SUMMARY: Reporting to the Social Work Manager, the Social Worker LSW evaluates, diagnoses and treats a wide range of psychosocial problems, applying individual clinical social work skills in order to assist patients and their families in the resolution of personal and environmental problems, attributable to mental or physical illness, which prevents them from achieving their highest level of psychosocial functioning. The Social Worker assigned to the impatient units will be an active participant in the co management model along with the unit leadership in supporting the patient flow initiative. Adheres to social Work professional standards of practice based on NASW Code of Ethics. Is responsible for knowing and acting in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct.Completes bio-psychosocial evaluations to eliminate barriers to treatment, discharge and enhance the patient’s health status and improve utilization of resources.Provides consultation regarding patient’s background, physosocial functioning and needs to medical and support staff, in order to assist in planning a comprehensive care program for the patient in order to increase treatment compliance and care coordination with the aim at providing excellent patient experience and increased satisfaction in care.Develops treatment plans and initiates clinical intervention, utilizing individual and group treatment modalities, and analyzes course and result of treatment to maximize health status and increase health care decision making. In accordance with departmental standards regarding frequency and content, documents social work activity in patients’ medical records. Participates as a team member in conferences (collaborative care rounds) concerning patients with other care personnel and community agencies to improve care coordination and moving patients safely into their communities and less restrictive settings.Provide education to assist patients and families in understanding, accepting and following medical and social work recommendations designed to restore the patient to optimum functioning within their capacity.Assesses patient and family advocacy needs, and coordinates appropriate advocacy activities within the hospital and in the community with an emphasis on self-determination for the client system with the aim at improving social supports and community resources.Provides field placement supervision and evaluation for BSW and/or MSW degree candidates as part of the Department of Clinical Social Work Student program (must have LICSW, or, at least 5 years of demonstrated experience and knowledge in service area).Completes case related statistics and other reports according to departmental standards.Participates in hospital wide, departmental and community studies, projects, and trainings with the aim at educating the community and hospital staff on the psychosocial issues related to illness and identifying barriers in service delivery systems.As relevant to clinical assignment, remains current on available community resources, and accesses them as needed.Maintains and enhances professional expertise through appropriate educational activities. May attend training for specific areas of clinical responsibility.Participates in quality assurance activities in accordance to department, hospital, and The Joint Commission standards.Adheres to Brown University Health, Hospital, and department policies and procedures to maintain a safe environment of care and be in compliance with all laws, regulations and standards. MINIMUM QUALIFICATIONS: BASIC KNOWLEDGE:Masters Degree in Social Work from an accredited graduate school of Social Work and a current license from State of RI, Board of Social Work Licensure.Minimum requirement for licensure is Licensed Clinical Social Work level (LCSW). When Social Worker is eligible (two years or 2000 hours of post masters supervised practice) it is mandatory that they obtain Licensed Independent Clinical Social Work (LICSW.) status.Demonstrated knowledge and skills necessary to provide care patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.EXPERIENCE:Between six months and one year of clinical experience preferably in a health or mental health setting with an emphasis on populations to be served in the assigned area.Training and experience necessary to address the specific needs of the patients served in assigned areas of responsibility; this includes issues relating to age and stage of development and the particular special needs and cultural patterns of the patient populations being served.Analytical and therapy skills necessary to evaluate patients, develop treatment plans, initiate therapy, and evaluate results. Ability to effectively interact with patients, families, medical staff and outside agencies in providing services.WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:Works in a normal office environment with exposure to patient care.SUPERVISORY RESPONSIBILITY:Provides functional guidance to Social Work Interns with recommendation to pursue LICSW level of licensure.

