Essential Functions
- Evaluates patients, selects and administers assessment techniques, synthesizes information to establish appropriate patient care plan to include necessary modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by a holistic view of the patient's diagnosis, condition, age, and discharge needs.
- Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation.
- Establishes functional goals in conjunction with the patient and provides evidenced based intervention to work towards meeting these goals with ongoing assessment to adjust the plan of care as needed .
- Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed.
- Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff, coordinates patient's schedule and treatment programs, and discharge planning.
- Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
Physical Requirements
Hearing (corrected) adequate for oral/aural communication. Vision (corrected) adequate for reading. Intelligible speech and adequate language/cognitive skills to perform job duties. Sitting, standing, and walking required throughout the day. Job duties sometimes require climbing stairs, kneeling, twisting, bending; on occasion, crouching, crawling and reaching overhead. Lifting of patients, equipment or supplies will be required up to 20 pounds frequently and 50 pounds occasionally. Able to laterally transfer patients 100 pounds rarely. Must be able to demonstrate any appropriate exercise and activities to patients/caregivers. Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids. Work is in a fast-paced clinical environment. The work environment is primarily indoors but occasionally outdoors.
Education, Experience and Certifications
Bachelor's degree in Physical Therapy program required. applicable state license for Physical Therapy required. BLS required per policy guidelines.

Probility Physical Therapy is looking for dedicated Personal Trainers/Fitness Instructors and/or Kinesiology and Exercise Science professionals to join our team in the outpatient physical therapy setting in Ann Arbor, MI. Specialized training is provided to support our clinical staff and guide patients with the instruction of exercise programs. - Conducts a physical therapy evaluation of each patient and establishes a plan of care and treatment goals. Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluates outcomes within an appropriate time frame.
- Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
- Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
- Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Manages assigned caseload for optimal functional and financial outcomes; reviews caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.
- May manage a caseload of Physical Therapist Assistants (PTA) including evaluating their patients, insuring compliance with established Care Plan, and adhering to all State of Wisconsin regulations regarding PTA supervision.
- Performs Home Health Aide joint visits as appropriate to maximize patient's functional abilities in the home. May assist with evaluating patients, turning patient over to PTA and informing PTA of Plan of Care (POC) per State of Wisconsin regulations. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.
- Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management, and clearly states plans, actions and goals so changes are reflected as they occur.
- Participates in chart audits and continuous quality improvement activities. Facilitates discharge planning process and utilizes interagency/system and community resources to assure continuity of care after discharge.
- Acts as a resource to employees, which includes training/orienting, mentoring, and providing guidance on more complex issues. Serves as an instructor for students, meeting all expectations of the Clinical Student Program.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Physical Therapy License in the State of Illinois or State of Wisconsin, must be licensed in the State upon which you work.
- Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
- A valid drivers license issued by the Division of Motor Vehicles.
Education/Experience Required:
- Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.
- Requires 1 year of clinical experience or demonstration of strong spectrum of clinical affiliation experience
Knowledge, Skills & Abilities Required:
- Demonstrated knowledge of indications and contraindications of modalities and exercise.
- Demonstrated skill in use of modalities, exercise equipment, and other devices that are available in work area.
- Demonstrated proficiency in clinical skills with the ability to work independently and make sound judgments.
- Good human relations, organization, problem solving, and decision-making skills.
- Basic proficiency in the use of Microsoft Office (Excel, PowerPoint and Word) or similar products.
Physical Requirements and Working Conditions:
- Must be able to obtain transportation to attend to the daily schedule of home visits and other business in a timely manner.
- Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment.
- Must have the physical ability to move about in confined spaces, including bending, twisting, kneeling, squatting and occasionally reaching one or both arm overhead.
- Must be able to work safely with people of various sizes and weight with diverse and complex medical conditions and physical limitations.
- Must be able to:
- Lift up to 50 lbs from floor to waist.
- Lift up to 20 lbs from waist to overhead.
- Lift and carry up to 40 lbs. at waist height reasonable distance.
- Must be able to:
- Push/pull with 30 lbs. of force.
- Perform a sliding transfer of 150 lbs. with a second person present.
- Must be able to sit, stand, walk and climb stairs for limited to prolonged periods of time to complete required paperwork, gain entry to patients’ residences, travel from assigned visits and attend a variety of work related activities.
- Must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest.
- Operates all equipment necessary to perform the job.

