About the Role
We are seeking a motivated and compassionate Physical Therapist to join our growing team. In this role, you will treat a diverse orthopedic patient population—many with complex, multi-condition injuries—through both conservative and post-surgical rehabilitation.
Unlike traditional outpatient clinics, our practice is not restricted by typical insurance limitations or visit caps, allowing you the flexibility to deliver thoughtful, individualized care.
Team Environment
You will work alongside:
· Physical therapists, PTAs and PT Aides
· Medical doctors, chiropractors, psychologists, acupuncturists, and massage therapists

Key Responsibilities
· Evaluate and treat patients with orthopedic conditions
· Develop and implement individualized treatment plans
· Provide hands-on therapy and exercise prescription
· Collaborate with other healthcare providers for comprehensive care
· Supervise and delegate to PTAs and PT Aides when appropriate
· Complete thorough and timely documentation using EMR
Qualifications
· Graduate of an accredited PT program (Master’s or Doctorate; new grads welcome)
· Licensed (or eligible) to practice Physical Therapy in Colorado
· Strong clinical, manual therapy, and communication skills
· Ability to work full-time (36-40 hours/week)
· Comfortable working within a team and supervising support staff
· Spanish speaking (preferred but not required)
Compensation & Benefits
· Starting salary: $85,000-$120,000 (based on experience and bonus opportunities)

Provide comprehensive Registered Nurse (RN) care for residents
Perform comprehensive nursing assessments of residents and evaluate changes in condition to ensure appropriate interventions and care planning

General Description: Responsible for providing a wide range of mental health services to families with children that have emotional disturbances requiring intensive intervention using family and group treatment methods.

Bachelor’s degree, or Pharm. D. (preferred), from an approved school of pharmacy
Clear/Current Pennsylvania Pharmacist License. Must not have disciplines on license with PA BOP.
Experience:
Two years experience as a practicing pharmacist is preferred
Retail pharmacy experience is a plus.
Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
Ability to communicate technical information to non-technical personnel.
Knowledge of polices and regulations pertaining to the disbursement of pharmaceutical drugs.
Ability to maintain quality, safety, and/or infection control standards.
Ability to supervise and train assigned staff.
Skill in the use of personal computers and related software applications.
Ability to assume essential responsibilities and duties in the absence of the Pharmacy Manager
Prior knowledge of local, state, and federal accrediting body and OSHA rules and regulations preferred.

*Physical Therapist (PT) Duties and Responsibilities:*
* Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
* Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
* Develop and revises the plan of care in consultation with the physician and other care team members.
* Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
* Preparing clinical progress notes and evaluate outcomes.
* Able to complete clinical documentation within 24 hours of patient contact.
* Supervising therapy assistants and home health aides as appropriate.
* Educate patients and/or families about proper safe use and recommend methods for the installation of adaptive equipment.
*Physical Therapist (PT) Qualifications and Skills:*
* Currently licensed by the Massachusetts Board of Physical Therapy (PT) Examiners.
* Experience in acute care settings or Home Health is preferred but not required.
* Able to maintain strict confidentiality about protected and sensitive information.
* Must be able to solve problems independently and confidently by applying analytical and logical thinking.
* Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
* Able to write reports and correspondence at a professional level.
* Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.

Essential Functions
Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
Recommend modification of plan of care to physician, as needed.
Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).
Include and educate the patient and caregiver(s) regarding treatment plan and goals.
Attend physical therapy staff meetings and in-service presentations.
Participate in case conferences.
Participate in the development and periodic revision of the physician’s Plan of Treatment.
Assures appropriate safety practices and maintenance of equipment.
Other duties as assigned.

Education Requirements
Graduate of an accredited Physical Therapy program.
Qualifications and Requirements
Current, valid Physical Therapy license issued by NYS Department of Education.
One year of post licensing experience as a licensed physical therapist.
Work Environment
The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.
The working conditions are classified as medium work:
Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
