Advanced Home Health is looking for a Full-Time or per diem Occupational Therapist / OT to join our Home Health Team.
A Home Health Occupational Therapist works closely with the patient care team: RN, LVN, PT, PTA, OT, COTA & HHA.
Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.).
Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs.
Occupational Therapist participates in the development of the total plan of care.
Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other organization professionals.
May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.

The licensed practical nurse (LPN) is responsible for assisting patients in collaboration with the provider and other members of the health care team with all clinical services available at the clinic. Coordinate and manage care of patients seen by provider including but not limited to following up and tracking testing, referrals.
Stays informed of insurance requirements and guidelines for patient care. Performs prior authorizations and acts as patient advocate.
Regularly monitors patients as appropriate to promote wellness and health maintenance including preventative and chronic care.
Participates in the development of the plan of care with other team members and safely implements that plan.
Promotes wellness by providing patient education materials and communicating provider advice/instructions.
Schedules, coordinates and communicates referrals to specialist and services ordered by the provider.

What You’ll Do
Provide evidence-based speech-language therapy in the home and community
Perform evaluations, assessments, and ongoing progress monitoring (SLPs)
Support evaluation and treatment implementation under supervision (CFs)
Develop and implement individualized treatment plans and goals
Deliver interventions for speech, language, communication, and feeding/swallowing disorders
Educate and empower families and caregivers for carryover in daily routines
Collaborate with physicians and interdisciplinary care teams
Document care in an electronic medical record system
Maintain compliance with all clinical and regulatory standards
Qualifications
Required:
Master’s degree in Speech-Language Pathology
Active Colorado SLP license (or eligibility for CFs)
ASHA CCC-SLP or Clinical Fellow pursuing certification
Current CPR certification
Strong communication and interpersonal skills
Preferred:
Pediatric or home health experience
Experience with feeding and swallowing therapy

* Perform treatments on hospitalized patients including the administration of medications by various routes
* Obtain vitals, blood pressure, ECG, and other monitoring parameters
* Monitor hydration status, administering CRI’s, setting up IV fluids with multiple lines, etc.
* Use phlebotomy skills and have a basic knowledge of hematology and “normals”

* Record and deliver organized rounds, discuss cases and communicate additional information as necessary