Major Responsibilities:
- Implementation of Occupational Therapy treatment programs for patients with multiple and specialized diagnoses safely and effectively, as planned, in conjunction with OTR.
- 1)Performs therapy treatment programs on inpatients and/or outpatients.
- 2)Instructs patients and patients' families in home programs and provide education as needed.
- 3)Trains patients in ADL skills and use of ADL equipment.
- 4)Monitors patient response to treatment modifying as necessary.
- 5)Follows established procedure and safety precautions in the application and use of modalities.
- 6)Acts as an assistant to the OTR when the OTR is performing tests, evaluation, and treatment procedures.
- 7)Conferences with physicians and other allied health professionals to make recommendations for occupational therapy treatment programs.
- 8)Schedules patients according to need/priority.
- 9)Maximizes support personnel by delegating as appropriate. 11. Shares treatment and office space.
- 10)Shares office and treatment space.
- Documents and maintains appropriate records and productivity.
- 1)Prepares, maintains, and transmits accurate patient documentation including, EMR, charges, and productivity logs.
- 2)Prepares discharge summaries and evaluation notes to be cosigned by OTR/L.
- 3)Observes and records patient’s progress throughout the treatment period to assure continuity of care and provide accurate records.
- 4)Provides at least 5 billable hours/8 hour day inpatient at least 6 billable hours/8 hour day outpatient treatment.
- Communicate information/observations to appropriate personnel.
- 1)Communicates effectively with Supervisor and OT intra and interdisciplinary team members, as well as other hospital personnel to insure familiarization with the department’s purpose and functions; to promote cooperative and efficient working relationships and insure consistent, quality OT patient treatment.
- 2)Attends and participates in department and multi-disciplinary meetings on regularly scheduled days and checks meeting minutes if unable to attend.
- 3)Attends case conferences as indicated with physicians, nurses, other related personnel and family to discuss and evaluate patient’s condition, progress and plans for further OT
- 4)Maintains OT equipment in proper working condition and reports any malfunction to the Physical Medicine Supervisor.
- Participation in Educational and Performance Improvement Activities.
- 1)Presents a minimum of one in-service to OT and/or physical medicine staff annually.
- 2)Attends department in-services on regularly scheduled days.
- 3)Attends a minimum of one continuing education course annually.
- 4)Provides in-service training to hospital personnel and OT staff regarding OT techniques and equipment as needed.
- 5)Assists in student program by assisting with student observation opportunities. After 1 year of experience, COTA may also supervise a COTA student as assigned.
- 6)Participates in performance improvement initiatives within the department or organization.
- 7)Completes all mandatory competencies and CBTs within specified time frames.
- 8)Keeps abreast of current practices via self-study, networking; internal and external resources.
- Performance of “Activities of Daily Living” evaluations and structured assessments as directed by an OTR.
- 1)Beginning competency in ADL evaluations and contributes to overall initial intake evaluation as directed by OTR.
- 2)Re-evaluation of patient at timely intervals for caseload and implement goal modification and co-signed by the OTR.
- 3)Perform discharge assessment on caseload (outpatient) and contribute to discharge planning with patient treatment team.
- Assumes other related responsibilities as assigned.

Licensure, Registration, and/or Certification Required:
- Occupational Therapy Assistant
- -Current AOTA Certification -Current IL/WI licensure or obtain IL licensure within 90-days of hire -Current CPR Certification
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
A valid driver’s license issued by the Division of Motor Vehicles.
Education/Experience Required:
- -Graduate of an AOTA approved Occupational Therapy Assistant Program -Successful completion of two month supervised Level II fieldwork experience. -Successful completion of certification process.

