The Clinical pharmacist is responsible for ensuring the safe, appropriate, and cost-effective use of medications. He/she provides expertise on drug therapy initiation and monitoring to all customer groups served by MercyOne North Iowa Medical Center, working collaboratively with other health care professionals to devise the most appropriate drug treatment for patients. The role of the clinical pharmacist sometimes extends outside the hospital, sharing their specialized knowledge with other hospitals, clinics, and care settings.
POSITION QUALIFICATIONS
Education:
1. Completion of the requirements for a Pharm.D. required.
2. Iowa Pharmacist licensure, either by examination or reciprocity, is required
B. Experience
1. Demonstrated competence in pharmacy for 2-3 years, focusing on the provision of pharmaceutical care is required.
2. Residency training and/or certification in area related to practice site is preferred.
3. Demonstrated competence in previous clinical practice is preferred.

Works under the supervision of a licensed Respiratory Care Practitioner and performs various general respiratory care duties to include respiratory care treatments, oxygen administration, suctioning, and arterial blood gas puncture and analysis. Demonstrates the knowledge and skills necessary to provide care or arrange for the provision of care for neonatal to geriatric patients.
Essential Functions
- Demonstrates knowledge of principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's status.
- Interprets the appropriate information needed to identify each patient's requirements and ensures and documents that age specific needs are being met as described in the department policies and procedures. Provides and assesses effectiveness of patient and family education.
- Performs respiratory care treatments and oxygen administration.
- Suctions patients and obtains and runs ABG samples.
Physical Requirements
Work requires frequent lifting, carrying, pushing, and pulling objects. Must be able to lift up to 25 pounds. Maybe exposed to hazardous work environment this includes contagious patients and biohazardous materials. Rapid coordination of eyes and hands to fingers. Sufficient noise and disruptions to cause distractions and stress. Hazardous working conditions in which individual is exposed to definite risk of bodily injury through contact with moving instrumentation, chemicals, or body fluids.
Education, Experience and Certifications
Currently enrolled in good standing in an accredited Respiratory Care educational program approved by the Committee on Accreditation for Respiratory Care (CoARC) and successful completion 1 year of such program required. Registered with the North Carolina Respiratory Care Board (NCRCB) as an RCA required. Completed competency evaluations for each of the duties of the job required. BLS required per policy guidelines. Additional certifications per facility/policy standards.

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.

POSITION SUMMARY
This position is responsible for the administration of physical therapy treatment under the supervision of a Physical Therapist, in accordance with physician referral, state and local organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS
1. Administers physical therapy treatment according to the plan of care established by the Physical Therapist. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner. Monitors ongoing effectiveness of treatment and collaborates with Physical Therapist in modifying the plan of care when indicated. Identifies and prioritizes key limiting factors. Participates in discharge planning. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
2. Demonstrates understanding of scope of practice. Documents communication with supervising Physical Therapist. Communicates any significant change in the patient's physical function, behavior, and/or tolerance to treatment to the supervising Physical Therapist. Makes recommendations to Physical Therapist on additions to or modifications of referring orders. Provides input to the Physical Therapist to modify/update treatment goals so they are functional, measurable, patient related and reflect key limiting factors. Refers patient when treatment requires skills of Physical Therapist.
3. Documents according to standards set by the department and regulatory agencies. Completes documentation within the established time frames of the department. Documents innovative and individualized treatment techniques/plan. Documentation reflects comprehensive assessment of patient's progress or lack of progress. Treatment goals are reassessed according to department policies.
4. Communicates pertinent patient information to ensure the best possible follow through of care. Communicates treatment results, and patient progress to physicians, nurses, patient, family/caregiver and the interdisciplinary team. Communicates in a clear and timely manner. Defends treatment rationale with standard practices and/or current literature.
5. Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing shifts. Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs.
6. Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality cost effective patient care. Supervision and delegation of the Therapy Technician and PTA student complies with department policies and regulatory agencies. Participates in the training and orientation of new personnel. Assures competency of personnel, prior to delegation of task.
7. Department operational activities. Attends required department staff meetings and/or follows up to obtain pertinent information. Participates in department/facility process improvement. Meets productivity standards set by the department.
8. This position has responsibility for interacting with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community providers and agencies concerning the health care of the patient. Also interacts with the physician offices in order to report, requires or clarify information.
MINIMUM QUALIFICATIONS
Must have graduated from Physical Therapy Assistant program accredited by APTA, or equivalent.
Must possess a current Physical Therapy Assistant license from the state seeking practice if required. BLS certification required.
Relates throughout the interview process their clinical competency, experience, training and education they have received that is required to perform the job.

