Job Description:
This position is responsible for screening, testing, evaluating, diagnosing and treatment of injuries, diseases, and disabilities using physical therapy procedures and modalities in accordance with standard physical therapy practices. In addition, this position is responsible for consulting, educating, and training patients, families, and caregivers and for collaborating with care teams and stakeholders to deliver quality, patient centered care.

This position follows a two-week rotating schedule:
- Week 1: Sunday, Monday, and Friday (30 hours)
- Week 2: Monday, Friday, and Saturday (30 hours)

Essential Functions
- Promotes mission, vision, and values of Intermountain Health, and abides by service standards.
- Competent Services: Provides skilled physical therapy services, staying updated on standard practices for different patient groups. Conducts evaluations and treatments according to professional standards (APTA), considering diagnosis, history, and physician referrals, and develops care plans with specific goals based on patient assessments.
- Productivity Standards: Meets established productivity standards for the department or service line.
- Documentation and Billing: Keeps thorough and timely patient records as required by regulations and facility policies. Completes patient billing accurately and promptly.
- Communication: Communicates effectively with patients, families, physicians, and healthcare providers about patient needs and goals. Ensures smooth operations and optimal use of rehabilitation services.
- Patient Care and Supervision: Manages all aspects of patient care and education, including supervising assistants, aides, office staff, and students to ensure high-quality care.
- Continuing Education: Participates in and promotes continuing education opportunities for self and staff, seeking new learning opportunities according to licensure requirements.
- Quality Improvement: Engages in quality improvement processes, including safe equipment use, improving clinical outcomes, patient access, patient experience and all aspects of the Intermountain Operating Model. Participates in utilization review audits.
- Meetings: Participates in patient care conferences, interdisciplinary meetings, staff meetings, and necessary professional and work groups in collaboration/consensus with leader.
- Program Development and Marketing: Contributes to program development and marketing strategies to grow the physical therapy program and achieve department goals.

Qualifications
- Current Physical Therapist license in states where you work.
- Basic Life Support (BLS) for healthcare providers.
- Basic Computer skills.

- Current driver’s license, reliable transportation, and acceptable driving record.
Physical Requirements
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
- May be expected to stand in a stationary position for an extended period of time.

This is a PRN Acute Ultrasound Tech position available for a competent and independent Sonographer. Our Sonographer will work with the imaging department as part of the team with patient centered expectations. This is a PRN, as needed position, working to cover shifts for sick time and PTO.

Located in Susanville in northeast California, Banner Lassen Medical Center offers comprehensive care in a state-of-the-art facility to aid in the prevention, diagnosis, and treatment of illnesses. Our 25-bed medical center has a proud history of providing vital caring to the community since 1883. Though we are a small community hospital, we have invested in the latest medical equipment, including a 124-slice CT scanner, nuclear medicine, MRI, ultrasound, bone density and mammography. The hospital also offers infusion, respiratory therapy and cardiopulmonary services as well as laboratory services, inpatient and outpatient surgery and a 24-hour emergency department. Away from work, you'll find that our location at the foot of the scenic Sierra Nevada Mountains offers an unlimited variety of recreational activities, including fishing, camping, golfing, hiking and skiing.POSITION SUMMARY
This position performs procedures by following and verifying referring providers’ orders. Uses independent judgment and applies learned methodologies according to established policy and procedures for one Ultrasound certification.
CORE FUNCTIONS
1. Performs all exams by following and verifying referring providers’ orders. Adheres to established protocols and provides all pertinent patient information utilized by Radiologist for complete dictation. Uses independent judgement and applies learned methodologies according to established policy and procedures for all ultrasound exams. Scanning competencies should include, at a minimum, all patient ages, acuity levels and classes, independently and without quality issues. Performs ultrasound guided procedures and works closely with radiologist or provider.
2. Educates patients and families about procedure and/or treatment to be performed. Demonstrates professional behavior and conduct in all interactions. Fosters teamwork, ensures efficient use of resources and advocates for quality patient outcomes.
3. Performs and evaluates equipment quality control measurements. Performs troubleshooting as needed and notifies appropriate personnel to maintain equipment performance. Maintains a safe environment for patients, personnel, and visitors. Adheres to safety policies. Ensures imaging areas and equipment are in a constant state of "Ready".
4. Demonstrates competence in performing prescribed invasive/interventional procedures if applicable and in accordance with established policies. Maintains proficiency and competencies signed off for all studies related to certification(s). Performs high quality imaging and in accordance with training received.
5. Accountable for the ethical, legal, professional and certification responsibilities related to the practice. Maintains confidentiality of all work information.
6. Assures the efficient operation of workflow in the department through patient triage and prioritization. Monitors and maintains an adequate inventory of supplies and material to ensure uninterrupted services.
7. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, patients, and family members.
MINIMUM QUALIFICATIONS
Requires national certification in one area of specialty from the American Registry of Diagnostic Medical Sonographers (ARDMS), Cardiovascular Credentialing International (CCI) or American Registry of Radiologic Technologists (ARRT). Uncertified new hires who are Registry eligible must obtain certification within 12 months of hire and image with an experienced technologist until they become fully proficient. BLS certification required.
Completion of an approved Ultrasound program.
This position requires clinical knowledge typically achieved by 0-3 years of experience.
Must demonstrate effective communication skills, human relations skills, ability to analyze data and solve problems.
PREFERRED QUALIFICATIONS
Additional related education and/or experience preferred.
Estimated Pay Range:
$40.91 - $68.19 / hour