Responsible for designated daily operations and/or clinical activities of assigned area.
Appropriately dispenses pharmaceuticals, including sterile products, chemotherapy, biologics, controlled substances, and investigational medications.
Evaluates all providers’ medication orders for completeness and appropriateness prior to dispensing; consults with providers and nurses to clarify orders as needed.
Performs quality control on medication orders and prepared medications and additional quality control activities as necessary (e.g., refrigerator temperature, compounding logs, hood cleaning, emergency cart, and controlled substance inventory, etc.).
Reviews and compares investigational protocols to medication orders for completeness and appropriateness prior to dispensing.
Oversees pharmacy technicians and students as assigned.
Participates in the training and orientation of newly hired pharmacists and technicians.
Competent in an advanced role in the area of routine assignment.
Provides contributions to training, education, and multi-disciplinary activities.
Other duties and projects as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Ability to effectively communicate both orally and in writing.
Ability to work effectively as a member of a unit team and cross functional teams both inside and outside of department (i.e. patients, providers, other institutions).
Strong working knowledge of and ability to use all technology required to perform primary job functions.

MINIMUM JOB QUALIFICATIONS:
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy. 5 years of hospital pharmacy, ambulatory care, sterile compounding experience with 4 years oncology experience required. Or completion of a PGY2 oncology residency or relevant PGY2 specialty.
LICENSE/CERTIFICATION/REGISTRATION:
Must be licensed to practice pharmacy in the Commonwealth of Massachusetts. Some satellite locations require additional licensure. Board Certification related to the area of practice is preferred.

Position Summary
The Occupational Therapist is an exciting opportunity at MultiCare, responsible for evaluating, planning and implementing occupational therapy services for in-patients and outpatients referred for Occupational Therapy. With varied and dynamic day-to-day functions, you will apply your service orientation, analysis and good judgment toward ensuring comprehensive occupational therapy care for patients.
Responsibilities
- You will perform well in stress and emergency situations
- You will perform patient evaluations/re-evaluations, establish a treatment plan, and modify the treatment plan according to the needs of the patient and response to treatment
- You will perform treatment interventions for outpatient and/or inpatient population as directed
- You will perform documentation in the medical record that is timely and accurate
- You will communicate well with patients, peers, other health professionals in the coordination of care for the patients

Cardiovascular Surgical ICU Unit Description:
• 20 Bed cardiac surgical ICU
• Nurses handle 1-2 patients on this unit based on acuity level.
• Nurses provide service to any patients that have undergone open heart surgeries such as CABG, Valve repair or replacement, aneurysm repair, TAVR, ablations, Heart Transplantation, and Mechanical Circulatory Support devices including the Total Artificial Heart and Extracorporeal Membrane Oxygenation (ECMO), and various lung surgeries.
• Nurses manage critically ill patients that require intensive hemodynamic monitoring, mechanical ventilation, multiple drug infusions, IABP therapy and CRRT.
The Sentara Heart Hospital, a 112-bed care facility, is #2 for Cardiology services and Heart Surgery in the State of Virginia and ranks #24 in the nation for best heart programs.

Sentara Norfolk General Hospital is hiring a Patient Care Technician (PCT) for the Progressive Ventilator Support Unit (PVSU). This is a Full Time, Night Shift (12-hour shift) position.
Hours: 7:00pm to 7:00am - Three 12-hour shifts (36 hours per week), with rotating weekends.
Department Overview: The Progressive Ventilator Support Unit (PVSU) is an eight-bed step-down unit, specializing in treatment of patients on mechanical ventilators. Most patients admitted to PVSU come from an Intensive Care Unit (ICU). PVSU focuses on maximizing the rehabilitation while continuing to treat the cause of the illness. This goal is accomplished through intensive physical therapy, respiratory care and mental stimulation.
The Patient Care Technician (PCT) provides patient care, supports independent and dependent activities of daily living, vital signs, mobility and ambulation, phlebotomy, point of care testing, EKG testing, peripheral IV removal, specimen collection, and foley catheter insertion and removal, and complies with patient safety practices under the direction of the professional RN nurse. Observes patients' physical, mental, and emotional conditions and documents in a timely and accurate manner while reporting any change to the nurse or medical staff.

Education:

- Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and treatment effectiveness.

Registered Respiratory Therapist who is responsible for performing various therapeutic procedures including advanced practice procedures such as, but not restricted to operating balloon pumps, working in NICU, performing Pulmonary Function testing, “PFT”, etc.