Physical Requirements and Working Conditions:
- This position involves: bending, lifting, assisting patients to and from wheelchair, bed toilet which often requires physical strength, endurance and use of proper body mechanics. Set up of certain equipment requires upper body strength to tighten knobs, etc. Lift weights, push/pull. Physical stamina during emergency situations may be required and often can not be delegated. Perform CPR. May be scheduled to work weekend, holiday and evening shifts. Have access to transportation. Potential exposure to blood/body fluids.
- This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
Addendum: ADDITIONAL INFORMATION: This position will need to recognize needs and behaviors of a variety of age groups of patients treated, i.e., understand child growth and development, normal patterns of adolescent behavior and look for signs of normal aging, prior experience with various age groups is preferred. Requires use of universal precautions as appropriate.

Position Summary
MultiCare has an exciting opportunity for a student enrolled in an accredited Respiratory Care Program within the State of Washington to join our Intern program. Under the general supervision of a licensed respiratory care practitioner, you will provide respiratory care to patients of all ages, limited to only those procedures that have been completed in the associated Respiratory Care School curriculum. In addition, to clinical duties, the Respiratory Care Intern will also function as an equipment assistant for the various Respiratory Care Departments within MultiCare.

- Provides respiratory care to assigned population under the guidance of a licensed Respiratory Care Practioner
- Cleans, assembles, and maintains respiratory equipment; distributes equipment and supplies within the facilities; assesses non-stock par levels and communicates needs for ordering process
- Assists in special projects as delegated by leadership team.

- BLS required
- Currently enrolled and in good standing in an accredited Respiratory Care Program within the State of Washington

Specialist technologists analyzes blood, urine, and bone marrow and body fluid specimens in order to provide data that helps our physicians determine the best treatment for their patient. Automated and manual techniques are routinely used to properly evaluate samples in patients with a wide range of clinical conditions. Testing responsibilities include moderate and high complexity assays which are performed in compliance with accrediting organizations as well as departmental policies and procedures. The specialist technologist may be required to evaluate results, troubleshoot, and assist in developing new test methods. This position provides direct line supervision to professional and technical staff in the Stem Cell Processing Lab section. The specialist technologist coordinates and assists in teaching and training students and residents, as well as maintenance of the lab section's QA, regulatory compliance and standard operating procedures.
Education:
Bachelor's Degree in Medical Technology or a chemical, physical or biological science from an accredited university and meets other technologist qualifications.
Experience:

Essential Functions:
1. Adheres to general hospital standards. Follows hospital policies and procedures. Follows departmental policies and procedures. As an Inpatient Pharmacy Tech Trainee at MercyOne, you will be learning to assist Clinical Pharmacists in preparing and distributing medications and maintaining the drug inventory and maintaining patient records in Pharmacy. These duties require skill and training but do not include making clinical judgments that require a pharmacist professional judgment. Pharmacy Technician Trainees work only under the supervision of a registered pharmacist. They do not perform duties that can legally be performed only by a registered pharmacist. Ages of patients served range from neonatal to geriatric.

General Requirements:
- The major job responsibilities (MJRs) of this position are: batch preparation, new dose preparation, stock preparations, Pyxis functions, inter/intra department orders outside routine processes, maintenance of assigned area(s), liaison activities, medication distribution processes and departmental service requirements.
- Procures and distributes medication orders per physician request, pharmacist request, or prearranged work assignment according to established policies, procedures, and protocols.
- Assigned Pharmacy work area is kept clean, neat, and well-organized during shift. Pharmacy equipment is kept clean and reports any potential or actual equipment failures in a timely manner.
- Assists the Pharmacist with filling physician orders, medication kit and/or trays, medication carts, and floor stock requisitions accurately.
- Prepares intravenous admixtures and other sterile preparations (including chemotherapy and parenteral nutrition) safely and accurately using appropriate aseptic techniques.
- Replenishes medications in emergency drug containers and floor stock areas accurately.

Job Description:
Assess, plan and direct patient mobilization and exercise programs in an inpatient setting. Works closely with other disciplines and specialties within the department and facility as well as patients and families to assure a cohesive approach to patient care.
EDUCATION/EXPERIENCE:Bachelor's degree in a Health/Fitness field required; prefer Exercise Physiology degree. Master's Degree preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION: Exercise Specialist certified by the American College of Sports Medicine or eligible to test for the same. ACSM-CEP (Clinical Exercise Physiologist) or ACSM EP-C certification preferred. Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.