- Performs, or assists in simulations and tumor localization using specified imaging devices including, but not limited to CT, MRI, and PET. Perform, or assists in the planning and implementation of custom shields, wedges, and other beam modifying devices.

- Provides assistance and technical support to the Medical Physicist, in radiation safety and protection, qualitative machine calibrations, quality assurance of treatment plans and radiation oncology equipment.

Inpatient Clinical Nutrition
Within our 37-bed community hospital, you will:
- Conduct comprehensive nutrition assessments and reassessments
- Utilize the Nutrition Care Process (NCP)
- Diagnose and monitor malnutrition

- Provide individualized nutrition education to patients and families

Qualifications (Registered Dietitian Clinical Specialist)
- Registered Dietitian with the Commission on Dietetic Registration.
- For graduates after Jan 1, 2024, completion of a minimum of a master’s degree by an accredited university. Education is verified.
- Completion of an ACEND accredited Didactic Program in Dietetics, Dietetic Internship, Coordinated Program, or graduate-level competency-based dietitian nutritionist program.
- State licensure or certification in accordance with the primary state of practice requirement for Registered Dietitians. If the primary state of practice requires licensure or certification, this must be obtained within 6 months of hire.
- Demonstrated ability to apply the Nutrition Care Process in multiple patient populations and/or disease types.

POSITION SUMMARY
This position is responsible for the administration of occupational therapy treatment under the supervision of the Occupational Therapist, in accordance with physician referral, state and local organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS
1. Administers occupational therapy treatment according to the plan of care established by the Occupational Therapist. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner. Monitors ongoing effectiveness of treatment and collaborates with Occupational Therapist in modifying the plan of care when indicated. Identifies and prioritizes key limiting factors. Participates in discharge planning. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
2. Demonstrates understanding of scope of practice. Documents communication with supervising Occupational Therapist. Communicates any significant change in the patient's physical function, behavior, and/or tolerance to treatment to the supervising Occupational Therapist. Makes recommendations to Occupational Therapist on additions to or modifications of referring orders. Appropriately collaborates with the Occupational Therapist to modify/update treatment goals so they are functional, measurable, patient related and reflect key limiting factors. Refers patient when treatment requires skills of Occupational Therapist
3. Documents according to standards set by the department and regulatory agencies. Documents legibly. Completes documentation within the established time frames of the department. Documents innovative and individualized treatment techniques/plan. Documentation reflects comprehensive assessment of patient's progress or lack of progress
4. Communicates pertinent patient information to ensure the best possible follow through of care. Communicates treatment results and patient progress to physicians, nurses, patient, family/caregiver and the interdisciplinary team. Communicates in a clear and timely manner. Defends treatment rationale with standard practices and/or current literature.
5. Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing shifts. Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs
6. Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality cost effective patient care. Supervision and delegation of the Therapy Technician and COTA student complies with department policies and regulatory agencies. Participates in the training and orientation of new personnel. Assures competency of personnel, prior to delegation of task.
7. Department operational Activities. Follow up to obtain pertinent information. Participates in department/facility process improvement. Meets productivity standards set by the department
8.This position has responsibility for interacting with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community providers and agencies concerning the health care of the patient. Also interacts with the physician offices in order to report, request or clarify information
MINIMUM QUALIFICATIONS
Must have graduated from an Occupational Therapy Assistant program accredited by the AOTA, or equivalent.
Must possess a current Occupational Therapy Assistant license from the State of practice. BLS certification required.

This position is responsible for the evaluation, planning, administration and supervision of Occupational Therapy treatment in accordance with physician referral, state and local organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.

1. Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals.. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflect the key limiting factors. Collaborates with patient and family/caregiver when setting goal Initiates discharge planning. Recommends additions to or modifications of referring orders.
2. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner.. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
3. Documents according to standards set by the department and regulatory agencies. Documents legibly. Completes documentation within the established time frames of the department. Documents innovative and individualized treatment techniques/plan. Documentation reflects comprehensive assessment of patient's progress or lack of progress. Treatment goals are reassessed according to department policies
4. Communicates pertinent patient information to ensure the best possible follow through of care. MINIMUM QUALIFICATIONS
