Position Summary
Mountain View Health & Rehabilitation is looking for a *compassionate, motivated, and team-oriented Physical Therapist* to join our growing rehabilitation team.
Our therapy department is a cornerstone of the care we provide. We believe therapy should be more than completing exercises—it should help residents regain confidence, maintain independence, and get back to doing the things that matter most to them.
The ideal candidate is someone who brings clinical expertise, positive energy, creativity, and genuine compassion to their work. You will have the opportunity to work closely with residents, families, nursing, therapy, and other members of the interdisciplinary team to provide individualized, resident-centered care.
Essential Duties & Responsibilities
* Evaluate residents to determine physical therapy needs and develop individualized treatment plans.
* Provide skilled physical therapy services in accordance with physician orders, care plans, and applicable regulations.
* Develop treatment goals based on each resident's medical condition, functional abilities, and personal goals.
* Provide therapeutic exercises, functional mobility training, gait training, balance activities, strengthening, and other appropriate interventions.
* Assist residents with improving mobility, strength, coordination, balance, endurance, and overall functional independence.
* Monitor residents' progress and modify treatment plans as appropriate.
* Document evaluations, treatments, progress, and discharge information accurately and timely.
* Communicate changes in residents' condition or functional status to the appropriate members of the care team.
* Collaborate with nursing, occupational therapy, speech therapy, social services, dietary, and other disciplines to provide comprehensive care.
* Participate in care plan meetings and interdisciplinary team discussions as appropriate.
* Educate residents and families regarding treatment plans, safety, mobility, exercises, and strategies for continued progress.
* Recommend and assist with appropriate adaptive equipment, positioning, mobility devices, and safety interventions.
* Promote fall prevention and safe mobility throughout the facility.
* Assist with discharge planning and provide recommendations for continued therapy or services when appropriate.
* Maintain compliance with facility policies, professional standards, and applicable federal and state regulations.
* Maintain a clean, safe, and organized therapy environment.
* Participate in quality improvement initiatives and facility programs as requested.
* Perform other duties as assigned.
What We're Looking For
* Graduate of an accredited Physical Therapy program.
* Current Physical Therapist license in the State of Montana or eligibility for Montana licensure.
* Current CPR certification or ability to obtain as required.
* Strong clinical assessment and treatment skills.

TYPICAL PHYSICAL/MENTAL DEMANDS
Able to stand, walk, bend, squat, and reach frequently. Possesses physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs. Must be able to see, hear and speak with or without assistive devices. Must be able to lift 50 pounds during patient mobility occasionally and 25 pounds or less frequently utilizing proper body mechanics. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. Proficiency in executive function skills, including planning, organization, focusing attention, flexible thinking/problem solving, memory, self-monitoring, and juggling of multiple tasks.
ESSENTIAL JOB FUNCTIONS
Under valid referral process, carries out an individualized program of physical therapy, including assessment, treatment, planning, implementation, education, and effective communication to maximize a patient’s progress toward achieving functional goals.
Provides direct patient care within scope of practice. Seeks excellence in practice by utilizing evidence-based or best practice techniques.
Has knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services. Is kind and welcoming to all, regardless of gender, ethnicity, race, sexual orientation, socio-economic status, or other factors.
Provides counseling to patients and patients’ families/caregivers about results of testing and recommended treatment options. Involves patients and families in goal setting and treatment decision making process.
Appropriate supervision of Physical Therapy Assistant, with regular and timely communication.
Appropriate use of Rehabilitation Aides.
Continuously monitors patients’ functional needs and progress, adjusting plan of care as needed to ensure optimal patient outcomes.
Clear, concise, and timely documentation including evaluation, plan of care, daily treatments, progress notes and discharge summaries.
Clear and timely documentation of other conversations with or regarding patients (e.g. telephone encounter to document any discussions with patients, providers, vendors, etc.)
Demonstrates integrity through compliance with insurance regulations, accountable billing procedures, judicious use of patient’s resources, and HIPAA rules.
Maintains good efficiency through effective use of downtime that occurs through patient no shows and/or cancellations.
Complies with hospital and departmental policies and procedures, Quality Improvement program, safety, environmental and infection control standards.
Supportive to ancillary staff. This includes clear communication with front office staff and compliance with department procedures for scheduling, cancellations, no-shows, and any other duties primarily associated with the front office.
Encourages innovation by enhancing professional growth and development of self and others through participation in educational programs, in-services, and workshops. Keeps up to date on professional literature.

· Provide physical therapy treatments to patients under the direction and supervision of a licensed Physical Therapist.

Essential Duties and Responsibilities
* Perform speech-language, cognitive-communication, voice, and swallowing evaluations per physician referral and establish individualized plans of care
* Conduct bedside swallow evaluations and assist with instrumental swallow studies (e.g., MBS/FEES) as clinically indicated
* Provide direct patient treatment, including speech/language therapy, cognitive-communication retraining, and dysphagia management
* Document evaluations, treatment sessions, progress notes, and discharge summaries in the EMR in compliance with Title 22, Joint Commission, and payer documentation standards
* Communicate and coordinate care with physicians, nursing staff, occupational and physical therapy, case management, and dietary services
* Participate in interdisciplinary rounds and discharge planning to support safe transitions of care