Uses "AIDET" principles when interfacing/treating every patient throughout the organization. (Acknowledge, Introduce, Duration, Explanation, Thank You).
Where appropriate, with each patient interaction, uses the concept of "Managing Up" – a form of communication that positions information, a person, or team in a positive manner.
Provides a consistent experience to patients, families, visitors and internal customers by using "Key Words at Key Times." Specifically, uses key words to help patients to understand what is happening with their care, help internal customers understand their service better, and build relationships.
Serves as a consultant, resource person, and role model representing respiratory care in the health system committees or work groups as needed.
Maintains the confidentiality of information according to HIPPA guidelines.
Facilitates good rapport and cooperative relationships approaching conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
Maintains safe respiratory therapy practices; alerts management of any safety issues or concerns.
Performs all duties and responsibilities of a Respiratory Therapist per AARC guidelines obtains and maintains competencies per RT II orientation. Some examples below:
Cardiopulmonary arrest situations Other emergency situations Participates in patient rounds Ensure treatments are provided in accordance with physician orders and approved medical protocols. Banner – University Medicine Family Medicine Clinic takes pride in the excellent care patients receive by combining academic training with faculty physicians. The Center’s care includes pediatrics, adolescents, adults, obstetrics/gynecology and geriatrics. We provide preventive care and manage chronic diseases. Family-centered maternity care is unique to our practice. We provide prenatal care, delivery, and newborn care for your baby. The office features 22 exam rooms with specialty areas for colposcopy, IUD insertions and removals, circumcision, skin procedures, joint injections, osteopathic manipulation, toenail removals and counseling.
As a LPN in this clinic, you will provide patient care under the direction and supervision of a licensed physician, and be accountable for the quality of services delivered by yourself or others who are under the physician's direction. This position observes, compiles and reports the patient’s physical, psychological, social, spiritual, educational, developmental, cultural and other needs. Experience with pediatrics preferred. Seeking 1-3 yrs of experience.
This position is full-time, 40 hrs wk., Monday-Friday 8am-5pm.

This position provides nursing care under the direction and supervision of a registered nurse and/or licensed physician, and is accountable for the quality of nursing services delivered by self or others who are under his or her direction. This position observes, contributes to the plan of care, implements, reports and documents patient care in accordance with department policies, standard of care and state scope of practice. This position utilizes specialized knowledge, judgment, and nursing skills necessary to contribute to the assessment of data and to plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Observes, compiles and reports the patient’s physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews available information obtained by health care team members.
2. Contributes to plan of care under direction of registered nurse, including the discharge plan, utilizing assessment data and patient, family and health team input. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of care. Plans care in collaboration with members of the multidisciplinary team. Reviews plan of care to reflect changing patient needs and provides input for updates as needed.
3. Implements care based on delegated interventions identified in care plan and medical orders, within scope of practice. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. May delegate and provide supervision in the provision of care to non-licensed personnel, in accordance with state scope of practice. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Reports condition of patient, including signs and symptoms which may be indicative of change in the patient’s condition to the registered nurse and/or physician. Observes and reports patient progress towards goals and expected outcomes in collaboration with other health care team members. Observes and reports patient’s response and the effectiveness of patient teaching.
5. Documents observation, implementation and data in the patient record. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. Notes physician orders accurately and in a timely manner.
6. May plan, organize, develop and coordinate the completion of documentation and the input of patient assessment data as required by federal regulation. Maintains, updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information for management utilization and review.
7. May coordinate the admission process including new resident evaluations. Provides verbal and written communication to residents, families and referrals sources regarding the process for admission to the center. Facilitates problem solving with residents, families, referral sources, insurance companies and clinical staff.
8. This position works under supervision, prioritizing data from multiple sources to provide quality care and support to the patient and family. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians.
MINIMUM QUALIFICATIONS
Must possess a current, valid LPN or LVN license in state of practice, temporary LPN or LVN license in state of practice, or compact LPN or LVN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice. Completion of a state approved IV therapy program may be required in certain facilities and/or departments in accordance with state scope of practice, regulatory requirements or facility/ department policy.
Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a LPN or LVN in the specialty area. Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Experience in designated clinical area is preferred.
Additional related education and/or experience preferred.
