Job Overview:
The autonomous nature of the Licensed Clinician role lets clinicians function like a private practice without the hassle of managing a back office.
Ability to work within a 40-hour work week! The majority of your time will be clinically focused, with the remainder focused on patient documentation and facility correspondence
You would be an on-site clinical provider working with residents and their families.
Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
You have the opportunity to be in charge of your caseload, your schedule, and ultimately, your salary.
Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support
Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
Key Requirements:
We’re looking for someone with an entrepreneurial spirit accompanied by strong medical knowledge, understanding of documentation (CMS, Medicare, Medicaid) and nursing home/ hospital facility experience.
Have a master’s degree and a valid LCSW/LMFT/LPC license in the state for which you are applying.
Two years of counseling and therapy experience
Ability to work as a member of a team with a great attitude
Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
Comfortable with the ever-changing environment of a skilled nursing facility
Additional Requirements:
Knowledge of the Medicare Benefit and commercial insurance plans
Experience in the healthcare setting
Passion for working with older adults
Exceptional organizational skills
Experience in the nursing home space is a plus

Team Rehab offers the opportunity to open and lead a clinic with an ownership path, supported by the systems, resources, and mentorship of an established organization.
Summary:
The Clinic Director for Team Rehabilitation is a unique model for the Physical Therapist who wants to have ownership in a clinic while still having the support of a company. This leadership role’s primary responsibilities are growing, developing and managing an outpatient physical therapy clinic. The only way to drive success is in providing exceptional physical therapy with measurable outcomes. The ideal candidate will be passionate about patient care while regularly exceeding industry standards.
Strategic Planning:
This is a partnership. The Clinic Director is key in clinic set up. If opening a new clinic, will provide input on clinic location and clinic layout..
Take control of Quality of Care in the clinic. Taking a patient-centered approach so that each patient receives the care that is unique to their goals and objectives.
The Clinic Director has all the normal responsibilities of a Physical Therapist with the addition of hiring and managing the right staff.
Marketing:
Supporting the Marketing Reps and fellow clinics’ marketing initiatives are a key component in both the success of the clinic and of Team Rehabilitation.
Maintain relationships with the key physicians and referral sources to promote Team’s commitment to care and increase patient referrals.
Attend lunches and functions set up by Marketing.
Ensure that the clinic is a positive influence in the community.
Financial and Operational Management
Keep the clinic full of patients by overseeing scheduling and treatments that will enhance the patient’s experience.
Manage productivity. Define and set clear, reasonable and measurable expectations.
Manage the clinic budget to make financial decisions that support the clinic’s overall financial health.
Be aware of clinic expenses, such as patient travel, office supplies and medical supplies.
Make sure that the clinic complies with all state and federal healthcare regulations. Part of those responsibilities include staying in touch with ever-changing healthcare laws and policies and ensuring the clinic is in compliance.
Collaborate with Revenue Cycle Management to ensure accurate billing and reimbursement.
Monitor key performance indicators (KPIs) to maximize clinic performance and profitability.
Give input on policies, procedures and strategy to ensure that all Team Rehabilitation clinics provide the highest quality of care.
Handle patient feedback and complaints promptly and professionally.
Leadership and Staff Management
Lead, mentor, and develop clinic staff, fostering a collaborative and growth-oriented environment.
Manage employee schedules, performance reviews, and training.
Promote staff engagement and retention through effective communication and support

Qualifications:
Doctorate or Masters in Physical Therapy

*Summary of Principal Accountabilities*

* Maintain completeness of each assigned resident’s medical record by completing all assigned evaluations, progress notes, discharge summaries, clarification orders, etc. according to posted schedules for the shift, making sure entries are timely, informative and accurate.
* Establish and maintain professional and positive working relationships with residents which reflects person-centered care. Honor the resident’s rights and provide care in a manner that promotes resident dignity and privacy.
* Establish and maintain a professional and positive working relationship with Certified Occupational Therapy Assistants (COTA). The OT is responsible to supervise the COTA in skilled tasks during group and individual treatment in a manner that promotes efficient and effective utilization of resources and the ongoing development of clinical skills. The OT will refer patients to the COTAs for treatment as appropriate and be available for consultation and clinical supervision to COTAs as needed. The OT should be familiar with what treatment can appropriately be delegated to a COTA and consult with the Director of Rehabilitation with any questions or concerns.
* Coordinate/collaborate on resident care with other members of the interdisciplinary and therapy teams. This may include attending or providing updates for resident care conferences and utilization review/case management meetings, reporting changes in resident condition or allegations of abuse, and making referrals to other members of the interdisciplinary and therapy teams as appropriate.
* Remain professionally informed and enhance clinical expertise as well as professional and management skills through interaction with managers, therapists and other professional staff, continuing education courses/trainings, and attendance at facility in-services and trainings. Implement new programs/techniques to enhance occupational therapy rehabilitation services.
* Understand relevant payment models related to billing and treatment guidelines as well as clinically appropriate means/modes of delivery. Provide quality care that falls within the guidelines of reasonable duration and type of services.

_*What You'll Do*_
As a member of our ECC nursing team, you’ll be integral to managing critically ill or injured patients through every step of their care. You will:
* Perform emergency triage and obtain vitals
* Assist in stabilization and intensive care procedures
* Administer treatments, medications, and fluid therapy
* Monitor pain, vitals, and clinical trends in real-time
* Assist in anesthesia and emergency surgical prep
* Communicate directly with doctors, support teams, and families
* Use advanced EMR systems for efficient and accurate patient tracking
_*What We're Looking For*_
* Credentialed technician (CVT, LVT, RVT) or eligibility within 6 months
* Minimum 2 years of ECC or emergency nursing experience
* VTS (ECC) candidates encouraged to apply