Position Summary
The Respiratory Care Practitioner is responsible for the delivery of skilled respiratory care modalities and interventions to designated patient populations with cardiopulmonary system deficiencies and abnormalities, which requires the use of advanced practitioner skills to intervene in complex patient care situations. This position requires specialized training and experience to perform independent, professional respiratory care, ventilator management, diagnostic and therapeutic procedures, and clinical evaluation of care, including the establishment of therapy and treatment objectives, and the review of care based on the age and needs of the patient. This position operates independently, under limited supervision. Internal contacts include patient care staff and management. External contacts include patients, families, physicians, outside health care providers, and other organizations.
Requirements
- Graduate of AMA approved Respiratory Therapy Training program
- Registered (RRT) by the National Board of Respiratory Care
- Licensed as a Respiratory Care Practitioner (RCP) with the State of Washington
- Registry eligibility notice to sit for RRT exam from NBRC required
- Advanced cardiac life support credential required within 90 days of hire depending on age of the population served: ACLS, NRP, PALS
- Health Care Provider level CPR certification
- Minimum one (1) year of acute care experience in the field of respiratory therapy and critical care as CRT preferred
Consideration may be given to internal candidates not meeting the minimum education qualifications.

Works with an established multidisciplinary team of professionals to provide quality nutrition education services including all department services. Also provides support the outpatient diabetes education services as needed.
Job Description:

CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral or BUMG, BUMCT in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. An Arizona Criminal History Affidavit must be signed upon hire. - Processes all orders/requests accurately and efficiently, verifying appropriateness of all drugs in reference to the patient’s diagnosis, allergies, height and weight. Exercises good judgment in patient care decisions per performance standard and documents patient care activities and patient outcomes impacted by pharmaceutical care and clinical interventions.
- Provides patient-specific and non-patient specific drug information support regularly.
- Serves as a member on one or more Medical Staff Committees and actively contributes to accomplishing the goals of the committee.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Collaborates with other health care professionals to manage patient’s drug therapy for maximal effectiveness while minimizing side effects, adverse drug reactions, toxicities, drug interactions, and incompatibilities using established internal and external guidelines.
- Provides pharmaceutical care at the fundamental to advanced level for patients within the designated practice area(s). Activities include, but are not limited to, proactive drug therapy reviews and monitoring, patient counseling, transitions of care management including medication acquisition process support, and management of pharmacist-based dosing protocols.
- Participates in quality management initiatives such as, but not limited to: medication use evaluations, adverse drug reaction and medication error reporting, clinical interventions, medication center reviews, and inventory management documentation and reporting.
FINANCE ESSENTIAL FUNCTIONS
- Contributes to the provision of pharmacoeconomically sound medication therapy by upholding HM Formulary standards in area(s) of responsibility and avoiding medication-related complications.

Job Description:
The Registered Dietitian functions within a multidisciplinary team to provide nutrition therapy for inpatients and/or outpatients at UofL Health facilities per scope of assignment. The dietitian will provide safe, effective, and evidence-based nutrition care and services in accordance with guidelines from the Academy of Nutrition and Dietetics, the American Society for Parenteral and Enteral Nutrition, and other industry principles. The dietitian supports quality clinical outcome practice, and development and growth of individual and team nutrition therapy skills. Responsibilities include meeting objectives for department, facility, and organizational goals; interacting with professional staff, patients, and families; providing comprehensive nutrition therapy and guidance within licensed area of expertise and responsibility; providing quality learning experiences for nutrition students/interns as applicable; and maintaining professional competency and skills required for professional practice.
Essential Functions:
- Provides medical nutrition therapy using the nutrition care process, including nutrition screening, assessment, diagnosis, intervention, monitoring, and education according to established departmental policies and standards of practice
- Conducts malnutrition identification and assessment including nutrition-focused physical exam where applicable
- Is fiscally responsible by adhering to departmental productivity standards while maintaining quality services, and monitoring appropriateness of product and mode of nutrition support
- Communicates and interacts effectively with patients and families, community members, and health professionals, in accordance with evidence-based nutrition practice and customer service skills
- Serves as a nutrition resource to other departments, maintaining effective working relationships with members of the medical staff, other healthcare professionals, and affiliates; participates in multi-disciplinary committees and/or activities as directed
- Improves practice based on quality evaluation, and assists in data collection/analysis and research as directed

Education:

Job Description:
The Ultrasound Tech performs a variety of ultrasound examinations to assist physicians in diagnosing medical conditions and monitoring patient health.
Requirements:
Associate's degree in Allied Health education program (diagnostic medical sonography, radiologic technology, respiratory therapy, occupational therapy, physical therapy, nursing) and 12 months of full-time clinical ultrasound/vascular experience; or 2. Diploma or degree from an ultrasound/vascular program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Canadian Medical Association (CMA); or 3. Bachelor's degree (any major) and 12 months of full-time clinical ultrasound/vascular experience; or 4. Bachelor's degree in sonography or vascular technology; or higher.
Registered or registry eligible by the American Registry of Medical Sonographers (ARDMS).
Completion of ARDMS Registry within six months of eligibility required.
BLS preferred upon hire, required within 90 days of hire.
Preferred Qualifications:
Prior experience working as an Ultrasound Technologist in a hospital or medical imaging facility preferred.
