Pay Range:
$30.70 - $49.60Job Description/Major Responsibilities:
- Provides crisis intervention, brief psychotherapy, triage, resource development and disposition recommendations for behavioral health patients of the Emergency Department and Medical Center via both in person and via telehealth as indicated by site.
- Provides direct specialized care with patients who have been identified as needing psychological assessment or interventions beyond the scope of general patient care practice, including assessment of patients’ mental status, psychosocial stressors, level of functioning, risk, support systems, strengths and needs.
- Provide ongoing rounding, counsels patients utilizing solution focused therapy and utilizes problem solving collaboratively with patient to stabilize condition/crisis.
- Uses short-term crisis intervention techniques and/or referrals to other support services to support patients and families dealing with acute emotional distress, addictions, grief, loss, disability and traumatic lifestyle changes.
- Develops discharge and safety plan in direct consultation with patient, family, physician and health care team. Coordinates arrangements to implement plan.
- Facilitates admission process to inpatient psychiatric unit (site specific).
- Ensure patients are pre-certified for recommended level of care.
- Communicates with staff and physicians of the Emergency Department, medical unit, Behavioral Health Services and/or human service agencies to obtain all relevant patient information, refers patients and consults with other staff regarding patient progress to provide optimum patient care.
- Participate in unit meetings, trainings related to departmental changes or enhancements and education.
- Adheres to relevant state and federal guidelines governing patient care and treatment of sensitive information, particularly the Illinois Developmental Disability and Mental Health Code, the Illinois Confidentiality Code, HIPPA and EMTALA.
- Ability to flex hours within the department and cross train to related job roles/duties to meet the needs of the department.
- Ensure accurate data collection for billing, the management information system, and other purposes in accord with organizational policies and procedures.
Licensure:
- LSW, LPC minimum
- LCSW and LCPC strongly encouraged to apply and would be considered at the Level 2 candidacy.
- Candidates without licensure but preparing for licensure may be eligible, but hiring would be contingent upon receiving minimum licensure within an established time at point of hire.
Education/Experience Required:
- Master's Degree in behavioral health sciences or related field with a minimum of two years’ experience in a mental health or related setting.
Knowledge, Skills & Abilities Required:
- Ability to work under stressful conditions and in difficult situations with strong interpersonal skills; knowledge of techniques to de-escalate aggressive behavior. High tolerance for emotionally stressful situations sometimes encountered daily with psychiatric and substance abuse patients
- Risk assessment, psychotherapy and family/group treatment skills, and knowledge of behavioral health referrals and resources.
- Knowledge of DSM-V diagnostic categories.
- Knowledge and education on the typical development related to the lifespan (i.e., normal patterns of adolescent behavior and look for signs or normal aging, prior experience with various age groups is preferred.)
- Basic computer and typing skills; Organization and time management skills.
Physical Requirements & Working Conditions:
- Ability to work under stressful conditions and in difficult situations; May be exposed to hazardous materials and life-threatening diseases; Knowledge of universal precautions a must. Works within and supports a shared governance model. Is customer focused. Transporting patients may be required. May at times come in contact with violent and may be involved in restraining patients.
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

This Full-Time position will have a schedule of Monday - Friday from 8:30AM - 5:30PM. This position will support physical therapy and occupational therapy providers at the STARS Clinic location. This clinic is located at 4424 E Flamingo Ave #120, Nampa, ID 83687.
MINIMUM QUALIFICATIONS:
High School Diploma or equivalent preferred.
Previouscustomer service experiencerequired. Healthcare experiencepreferred. Electronic health record (EHR) experiencea plus. Medical terminologypreferred.
Experience with process improvement or lean philosophy preferred.
ESSENTIAL FUNCTIONS:

Our Pharmacy Interns support pharmacists in delivering safe, accurate, and timely medication therapy to patients. The Pharmacy Intern assists the pharmacist in assuring timely and accurate provision of medications to patients. Routine intern functions are performed under the direct supervision of a pharmacist. These functions may include unit dose dispensing, intravenous admixing, inventory control, record keeping and billing, and the extensive use of equipment and automated technology. Based on ability, experience, and performance, the Pharmacy Intern may perform increasingly clinical activities of the pharmacy, under direct supervision of a pharmacist.

What you will do:

Prepares intravenous admixtures and other sterile products, including chemotherapy by using proper aseptic technique.

Prepares sterile products; properly labels medications and maintains records of packaging as required by department Policy and Procedures and state and federal law
Assists the pharmacist in providing drug information to patients and other health care professionals, advising on proper methods, usage, and cautions associated with medications.
Prepares and restocks drugs for automated dispensing cabinets.
Performs clinical duties under direct supervision of a licensed pharmacist, including medication reconciliation, adverse drug event reviews, renal dosing, medication utilization reviews, patient rounds with physicians, presentations for education purposes, and other clinical duties as directed by preceptor.
Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.

This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.
As a Pharmacist, you will:
Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.
Minimum Qualifications:
Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.

Contingent Hospice RN Case Manager position summary
Our Hospice RN Case Managers provides professional nursing care to the organization’s hospice clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. The RN is fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, is designated as the case manager for hospice patients. The Hospice RN in collaboration with the physician and other Hospice team members anticipates and manages patient’s symptoms, assists in identifying family/caregiver grief/loss issues and assists in implementing the Hospice plan of care. Provides functional support to Hospice Aides and LPN/LVNs as needed.

Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. This position qualifies for a $10,000 sign-on bonus paid out in 4 installments.

What you will do:
Plan and prioritize visits based on client needs and physician orders.
Assess clients at admission and key intervals and document accurately.

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).

3-5 years' experience in adult outpatient oncology, infusion center workflow preferred

Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration
