reviews and declotting. Utilizes ultrasound technology to ensure accurate insertion of the PICC line. Reviews chest x-rays to ensure accurate placement of the central venous catheter or device. Maintains sterile field throughout the insertion process to ensure quality outcomes. Maintains accurate, timely and appropriate documentation.
3. Inserts and maintains all central lines (simple and complex) in the neck and chest according to department policy and procedures to include dressing changes, line reviews, port access and declotting. Utilizes ultrasound technology, advanced knowledge of anatomy and specialized insertion techniques to ensure accurate and appropriate insertion of the central line. Reviews chest x-rays to ensure accurate placement of the central venous catheter or device. Maintains sterile field throughout the insertion process to ensure quality outcomes. Maintains accurate, timely and appropriate documentation.
4. Participates in ongoing monitoring of quality initiatives, department database, appropriateness and effectiveness of bundle compliance, acts as a change agent and participates in the development, interpretation, implementation and evaluation of quality goals/objective and quality management activities within the clinical service lines. May coordinate oversight of quality and safety standards, regulatory and accreditation requirements, as well as applicable professional standards.
5. Acts as a resource to therapists and other healthcare practitioners regarding patient status, equipment and operational issues. Participates in interdisciplinary rounding, making recommendations for changes in therapy dictated by the patient’s condition.
6. Provides training, orientation and patient education to staff, patients and families, in accordance with policy and practice guidelines. Assists in the development of departmental policies and procedures based on best practice.
7. Assures the efficient operation of workflow of the department. Monitors and maintains an adequate inventory of supplies and material to ensure non-interruption of services. Provides maintenance of associated equipment as demonstrated through observation and documentation.
8. May, on occasion, provide respiratory therapy services as prescribed and in accordance with established department/hospital policies and procedures, including inserting and maintaining arterial lines, assisting in FDPAC and IAPB insertion and associated invasive cardiac outputs/profile and non-invasive cardiac outputs. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Banner Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.
9. This position independently provides central and peripheral vascular access, and advanced clinical monitoring techniques to a diverse patient population. Serves as a resource person and may provide direction to staff. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physicians and their office staff, payers, community agencies and regulatory agencies.
MINIMUM QUALIFICATIONS
Associate or technical degree. Successful completion of a training course in PICC and Banner Health Sims Lab training for CVC insertions.
Proof of good standing with the National Board for Respiratory Care (NBRC) as a Registered Respiratory Therapist (RRT). A current license in state practiced as required by state law.
Certification for Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) required.
In-depth knowledge of clinical techniques, applications and equipment as typically obtained through 4 years of clinical experience as a RRT, including 1 year of independent PICC or CVC line insertions.
Must possess highly effective human relation skills necessary to interface with patients, families, staff and other healthcare personnel.
PREFERRED QUALIFICATIONS
CPUI and VA-BC certifications preferred.
Additional related education and/or experience preferred.

MINIMUM QUALIFICATIONS
Must have graduated from an APTA accredited school of Physical Therapy, or equivalent.
Must possess a current Physical Therapy license within the State of practice. Per California regulations, internationally trained candidates have six-months from the time of hire to obtain a current license. BLS certification required.

Department/Position Overview:
Inpatient Rehab (IPR) is an 8-bed unit specializing in providing patients with additional rehabilitation needs for 2-3 weeks when they are not independent enough to go home but do not require a skilled nursing facility. We work closely with physical therapy, occupational therapy, & speech therapy teams while providing care to these patients. We become familiar with our patients and their routines over this longer stay. We enjoy watching our patients progress to going home and that is rewarding as a patient care technician. The patient population includes but is not limited to stroke, spinal cord injury, patients that required amputation or other orthopedic patients that require more therapy after being medically cleared from the hospital.
Patient Care Technicians are unlicensed staff members who are accountable to, and work under the direct supervision of a professional nurse to implement delegated aspects of nursing care. Provide patient care, support the admissions and discharge process, and comply with patient safety practices under the direction of the professional nurse. Observe patients' physical, mental, and emotional conditions and report any change to the nurse.

This position independently provides respiratory care, pulmonary and cardiology diagnostic testing to patients in a variety of health care settings.
CORE FUNCTIONS
1. Performs all basic modalities of respiratory care as prescribed by physicians and/or licensed independent practitioners in accordance with established department/hospital policies and procedures.
2. Applies and manages mechanical ventilators in collaboration with physicians and/or licensed independent practitioners, recommending adjustments to improve clinical condition, patient comfort and to expedite the weaning process.
3. Assembles, disassembles, operates, maintains, troubleshoots, cleans and performs adjustments in the applications of durable medical equipment and circuitry used in the delivery of respiratory care as prescribed in accordance with established department/hospital policies and procedures.

5. MINIMUM QUALIFICATIONS
Associate or technical degree.
Proof of having passed the National Board for Respiratory Care (NBRC) as a Registered Respiratory Therapist (RRT). RRT required for all new hires to the position after October 1, 2015. Incumbents in the position as of October 1, 2015 must possess their RRT by October 1, 2017. A current license in state practiced as required by state law. In the state of Nevada, all incumbents must have their Blood Gas Technologist certification.
Banner Registry positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Respiratory Therapist in the specialty area.
Basic Life Support (BLS) required. In Home Care setting, must posses valid drivers license and be eligible for coverage under the company auto insurance policy.
Must be knowledgeable of clinical techniques, applications and equipment as typically obtained through one year of clinical experience. This position is 40-hours per week will attend key facility, regional and system meetings. This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost effective patient care. Demonstrates clinical expertise and oversees the leadership of clinical, financial, and personnel management of the assigned areas.

Requires a current RN license in state of practice. Requires BLS certification.
Must possess a strong knowledge and understanding of clinical care management as normally demonstrated through two years of clinical management experience relevant to patient population. The Integrated Mental Health Therapist (IMHT) works within a multidisciplinary team embedded in the primary care clinic and utilizes the Collaborative Care model of care. This team includes the IMHT, Psychiatric Consultant, & Primary Care Provider to provide comprehensive behavioral health services within the primary care setting. IMHT will provide evidence-based interventions, in partnership with the treatment team, to promote symptom reduction and optimal patient outcomes.
Responsibilities:
- Assesses patient’s symptoms and create customized treatment plan based on individual patient needs
- Screens patients for common behavioral health disorders using PHQ9, GAD7, Columbia Suicide Risk Assessment, or other tools as required.
- Monitors patients for changes in clinical symptoms and treatment side effects/complications, with a goal of reduction in symptoms.
- Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and treatment effectiveness.
- Tracks patient follow-up and clinical outcomes using a registry, and uses the system to identify and re-engage patients
- Facilitates care plan changes for patients who are not improving, including changes in medications or psychosocial treatments or appropriate referrals for additional services.
